OUR BRAKE CAR DERAILED IN THE MINE SLOPE ON IT'S WAY INTO THE MINE. EMPLOYEES WERE ON THE CAR AT THE TIME OF DERAILMENT BUT NO INJURIES TO BE REPORTED. THE BRAKES SET DURING DERAILMENT AND THE CAR WAS DOWN FOR APPROXIMATELY ONE HOUR TO RELEASE BRAKES AND PUT CAR BACK ONTO THE TRACK.

EE WAS CARRYING A BUCKET OF CHAINS WHEN HE STEPPED ON A ROCK AND LOST HIS BALANCE, AT THIS TIME HE FELT A POP IN HIS LOWER BACK AND LATER NUMBNESS IN HIS LEG.
EE WAS WALING TO THE BACK OF HIS ROOF BOLTING MACHINE WHEN A ROCK FELL FROM THE ROOF MEASURING 4' X 5' X 3"" THICK STRUCK HIM ON HIS RIGHT SHOULDER AND RIGHT ARM.
EMPLOYEE WAS TAKING A ROPE THAT WAS HOLDING THE MINER CABLE AND WATER HOSE FROM THE MINER WHEN HE STRAINED HIS RIGHT SHOULDER.
EMPLOYEE WAS IN THE PROCESS OF INSTALLING A 10' ROOF BOLT WHEN HE FELT A PAIN IN HIS CHEST AND NECK AS HE WAS REACHING. HE MISSED THREE DAYS. DOCTORS WERE CHECKING HIM FOR POSSIBLE HEART PROBL EMS. NOW IT HAS BEEN DETERMINED TO BE A PINCHED NERVE AND WORK-RELATED. FIRST DAY MISSED WAS 12/7/02.
EE WAS IN THE PROCESS OF HANGING A MINER CABLE WHEN HE FELT A POP IN HIS RIGHT SHOULDER. THE EE HAS TO UNDERGO SURGERY FOR THIS ACCIDENT DUE TO THE DETERMINATION THAT HE ACTUALLY HAS A ROTATOR CUFF TEAR.
EMPLOYEE WAS STANDING ON A LADDER TRYING TO PLACE A SCREEN ONTO THE FLOOR IN THE PREP PLANT WHEN HE LOST HIS BALANCE AND FELL CAUSING HIM TO INJURE HIS KNEE BY BRUISING AND TWISTING IT.
EMPLOYEE WAS IN THE PROCESS OF WORKING ON A CONTINUOUS MINER WHEN A PIECE OF DRAW ROCK FELL FROM THE ROOF AND STRUCK THE EMPLOYEE. THE RESULT WAS SORENESS AND A CONTUSION.
EE WAS INSTALLING A TRANSMISSION INTO A 4 MAN RIDE WHEN HE FELT A PAIN IN HIS SHOULDER. THE EE'S FIRST DAY AWAY FROM WORK WAS 09-20-02. HE WILL BE UNDERGOING PHYSICAL THERAPY FOR A PARTIAL TEA R TO HIS ROTATOR CUFF.
EE IN THE PROCESS OF INSTALLING A 2PIECE ROOF BOLT WHEN HE WENT TO COUPLE THE BOLTS TOGETHER HE USED THE ROTATION ON THE BOLTER & HIS GLOVE CAUGHT BETWEEN THE COUPLER & THE BELT CAUSING HIS HA ND TO BE WRAPPED AROUND THE BOLT & IT RESULTED IA TORN LIGAMENT IN HIS RIGHT ARM.
EE WAS INT HE PROCESS OF LOADING A BUNDLE OF ROOF BOLTS ONTO A ROOF BOLTING MACHINE WHEN HE FELT A POP IN HIS SHOULDER EE STARTED MISSING WORK 9-27-02 AND WAS DETERMINED BY A MEDICAL PROFESSIO NAL 11-14-02 THAT IT WAS A WORK RELATED INJURY AND NOT THE RESULT OF A NON WORK RELATED INJURY.
EMPLOYEE WAS IN THE PROCESS OF LIFTING A MINER CABLE AND WATER HOSE WHEN THE CABLE FELL BACK TOWARD THE EMPLOYEE AND CAUSED HIM TO STRAIN HIS BACK.
EE WAS TRYING TO LINE UP A PIN INTO A DUSTER TONGUE HOOK UP WHEN THE EE WHO WAS HELPING HIM PULLED THE SUPPLY TRACTOR UP CAUSING IT TO JUMP AND PINCH THE EE'S HAND BETWEEN THE JEEP AND THE PIN .
EMPLOYEE WAS IN THE PROCESS OF DISCONNECTING A FULLY CHARGED SET OF SCOOP BATTERIES WHEN THE DISCONNECTING PLUG LEADS SHORTED TOGETHER CAUSING THE BATTERIES TO SHORT OUT AND RESULTED IN 2ND DE GREE BURNS TO THE LOWER HALF OF HIS BODY.
EMPLOYEE WAS THROWING CONCRETE BLOCKS ONTO A OVERCAST WHICH CAUSED A STRAIN IN HIS LEFT FOREARM AND ELBOW AREAS.
EE WAS IN THE PROCESS OF WALKING BESIDE OF A 270 BATTERY TRACTOR THAT WAS HUNG UP IN AN UNDERPASS WHEN A RIB ROLLED OFF THAT MEASURED 8.5'X6.5'X0""-12"" THICK AND STRUCK HIM IN THE SIDE.
EMPLOYEE WAS WALKING FROM A BELT DRIVE TO HIS MAINTENANCE RIDE WHEN HE STEPPED OFF OF A 1 FT LEDGE AND TWISTED HIS KNEE (LEFT KNEE).
EE WAS CAUGHT BETWEEN A SET OF AIRLOCK EQUIPMENT DOORS AS HE ATTEMPTED TO CLOSE THEM. HE CONTINUED TO WORK WITH NO RESTRICTIONS NOR DAYS MISSED UNTIL HE WENT TO A DR. DUE TO CONTINUOUS PAIN IN HIS SHOULDER, A TEAR TO HIS ROTATOR CUFF WAS FOUND AND HE UNDERWENT SURGERY 9-19-02. THIS WAS 1ST DAY AWAY FROM WORK.
EE WAS IN THE PROCESS OF CARRYING A TOP ROLLER TO INSTALL ON BELT STRUCTURE WHEN HE STEPPED ON A 6' X 6' WOODEN POST AND LOST HIS BALANCE WHICH CAUSED HIM TO PULL A MUSCLE IN HIS BACK.
EE WAS WALKING & CARRYING A PUMP CONTROL BOX WHEN HE SLIPPED & FELL CAUSING HIM TO FRACTURE HIS WRIST.
EE WAS OPERATING A ROOF BOLTING MACHINE WHEN A PIECE OF DRAWROCK FELL FROMT HE ROOF AND STRUCK HIM IN THE BACK.
EE WAS LOADING ROOF BOLTING MATERIALS INTO A SCOOP BUCKET WHEN HE FELT A BURNING PAIN IN HIS LOWER BACK.
EE WAS IN THE PROCESS OF LOADING MUDDY VENTILATION CURTAIN INTO A SCOOP BUCKET WHEN HE PULLED A MUSCLE IN HIS BACK.
EE WAS WALKING IN THE BELT ENTRY AND TWISTED HISRIGHT KNEE. THE FIRST DAY MISSED DUE TO THIS ACCIDENT WAS 3-4-02.
EE WAS IN THE PROCESS OF MOVING THE MINER CABLE OVER FROM AWAY FROM THE CONTINUOUS MINER BY USING A PRY BAR, WHEN THE BAR SLIPPED AND HIT THE EE IN THE MOUTH DISLODING A PERMANENT TOOTH.
EE WAS IN THE PROCESS OF INSTALLING A MANIFOLD ON A 6"" WATERLINE WHEN HE KICKED IT WITH HIS RIGHT FOOT AND FELT PAIN.
EE WAS INSTALLING THE BOTTOM SECTION OF A 6' TWO PIECE BOLT WHILE APPLYING BOOM PRESSURE HE CAUGHT HIS HAND BETWEEN THE TWO, CAUSING A LACERATION REQUIRING SUTURES.
EMPLOYEE WAS IN THE PROCESS OF RAISING THE RUBBER TIRES OF A SUPPLY CAR TRAILER WHEN HIS SHOULDER POPPED AND HE THEN EXPERIENCED PAIN IN HIS SHOULDER. EMPLOYEE IS UNDERGOING PHYSICAL THERAPY.
EMPLOYEE WAS STANDING BESIDE A FLETCHER ROOF BOLTING MACHINE WHEN A PIECE OF ROCK FELL FROM THE ROOF AND STRUCK HER LEFT HAND INJURYING 2 OF HER FINGERS.
EE WAS TRYING TO ASSIST A ROOF OBLTER OPERATOR IN GETTING HIS ROOF BOLTER UNSTOPPED, HE LAID HIS HAND OVER THE CHUCK ON THE DRILL POT WHEN A PIECE OF THE SECTIONAL DRILL STEEL FELL FROM THE RO OF AND STRUCK HIS HAND.
EMPLOYEE WAS IN THE PROCESS OF INSTALLING A TOP ROLLER IN SOME BELT STRUCTURE WHEN HIS FINGER WAS CAUGHT BETWEEN THE ROLLER AND THE STRUCTURE, THIS INJURY WAS A LACERATION THAT NEEDED SUTURE S.
EE WAS IN THE PROCESS OF TAKING A CAT PAD OUT OFA CONTINUOUS MINER WHEN THE WEDGE BLOCKING MATERIAL CAME OUT AND HIS HAND WAS SMASHED BETWEEN THE CAT PAD AND THE PAN OF THE MINER. WHICH RESULT ED IN A HAIRLINE FRACTURE TO HIS FINGER.
EE WAS INSTALLING A ROOF BOLT WHEN A PIECE OF DRAW ROCK FELL & STRUCK HIM IN THE NOSE. THIS INJURY REQUIRED THE EE TO RECEIVE SUTURES TO A LACERATION ON HIS NOSE.
EE WAS WALKING IN THE BELT ENTRY BESIDE THE 1 D BELT WHEN HE STRUCK HIS HEAD ON A PINBOARD AND JAMMED HIS NECK.
EE WAS STANDING BESIDE OF HIS ROOF BOLTING MACHINE WHEN A PIECE OF ROOF AND RIB ROLLED OFF AND STRUCK HIS BACK AND FOOT CAUSING SORENESS. EE WAS AT WORK HIS NEXT SHIFT AND 7 DAYS LATER WENT TO THE DR. AND IT WAS DETERMINED HIS FOOT WAS BOTHERING HIM DUE TO GOUT.
