An unintentional roof fall occurred in #6 entry, #60xc, intersection of the 3rd South Sub Main travelway entry. This area was supported with 10' point anchor roof bolts. The roof fall measured 25'in length by 18'in width & 10'in thickness. The roof fall will be cleaned up.

On 11/21/06, an unintentional roof fall occurred on the 3rd South Sub Main travel way, #6 entry between #60 and #61 cross cut, which is the secondary escape way for #1 unit. This area was supported by 10' and 15' point anchor roof bolts, the roof fall measured: 20' in length, 18' in width and 12' to 15' in thickness. This roof fall will be cleaned up and the area re-supported.

Section C-9. On 11/20/06 an unintentional roof fall occurred in #7 entry, #16xc, intersection of the 3rd South Sub Main return entry. This area was supported with 10' point anchor roof bolts. The roof fall measured 23' in length X 18' in width X 14' thick. The roof fall will be cleaned up.

On 11/14/06, an unintentional roof fall occurred in #8 entry, #24xc. Intersection of the 4th North Sub Main return. This area was supported with 10' point anchor roof bolts. The roof fall measured 22' in lenght X 18' in width X 10'-11' in thickness. The roof fall will be cleaned up.

Section C-9--On 11/13/06 an unintentional roof fall occurred in the last open cross cut, #7 entry, #33xc to #34xc. This area was supported with 6' and 10' point anchor roof bolts. The roof fall measured 50' in length X 18' in width and 12' to 14' in thickness. The roof fall will not be cleaned up. All approaches to this fall have been cribbed off.

This roof fall was supported with 6' point anchor roof bolts and 10' point anchor roof bolts. The roof fall measured 30' long X 30' wide X 12' high and the roof fall will be cleaned up and resupported.

On 9/8/06, an unintentional roof fall occurred in the #1 unit #3 entry at #85 cross cut in the last open crosscut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 35'long x 18' wide x 10' high and the roof fall will not be cleaned up and resupported.

On 9/4/06 an unintentional roof fall occurred on the 1st 42, North Belt line, #4 entry between #52xc & #53xc. This area was supported with 6 foot point anchor roof bolts. The roof fall measured 45 feet in length by 18' feet in width and 8 feet in thickness. The roof fall will be cleaned up and resupported.

Section C-9. On 8/17/06, an unintentional roof fall occurred on the 2nd 42 South haul road, in #6 entry between #61 to #62 Cross Cut. This roof fall was supported with 10' point anchor roof bolts. The roof fall measured 50' long x 18' wide x 10' high and the roof fall will be cleaned up and resupported.

On 7/03/06 an unintentional roof fall occurred on the #2 Unit, MMU-002, #1 return entry between #31xc & 32xc. This roof fall was located in the #1 entry and was supported with 6' anchor roof bolts. The roof fall measured 50' in length by 18' in width and 6 to 7' in thickness. The roof fall will not be cleaned up and all approaches have been cribbed off & flagged off.

5/18/06 an unintentional roof fall occurred on the #4 Unit, MMU-004, #3 intake entry at #29xc. This roof fall was located in the intersection & was supported with 6ft point anchor roof bolts. The fall measured 25'L x 18' W x 7-8' in thickness. The roof fall will not be cleaned up and all approaches have been cribbed off and flagged off.

Roof fall inby loading point on #4 unit, mmu-004, #11 entry return last open xc, which was #20xc. 3 way intersection. 18'long x 18'wide & 12'high. This area was roof bolted with 6'point anchor bolts. #11 entry was drove up approx. 60'inby the fall. Bolted & curtained off & #10 entry was mining towards #11 entry at #21xc within 25-30' to blow thru connecting the 2.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE 1ST WEST SUB-MAIN OFF THE 4TH N. SUB-MAIN IN #9 ENTRY AT #18 CROSS CUT. THIS ROOF FALL WAS LOCATED IN AN INTERSECTION & WAS SUPPORTED WITH 6'POINT ANCHOR ROOF BOLTS. THE ROOF FALL MEASURED 20'LONG X 18'WIDE X 9'HIGH & THE ROOF FALL WILL NOT BE CLEANED UP.

On 3/30/06, an unintentional roof fall occurred on the 1st south off the 2nd west sub-panel at X-cut #39 in #6 entry on the #2 working section. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 25'long X 20'wide X 10'high and the roof fall will not be cleaned up.

On 3/8/06 an unintentional roof fall occured on the MMU 002 working section. This roof fall occu- red in the #1 entry between #24 & #25 crosscuts, this area was supported with 6' fully grouted roof bolts. The roof fall measured 50'x 18' x 12' H. and will not be cleaned up.

On 3/3/06, section C-9, an unintentional roof fall occurred on the 1st South off the 5th West Sub-Panel at X-cut #26 in #7 entry on the #2 working section. This roof fall was located in an intersection and was supported with 10'point anchor roof bolts. The fall measured 70'long x 20'wide x 10'high and the roof fall will not be cleaned up.

An unintentional roof fall occurred on the MMU 002 working section. This roof fall occurred at a 4-way intersection located in the #7 entry at the #21 crosscut. The area was supported with 10' point anchor roof bolts. The roof fall measured 12' x 18' x 12'H. and will not be cleaned up.

1/16/06, an unintentiona roof fall occurred in the #8 return entry at the #6 crosscut of the #4 working section. The area was supported with 6' fully grouted roof bolts, the roof fall measured 18' x 18' x 10' H. This roof fall will not be cleaned up.

AN UNINTENTIONAL ROOF FALL OCCURRED ON THE #1 WORKING SECTION. THIS ROOF FALL OCCURRED AT A 4-WAY INTERSECTION LOCATED IN THE LAST OPEN CROSSCUT, THE AREA WAS SUPPORTED WITH 10'POINT ANCHOR ROOF BOLTS. THE FALL MEASURED 18'X 20'X 20'H. THE FALL WILL BE CLEANED UP & THE AREA RE-SUPPORTED.

Struck by falling object

Roof bolter operator drilling hole for rib pin. EE turned back to the face after reaching for a R.B. and plate he was struck on head/neck/shoulder by piece of rock from between permanent roof supports. Knocked to mine floor. Right hand caught between R.B. and falling rock. Right thumb amputated to end of thumb.

Fall from machine

Employee was riding in the passenger seat of a golf cart, traveling from work area to stope bottom at end of shift. As the golf cart went around a corner turn, he left the golf cart before it stopped and twisted his left knee causing him to fall. This resulted in a fracture to his left knee cap area.

Fall to the walkway or working surface

On 8/29/06 EE was setting up the survey instrument in #8 entry on #1 unit when a roof rock fell out from between roof bolts. The rock struck him on the back of the hard hat and right shoulder driving him to the ground causing a dislocation of the right hip.

Caught in, under or between (Not Elsewhere Classified)

A belt maint person in the process of winding a 3/8"" wire rope onto a cable spool injured his left foot when his foot became caught between the mine floor and the front edge of battery powered scoop bucket. This resulted in three simple fractures to the left metatarsal bones and he was placed in a walking cast.

Employee was working on a conveyor belt located on #2 unit. He was squaring the belt to make a splice using a utility knife. He was pulling the knife toward him while cutting the belt. The knife pulled out of the belt striking him in the right thigh causing a laceration. It took seven sutures to close the wound.

Rubbed or abraded

A mechanic was in a kneeling position using an angle grinder to cut through a piece of metal. When finished he stood and when he applied weight to his left leg his knee popped causing pain. A medical exam revealed a possible sprain. He was fitted with a knee support and returned to work. He worked until 5/1/06 when he was scheduled for surgery to repair a medial meniscus tear.

Struck by falling object

EE was located on right side of roof bolting machine. He was standing 3 rows of roof bolts back from last row & was watching the mine roof for his off-side operator while he installed bolts in area of adverse roof conditions. As he observed the roof, a piece of rock measuring 4'x4'x2"" thick fell without warning striking him on head & shoulder & landed on R foot.

On Feb 23,2006 General Inside Laborer injured his L ankel while working as a section utility person on the MMU 002 working section. He was walking through the last open crosscut when a shuttle car cable drug across the entry striking him on the L foot causing a sprain to his ankle.

Employee was installing metal sidding on the hoist house and metal sidding slipped and cut employee's finger.

Struck by falling object

EE was installing a roof bolt at face area. As the drill steel made contact with the roof, a piece of loose draw slate fell striking him on the right forearm, which inflected a 3"" long laceration to his right forearm. The Dr closed up the laceration with 19 sutures & released him to return to work. He returned to work the same day & finished installing roof bolts.

Fall onto or against objects

A piece of draw slate fell from between installed roof bolts approx. 4 rows outby the last row of roof bolts. The slate measured 54"" long, 32"" wide and 0"" to 6"" thick. The mining height was around 8.5'. Employee was standing alongside of the bolter machine; the draw slate struck his hard hat, middle back, left shoulder, left arm and drove him into bolter. His chest was bruised.

Struck against stationary object

Bolter operator turned around quickly while making up bolt and plate combinations. He struck his right forearm against a stack of roof plates, causing a laceration to his right forearm.

Struck by falling object

on 9/22/06, EE was installing belt framing on #4 unit. As he was installing a roller, he dropped the roller and caught his left index finger between the structures causing a laceration. The laceration took four sutures to close.

On 9/16/06 employee was using a lifting jack to lift the pan on a continuous miner when the handle slipped out of jack hitting him under the chin causing a laceration. The laceration took 10 sutures to close.

Struck by falling object

On 8/17/06, employee was drilling a hole in the mine roof for a roof bolt. As he was drilling the hole a small piece of rock fell from the mine roof striking him just below the right eye causing a laceration. The laceration was closed with two sutures.

Struck by falling object

A continuous miner operator was holding a ladder while another miner was hanging a curtain when he dropped his hammer which struck the miner on the right forehead above the right eye. He received three sutures to close the wound.

Employee was installing roof bolts, bolting up a fresh cut. While starting the drill steel rotations with steel against the roof, a small piece of rock fell from this area on drill point, striking his chin resulting in a 1"" deep laceration on his chin.

EE a roof bolter operator had installed the last bolt in the last row of bolts at the face and as he dropped the ATRS a small piece of rock broke out of the mine roof. The rock slid down the ATRS boom dropping off onto the drill pot and then bouncing up and striking the operator on the mouth He received a small laceration requiring two sutures to close.

On 02/24/06, a roof bolter operator finished drilling a roof bolt hole and as he lowered the drill boom the drill steel hung up in the hole then fell from the drilled hole striking his right index finger resulting in a laceration requiring 4 sutures to close.
