EE was loading belt framing for a belt lay. He picked up a piece of framing and felt a pain in his back. He continued to work without restrictions. Due to sporadic pain he was scheduled an MRI. He was given restrictions on 06/16/2015 that due not allow him to fulfill all duties of his regular occupation.

Steel slipped out of his hand striking his finger. Damage being, individual had to wear a splint on his finger.
The employee was cutting a piece of belt with a utility knife. The knife slipped and cut through the employee's glove, causing a laceration to his left thumb. Sutures were required to close the wound.
The employee was cutting a piece of conduit with a utility knife. When the blade cut through the conduit it cut through his leather gloves causing a laceration to his left index finger. Sutures were required to close the wound.
Employee was removing a crib block from a pallet, when one fell over mashing his right index finger. This caused a laceration which had to be closed with sutures.
Employee was roof bolting on #4 unit between #5 & #6 entries. When he was swinging the boom back in, it hit a roof bolt in the tray causing it to pop out of the tray hitting him in the right eye lid. This caused a laceration that had to be closed with sutures.
The employee was adjusting the position of a welder. During the process his finger got pinched between the welder & the table. This caused a laceration to the index finger on his left hand. Sutures were required to close the laceration.
While riding in a man trip, the trip moved over to pass parked supply trailers. In doing so it caused employee to hit his left leg on the trip causing a laceration which had to be closed with sutures.
The employee was changing out a hydraulic hose. During the process a frayed strand on the hose caused a laceration to his left index finger. 2 sutures were required to close the wound.
The employee was hooking a tow bar to a diesel tractor. When putting the pin in place he pinched his right pinky finger which caused a laceration. Sutures were required to close the wound.
EE was under a Getman Tractor removing a one inch hydraulic hose. As the hose came loose it struck him in the mouth cracking a least one tooth.
The employee was clearing loose material in the belt entry. He picked up a rock to place it on the belt and lost his footing. He lost his footing while placing the rock on the belt and his left middle finger got caught between the rock he was holding and the ground causing laceration. Sutures were required to close the wound
The employee was roof bolting in #8 entry. He had drilled his first drill steel to the roof and then reached for his second drill steel. When pulling his arm back his right arm contacted a roof bolt plate at his drill station, causing a laceration. Sutures were required to close the wound.
The employee went to the mantrip to get a drink a water. While standing beside the mantrip a piece of loose material fell from the mine roof causing a laceration on his neck. Sutures were required to close the wound.
While removing the outer reel bearing on a shuttle car with a hammer, a piece of hammer broke off and struck the mechanic in the right hand. The piece of metal had to be cut out and closed with sutures.
EE was pulling the head shaft out of the feeder. He had his left hand on the shaft. The other side of the shaft fell out of the feeder causing the end he had hold of to kick up and pinch his hand between the shaft and the feeder frame. This caused a laceration which had to be closed with sutures.
EE had finished bolting for the night and was just about to leave for the man trip when he realized he had left his wire cutters on the bolter. He reached back and grabbed the cutters, then turned around running into the edge of the ATRS on the bolting machine. This caused a laceration above the left eye which had to be closed with sutures.
As employee was walking along the West side of the 405 conveyor, he stepped in a hole, causing his right ankle to turn abnormally, causing a fracture to his ankle. He was treated with an ankle boot and returned to work.
The employee was knocking a bearing out. The bearing slivered and inserted in his left middle finger. A physician was required to remove the sliver.
Employee was changing a ball valve using a pipe wrench. As he was trying to tighten the valve the wrench slipped causing his right hand to strike the structural steel beam behind him. This caused a laceration which had to be closed with sutures.
Employee was cutting high voltage guarding with a utility knife when the knife slipped out of the guarding and cut his left thumb. The laceration had to be closed with sutures.
Employee was trying to match up his roof drill starter steel with the end pusher steel. As he was doing so, the web of skin between the thumb and index finger was caught between the two steels causing a puncture laceration which had to be closed with sutures.
The employee was installing a 10' roof bolt. He raised the bolt up and due to the height, he had to use a wrench. He dropped the drill boom and while inserting the wrench in the chuck, the bolt fell out causing a laceration to his left middle finger. Sutures were required to close the wound.
Employee was using a hammer and chipping hammer to remove a bolt from a classified cyclone. A piece of the hammer broke off and struck him in left hand. The metal didn't have to be removed by surgery. *This accident should not have been reported.
