Accident type, without injuries

An unintentional roof fall occurred on the 1st E. sub-main, off 4th N. sub-main on 004 unit #6 entry at spad #13+60. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The fall measured 25' in length by 18' in width and 14' in thickness. The roof fall will be cleaned up and the area will be re-supported.

An unintentional roof fall occurred on the 1st Main West, entry #1, crosscut #79. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 20' in length by 18' in width and 10' in thickness. The roof fall will be cleaned up and the area will be re-supported.

9/25/05 AN UNINTENTIONAL ROOF FALL OCCURRED ALONG THE SECOND WEST CONVEYOR BELTLINE AT THE #86 CROSSCUT. THIS AREA WAS SUPPORTED BY 10' POINT ANCHOR ROOF BOLTS AND ROOF FALL WILL BE CLEANED UP AND AREA PROPERLY RE-SUPPORTED.

Unintentional roof fall occurred on the 001 working section, in the #6 entry at #8. This area was supported with 10' point anchor roof bolts. Supplementary roof supports were installed around the roof fall area and the fall will not be cleaned up.

on 9/13/05, an unintentional roof fall occurred along the 2nd west beltline (#5 entry) at #75 cross cut. This area was supported with 10"" point anchor roof bolts and the roof fall will be cleaned up and re-supported.

9/13/05 AN UNINTENTIONAL ROOF FALL OCCURRED BETWEEN THE #8 & #9 ENTRIES IN THE LAST OPEN CROSS CUT LOCATED IN THE 001 WORKING SECTION. THE ROOF MEASURED 25'L X 18'W X 10' HIGH. THE AREA WAS SUPPORTED WITH 10' ROOF BOLTS. THIS ROOF FALL WILL NOT BE CLEANED UP.

On 9/6/05 an unintentional roof fall occurred on the 1st 42 South belt line, in #5 entry #45 cross cut. This roof fall was located in an intersection and was supported with 10 foot point anchor roof bolts. The roof fall measured 20' in length by 20' in width and 10' in thickness. The roof fall was cleaned up and the area was re-supported.

The roof fall measured 35' long x 18' wide x 10' high. The roof fall was cleaned up and the area was re-supported.

ON 08/12/05, AN UNINTENTIONAL ROOF FALL OCCURRED ALONG THE 2ND MAIN WEST AT CROSS CUT #91 IN THE #3 ENTRY. THIS ROOF FALL WAS LOCATED IN A INTERSECTION AND WAS SUPPORTED WITH 10' POINT ANCHOR ROOF BOLTS. THE FALL MEASURED 25'X18'X10' AND THE FALL WILL BE CLEANED UP

On 8/4/05, an unintentional roof fall occurred along the 3rd south, 1st west sub-man at cross cut #34 int he #2 entry. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 30'l x 20'w x 12'h and the roof fall will not be cleaned up.

ON 7/14/05 AN UNINTENTIONAL ROOF FALL OCCURRED ALONG THE 1ST MAIN WEST SUM MAIN AT CROSS CUT #16 IN THE #3 ENTRY. THIS ROOF FALL WAS LOCATED IN AN INTERSECTION AND WAS SUPPORTED WITH 10' POINT ANCHOR ROOF BOLTS. THE ROOF FALL MEASURED 22' X 18' X 10' AND THE ROOF FALL WILL NOT BE CLEANED UP

0N 07/13/05, AN UNINTENTIONAL ROOF FALL OCCURRED ALONG THE 3RD MAIN WEST AT CROSS CUT #140, BETWEEN THE #4 AND #4 ENTRIES. THIS ROOF FALL WAS LOCATED IN A CROSS CUT AND WAS SUPPORTED WITH 6' POINT ANCHOR ROOF BOLTS, THE ROOF FALL MEASURED 30' X 18' X 7' AND THE ROOF FALL WILL NOT BE CLEANED UP

An unintentional roof fall occurred along the #2 entry at #6 crosscut in the Main North roadway. This roof fall was located in an intersection and was supported with 10' point anchor roof bolts. The roof fall measured 20' long x 18' wide x 12' high and this roof fall will be cleaned up.

An unintentional roof fall occurred in the second main west at #28 crosscut between #5 & #6 entries. The roof conditions consisted of laminated shale supported with 10' fully grouted roof bolts.

A roof fall occurred in the Unit #1 intake aircourse at entry #6 at 18 xcut. The fall is approximately 20'L x 18'W x 10'H. The fall will not be cleaned up. The intersection was supported with 10' point anchor bolts.

A roof fall occurred old 4th south unit 2. Fall is in an intersection in a return entry. The fall affected ventilation and unit mmu 002-0 ceased production until the fall was cleaned up. The fall is 18' l by 18' w by 8' h. Supported with 6 and 10 ft point anchor roof bolts.

EMPLOYEE WAS BUILDING A BRATTICE WALL & AS HE BENT OVER TO PICK UP A CONCRETE BLOCK, A PIECE OF ROCK FELL OUT FROM BETWEEN THE ROOF BOLTS STRIKING HIM ON THE HEAD, NECK AND BACK.

On 10/06/05, a general inside laborer working on a producing section was reclaiming ventilation curtain. As he was stepping across a shuttle car cable the cable tightened up catching his right foot and caused him to twist his right knee.

ON 8/26/05 EE WAS PERFORMING SERVICE WORK ON A CONTINUOUS MINING MACHINE. HE WAS IN THE PROCESS OF ADDING OIL TO THE POTS WHEN A PIECE OF ROCK (4' X 6' X 2"" THICK) FELL OUT FROM BETWEEN THE PERMANENT ROOF SUPPORTS, STRIKING HIM ON THE TOP OF HIS RIGHT FOOT.

On 6/10/05, a mechanic working the afternoon shift was walking from #2 entry to #3 entry when a large piece of rock fell out of mine roof from between permanently installed roof supports, it measured 8' long x 2' wide x 8"" thick. This rock fell out of the mine roof between a roof bolt & the rib, striking him on left lower leg & ankkle, fracturing bones in lower leg.

Over-exertion in lifting objects

He was lifting & stacking concrete block when he felt pain in his lower back. He was treated for a lumbar strain & treated conservatively. He returned to work missing no scheduled work time and was later diagnosed as having a herniated lumbar dis.Surgery to repair the herniated disc back.He will off work for several days and should return to full duty after his recovery.

Struck by rolling or sliding object

Employee was starting to pin on unit #2 entry 4 xcut right. A rock approximately 11""L x 18"" - 48""W x 12"" - 48"" thick rolled out from rib and struck his left leg and lower back. He received a fracture to his left leg and a compound fracture to his lower back. He underwent surgery to repair his left leg. No surgery on back.

WHILE CAPPING OFF A BRATTICE, EE MASHED HIS LEFT THUMB BETWEEN A BOARD AND A CONCRETE BLOCK. AREA WAS SOMEWHAT MUDDY - BOARD AND HAMMER HAD MUD ON THEM. (HAMMER SMASHING THUMB - SLIPPED ON MUDDY BOARD. HAD 11 SUTURES.)

11/25/05 roof bolter operator was moving bolting supplies from the center area of the machine to his work tray near the drill boom, when a piece of rock fell out from between roof supports striking him on the right index finger causing a laceration requiring 5 sutures to close. Was treated & released back to work with no restrictions.

Struck against stationary object

On 10/19/05, a roof bolter operator was replacing a suction hose, he was cutting the hose off to size when the cutting blade in the utility knife he was using broke & lodged in the hose. As his arm brushed across the hose the broken blade lacerated his left arm requiring seven sutures to close the wound. The miner was treated & released back to work without restrictions.

EMPLOYEE WAS WALKING NEXT TO A DOUBLE BOOM ROOF BOLTER WHEN HE TRIPPED & FELL CATCHING HIS LEFT HAND & FINGERS ON THE EDGE OF THE ROOF BOLTER CAUSING A LACERATION TO HIS FINGER.

EE was as roof bolter helper, making up roof bolts for roof bolter op. Standing outby the roof bolt machine when piece of rock 3'x4'x6"" fell out from permanent supports strike him abrasions his right shoulder, left knee, and laceration his left index finger 7 sutures to close. return to work for next regularly scheduled with no restrictions.

7/5/05, ee was working on a conveyor belt located int he wash plant, he was removing a belt wiper when the belt wiper frame dropped out of its hanger and caught his right hand between the wiper and belt structure causing a contusion and laceration to his right hand.

EMPLOYEE WAS CONSTRUCTING A RETURN BRATTICE WALL. SHE WAS USING A BLOCK HAMMER TO SIZE CONCRETE BLOCKS FOR THE BRATTICE WALL WHEN SHE STRUCK HER LEFT HAND WITH THE HAMMER CAUSING A CONTUSION AND A LACERATION REQUIRING TWO SUTURES TO CLOSE.

Employee was operating a roof bolter, installing permanent roof bolts. As he was removing a drill steel from a hole, the two-piece steel parted and the end of the drill steel struck him on the left side of his face causing a small laceration and fracture to his cheek just under his left eye.

WHILE CHANGING OUT A BELT CHAIN, THE CHAIN HOIST BROKE & STRUCK HIS RT. HAND, CAUSING INJURY TO HIS INDEX & MIDDLE FINGERS.

EE was kneeling on the mine floor using a utility knife to cut through a piece of mine belt. As he was cutting the belt he accidently cut his left knee with the knife. 17 sutures were required to close the laceration and he returned to work.

Employee was changing out tail roller on unit 3 belt tail. He and his co-workers slipped with the roller and it caught his left thumb between the roller and frame of the tail piece. He received a laceration to his left thumb. He received seven sutures and returned to work.

Employee was installing a roof bolt and as he pushed the bolt and plate to the roof he got his left index finger caught between the roof and roof bolt plate. He received a severe laceration to the end of his left index finger. Several sutures were required to pull skin back over the end of his finger.

EMPLOYEE WAS CUTTING OPEN A SPLICE ON A SHUTTLE CAR CABLE USING A RAZOR KNIFE. AS HE WAS CUTTING OPEN THE CABLE HE SLIPPED WITH THE KNIFE & CUT THE TOP OF HIS LEFT INDEX FINGER. HE RECEIVED 3 SUTURES TO CLOSE THE LACERATION.
