EE WAS DRIVING MANTRIP TO ONE OF THE PRODUCTION SECTIONS WHEN A PIECE OF DRAW ROCK FELL FROM BETWEEN ROOFBOLTS STRIKING HIM IN THE HEAD, NECK AND SHOULDER. EE STARTED MISSING WORK ON 7-30-01 D UE TO ACCIDENT.

HE WAS WORKING ON SHUTTLE CAR CABLE REEL - TOLD CAR OPERATOR TO TURN CAR ON AND PICK UP SLACK CABLE. NOT REALIZING WHERE HE WAS STANDING, THE CABLE SLAPPED UP AND KNOCKED HIM DOWN.
EE WAS LOADING PIN PLATES ONTO HIS ROOF BOLTING MACHINE WHEN HE PULLED SOMETHING IN HIS LEFT SHOULDER.
EE WAS WALKING TO GET SOME GREASE WHEN HE LOST HIS FOOTING AND TWISTED HIS KNEE.

HE WAS HELPING SET AN ARCH. TRIPPED AND FELL FACE FIRST ONTO THE MINE FLOOR. HIS HIS HEAD WHEN HE FELL.
