The slope car was outside the slope and in the process of being jogged back to the charger and in so doing the slope car was jogged back too far causing the hoist to go into overtravel. The hoist was down 45 minutes.

THE SLOPE CAR WAS IN THE PROCESS OF COMING OUT OF THE MINE WHEN IT LOST POWER AND THE BRAKES SET. IT WAS APPROXIMATELY 500' FROM THE BOTTOM WITH AN EMPLOYEE IN IT. THE BATTERIES ON THE SLOPE CAR WERE CHANGED ALONG WITH AN ELECTRICAL COIL. THE CAR WAS BACK IN SEVICE AT 7:50 A.M.

THE HOIST USED FOR SUPPLYING & TRANSPORTING MEN IN & OUT OF THE MINE WAS DOWN FROM 1005 TO 1200 SATURDAY 3-13-04 DUE TO A BAD PLC. THE HOIST WAS DOWN FOR A TOTAL OF ONE HOUR & FIFTY FIVE MINUTES.

Employee was installing a 10 ft 2 piece roof bolt in the roof. He was holding the coupler with his right hand and using the rotation to tighten the two bolts together. His glove got caught in the coupler causing his right arm to be pulled toward the bolter. He receieved a severe right shoulder dislocation.

Fall onto or against objects

Ee stepped across into a battery scoop bucket, his right foot slipped and he hyper extended his right knee. he sought medical attention and the mri showed a torn acl and torn meniscus cartialge. he is going to have surgery to repair his right knee.

Over-exertion in lifting objects

EE bent over to pick up a concrete block for the brattice he was building. He felt pain in his lower back. He continued to work up until 10-18-04. He had surgery to repair a disc in his back on 10-19-04.

Fall from machine

EMPLOYEE WAS IN THE PROCESS OF HELPING ANCHOR A BELT TAIL PIECE. HE WAS STANDING ON THE TAIL PIECE HOLDING A JACK PIPE WHEN THE JACK PIPE SLIPPED. HE LOST HIS BALANCE AND FELL OFF THE BELT TAI L. HE TWISTED HIS RIGHT KNEE WHEN HE HIT THE GROUND. EMPLOYEE CONTINUED TO WORK UNTIL MARCH 9TH. FIRST DAY MISSED DUE TO INJURY 3-10-04. KNEE SPRAIN.

EE WAS IN THE PROCESS OF GOING THROUGH A MAN DOOR WEHN HE FELT HIS RIGHT KNEE POP. HE HAS HAD PRIOR KNEE SURGERY TO THE RIGHT KNEE. HE CONTINUED TO WORK UP UNTIL APRIL 13, 2004. HE HAD SURGERY ON APRIL 14, 2004 TO REPAIR A TORN MENISCUS AND REPAIR A ACL DEFICIENCY.

Over-exertion in lifting objects

EMPLOYEE WAS LIFTING A 2' X 4' PIECE OF METAL INTO A MANTRIP WHEN HE FELT PAIN IN HIS LOWER BACK. EMPLOYEE CONTINUED TO WORK UNTIL 2-24-04. HIS FIRST SHIFT MISSED WAS 2-25-04. HE IS UNDERGOING PHYSICAL THERAPY TO TREAT HIS BACK STRAIN.

Struck by falling object

EMPLOYEE WAS INSTALLING ROOF BOLTS ON UNIT #1 AT ENTRY #3 IN THE FACE. SOME ROCKS HAD DROPPED OUT IN THE FACE AREA. AS INJURED WAS INSTALLING A ROOF BOLT A ROCK FELL OUT BETWEEN THE RIB AND RI B BOLT. HE WAS STRUCK ON THE NECK, BACK, AND RIGHT LEG. CONTUSION TYPE INJURY. ROCK MEASURED 4' X 18"" X 2"".

Over-exertion (Not Elsewhere Classified)

EE WAS STEPPING OFF OF A PLATFORM ON THE ROOF BOLTER AND TWISTED HIS LEFT KNEE WHEN HE STEPPED DOWN. HE WORKED UP UNTIL 2/17/04. HIS FIRST MISSED WORK DAY WAS 2/18/04. HE HAS A KNEE STRAIN AND IS SCHEDULED FOR AN MRI.

Over-exertion (Not Elsewhere Classified)

EMPLOYEE IS A SHUTTLE CAR OPERATOR. HE GOT OFF HIS CAR TO HANG UP CONTINUOUS MINER CABLE. WHEN HE BENT OVER TO PICK UP THE CABLE, HE FELT A SHARP PAIN IN HIS STOMACH AND GROIN AREA. HE CONTINU ED TO WORK UNTIL 2-3-04. HIS LAST SHIFT WORKED WAS 2-2-04. HE IS TO HAVE SURGERY TO REPAIR THE HERNIA.

Struck by powered moving object

EE WAS IN THE PROCESS OF SPLICING A ROOF BOLTER CABLE. HE WAS SITTING ON THE MINE FLOOR WITH HIS BACK TO THE FACE AREA. A SCOOP TRAVELED DOWN THE ENTRY HE WAS SITTING IN AND STRUCK THE VICTIM IN THE BACK. HE RECIEVED CONTUSION TO HIS BACK AND SORENESS IN HIS LEGS.

Fall to the walkway or working surface

Ee twisted his left ankle in the slope track while placing a small part in the slope car seating compartment. When his ankle turned over he fell and as a result of the fall he received a compound fracture to his lower left leg. He had surgery to repair the fracture on 7/28/04.

Struck by flying object

EE WAS AT LANDING AT THE ROTARY BREAKER. A CABLE WAS BEING USED TO HOIST A DRIVE CHAIN TO THE LANDING. EE REACHED TO PULL THE CABLE TOWARD HIM AND THE CABLE BROKE. THE CABLE STRUCK HIM ON THE LEFT HAND. HE RECEIVED A FRACTURE TO THE BASE OF HIS LEFT LITTLE FINGER. A FINGER SPLINT WAS PUT ON FINGER.

Struck by falling object

Employee was drilling a hole to install a roof bolt. ashe was drilling a rock 1'x1'x1"" fell and struck his drill steel. the rock then struck him on the left upper part of his top lip. he recieved a laceration/puncture wound injury. 12 sutures required to close the wound.

Employee had his left hand on the canopy post. He and other ee were looking for an oil leak. Injured had other ee raise up the drill pot and ee had his left finger pinched between the canopy and the mine roof. 10 sutures to close the laceration to his left middle finger.

Employee got right hand pinched between the head and pan of the continuous miner. he recieved several sutures to close the laceration and returned to work.

EMPLOYEE WAS CUTTING OPEN ROCK DUST BAGS WITH HIS SUPERVISOR WHEN HE WAS CUT WITH A RAZOR KNIFE ON HIS RIGHT INDEX FINGER. SIX SUTURES WERE REQUIRED TO CLOSE THE LACERATION.

EMPLOYEE WAS WALKING BESIDE HIS ROOF BOLTING MACHINE & A METAL BAND USED TO HOLD BUNDLES OF ROOF BOLTS TOGETHER WAS STICKING OUT & HE STRUCK IT WITH HIS LEFT ARM. THREE SUTURES WERE REQUIRED TO CLOSE THE LACERATION.

Struck against stationary object

EMPLOYEE WAS SECURING A BUNDLE OF ROOF FBOLTS ON A SUPPLY CAR. HE LEANED OVER THE BUNDLE AND A METAL BAND THAT HOLDS THE ROOF BOLTS TOGETHER CUT HIS LOWER STOMACH. HE RECEIVED A LACERATION THAT REQUIRED 5 SUTURES TO CLOSE THE WOUND. HE RETURNED TO WORK THE NEXT SHIFT.

EMPLOYEE WAS DUMPING OFF MATERIAL FROM BREAKER FEED BELT MAGNET. A PIECE OF METAL WAS CAUGHT ON THE MAGNET AND AS HE GOT THE METAL LOOSE AN UNKNOWN OBJECT ON THE BELT STRUCK HIS LEFT LITTLE FINGER. HE RECEIVED A LACERATION TO THE LEFT LITTLE FINGERS. 5 SUTURES WERE REQUIRED TO CLOSE THE WOUND & HE RETURNED TO WORK.

EMPLOYEE HAD FINISHED ROOF BOLTING & WAS PULLING BACK ROOF BOLTER CABLE WHEN A ROCK APPROX. 3'X 3'X 2"" FELL OUT BETWEEN THE RIB & RIB PIN & STRUCK HIM ON THE LOWER BACK. HE RECEIVED A PUNCTURE WOUND TO HIS LOWER BACK AND 5 SUTURES WERE REQUIRED TO CLOSE THE WOUND.

EMPLOYEE WAS REMOVING A BOLT WITH AN IMPACT WRENCH USING AN UNIVERSAL SWIVEL JOINT BETWEEN THE WRENCH AND THE SOCKET. THE SWIVEL BROKE AND HE RECEIVED A LACERATION TO HIS LEFT INDEX FINGER. 7 SUTURES WERE REQUIRED TO CLOSE THE LACERATION AND HE RETURNED TO FINISH OUT HIS SHIFT.

EE WAS OPERATING A CONTINUOUS MINER WHEN HE WAS STRUCK ON THE BACK OF NECK AND LEFT SHOULDER. THE ROCK WAS APPROX. 3' X 3' X 7"" AND FELL OUT BETWEEN THE RIB AND RIB BOLT. EE RECEIVED A HAIRLINE FRACTURE TO THE LEFT SHOULDER AND WAS FITTED WITH A BRACE AND RELEASED TO RETURN TO WORK.

Struck by falling object

EE WAS ROOF BOLTING WHEN A ROCK 4"" X 4"" X 1"" FELL FROM THE ROOF AND STRUCK EE ON THE UPPER LIP. 5 SUTURES WERE REQUIRED TO CLOSE THE LACERATION.

Struck against stationary object

Employee was loading glue onto a roofbolter and cut his right ring finger on a metal band used to bundle up roof bolts. four sutures were required to close the laceration.

Employee was in the process of drilling a hole to install a roof bolt. He did not get the drill steel correctly installed in the drill chuck and when he turned on the rotation the drill steel struck his right hand resulting in a fracture to the hand. He was treated by a doctor on 3/18/04.

EE WAS IN THE PROCESS OF HANGING MINER CABLE FROM THE ROOF WITH TIE WIRE. THE TIE WIRE CAME LOOSE AND MINER CABLE FELL STRIKING VICTIM IN THE MOUTH RESULTING IN A CHIPPED TOOTH.

EMPLOYEE WAS ROOF BOLTING IN THE FACE OF #3 ENTRY ON UNIT #1. A ROCK 6' X 2 1/2' X 2"" FELL OUT FROM THE ROOF & RIB AND STRUCK HIM ON THE RIGHT ARM AND LEFT SHOULDER. HE RECEIVED A SEVERE LACER ATION TO THE RIGHT ARM AND A HAIRLINE FRACTURE TO THE RIGHT ARM. HE RECEIVED 50 PLUS STITCHES AND A CAST TO HIS ARM.
