A roof fall occurred in the 1B5 Panel, cross cut #32, entry 8-9. The fall is approximately 18' wide and the height is unknown.
The contract employee was hanging miner cable during a power move and felt a pain in shoulder. EE was diagnosed with a strain and was not able to fulfill normal work activities.
The employee stepped off a supply trailer and strained hip.
The employee got foot caught between the mine floor and the rear bumper of the truss bolter, causing a fracture to the 3rd metatarsal on right foot.
A contract employee was roof bolting. The bolter was moving when the employee thought the cable was going to get run over. They flagged and yelled at the operator to stop. When they bent over the operator thought they flagged to resume tramming. When tramming resumed the contract employee got caught between the rib and the ATRS pad.
A contract employee was operating a mini-trac and a piece of loose material fell from the mine roof hitting the employee in the groin.
A contract employee was roof bolting, and while bending a roof bolt felt a pain in left elbow.
Contractor was said to have been struck in the back by a rock of unknown dimensions while servicing a miner. EE began missing work the following shift.

While operating the continuous miner a piece of draw rock fell from the roof, measuring 6"" wide X 6"" long X 1/2 inch thick. It struck the employee on the back of the neck. 4 sutures were used to close the wound.
The employee had a pry bar slip while replacing a tram motor in a shuttle car, causing a laceration to left index finger. 2 sutures were required to close the wound.
The employee hit a bump while operating a shuttle car and chipped employee's upper left lateral incisor.

While getting supplies out of toolbox the lid shut on employee's left hand. This caused a laceration. Sutures were required to close the wound.
