Accident type, without injuries

On 12/29/2007 an unintentional fall of roof occured on the 4th North Sub Main in #5 entry between cross cut 88 to 89. The fall measured approximately 16' wide x 35' long x 6' to 8' thick. The area was bolted with 6' point anchor roof bolts. The fall will be cleaned up as approved by MSHA.

On 12/07/2007 an unintentional fall of roof occurred on the 4th South Sub Main, entry #5, at cross cut #29. The fall was approximately 45' long x 18' wide x 15' thick. The area was bolted with 10' point anchor roof bolts and cribs. The fall was in the belt entry. The area was fractured stack rock.

An unintentional fall of roof occured in #5 entry, cross cut #33, on the 4th South Sub Main. The fall measured 20' long x 18' wide x 15' thick. The fall consisted of fractured lamanated stack rock. The area was bolted with 10' point anchor bolts. The fall will be cleaned up and resupported.

On 10/24/07 an unintentional roof fall occurred on the 4th South Sub Main, entry #6, at cross cut # 24. The fall was 55' long x 18' wide x 12' high. The intersection was bolted with 10' point anchor roof bolts. The area was fractured lamanated rock.

On 10/04/07 an unintentional fall of roof occured on the 4th South Sub-Main, #5 entry, at cross cut number 31. The fall was 18' long x 18' wide x 10' high and was bolted with 10' point anchor roof bolts. The fall was caused by fractured laminated stack rock and will be cleaned up.

On 9/25/07 an unintentional fall of roof occurred at cross cut #32 in #4 enrty on the 4th South Sub Main. The fall was 18"" Long x 18 wide X 10' thick, and will be cleaned up. The fall is fractured laminated rock.

On 9/17/2007, an unintentional fall of roof occurred in # 5 entry at cross cut # 28 on the 2nd West Sub Main off the 3rd South Sub Main. The fall was approxmiately 22' long x 18' wide x 6' thick. The area was bolted with 6' & 10' point anchor bolts. The fall was laminated stack rock and will not be cleaned up.

On 9/2/07 an unintentional roof fall occurred on the 1-A travel road at cross cut #27 on the 2nd West Sub Main off the 3rd South Sub Main. The fall was 20' long x 20' wide x 20' high. The fall was fractured stack rock bolted with 6' and 10' point anchor roof bolts. The fall will be cleaned up.

An unintentional fall of roof occurred on the 2-B conveyor belt between cross cut # 22 and 23 on the 4th South Sub Main, off the 5th West Parallel. The fall was approximately 30' long x 18' wide x 6' thick. The fall appeared to be loose stack rock that was supported by 6' point anchor roof bolts. The fall will be cleaned up.

An unintentional fall of roof occurred on the 1st Main West number 4 entry at cross cut number 13. The fall was 20' long x 20' wide x 25' thick. It occurred in a 4-way intersection in the supply road. The area was supported by 10' point anchor roof bolts and steel rails. The fall appeared to be loose stack rock. The fall will be cleaned up.

On 08/18/2007, an unintentional roof fall occurred at cross cut # 13, on the working section 001-0 in entry #2 in the 2nd East panel, off the 1st South Sub main. The roof fall measured 35' long, 18' wide and 12' thick. The fall will not be cleaned up. The fall appeared to be broken stack rock. The area was bolted with 6' and 10' point anchor roof bolts.

On 8/3/07, an unintentional roof fall was found at cross cut #13 in #8 entry on the 2nd West sub main, off the 3rd South sub main. The fall was approximately 20' long X 24' Wide X 20' high. The roof was bolted with 10' point anchor bolts. The fall will be cleaned up.

On 8/02/2007 a continuation of an unintentional roof fall occured at cross cut #61 in #6 entry on the 3rd South Sub main. The roof fell between two sets of arches in an intersection that had been cribbed and rail bared. The roof fall will be cleaned up.

On 7/31/2007, an unintentional roof fall occurred at cross cut #6, entry #3, on the 1st East panel, off the 1st South Sub main. The roof fall measured 18' long X 18' wide X 12' high. The area was bolted with 6' and 10' roof bolts. The fall was caused by broken roof strata.

While being lowered in sinking bucket, a strap and clevis engaged with shaft door upsetting the sinking bucket exposing three individuals to a fall hazard resulting in three fatalities.

Caught in, under or between (Not Elsewhere Classified)

Employee was standing on a supply trailer stacking concrete bags on a pallet when his right leg fell in the hole where the wheel rail is located. He fell off the trailer on his back with his leg hung in the hole, hurting his right knee.

Struck by falling object

While drilling, a large rock fell of the rib approximately 15' above employee striking him in the head and knocking his hard hat off causing compression fractures of C5 and C6. Another rock then fell striking him in the head causing a laceration on the scalp.

On 05/03/07, EE was cutting coal on MMU 003 in the #8 left entry. While cutting coal, a rock fell from the roof between the rib & the first roof bolt. The rock was approx. 3' wide x 7' long x 2"" to 3"" thick. It hit him on the head & shoulders & rode him to the ground. The fall of rock caused a compressed disc & fracture to the lower back.

Struck by falling object

EE was pulling down loose roof rock on MMU 003-0. As he pulled down a piece of rock with a pry bar, the rock hit the canopy of the roof bolter, causing the rock to flip over and fall on his left foot, causing a fracture.

Employee was moving shaft forms and standing them up when the wind blew them over on his hip, leg and left hand fracturing his 1st and 2nd fingers on left hand.

Over-exertion in lifting objects

Employee was lifting a 90lb buster (rock drill) to hang on the side of concrete form stell when he strained his right shoulder.

Struck by rolling or sliding object

Employee had backed the miner out of #7 entry and was pulling his cable back with the miner when the slack tightened on the intake rib. The rib rolled off onto his lower legs while he was throwing cable over from the miner. This caused a laceration to his lower left leg which had to be closed with sutures.

While hanging a curtain in #2 entry, the hammer he was using slipped from his grip and hit him in the mouth, breaking his front tooth.

Struck against stationary object

Employee was pulling on a suction hose on a roof bolter when his forearm was cut by something sharp above his arm. The suction hose was running to the cyclone. He did not know what cut his arm.

EE was using a cylinder pin to knock out a cylinder pin on an ATRS cylinder on a Fletcher double boom roof bolter. The pin slipped, causing the hammer to hit his left thumb, causing a laceration that had to be closed with sutures.

Roof bolt drill steels where stuck together. When EE tried to get them apart the bottom steel flew out, hitting him on the right eye brow, causing a laceration which had to be closed with sutures.

Employee was handling panning (tin) when it cut through his leather ""Rider's"" glove and cut his right thumb requiring 5 stitches.

On 05/24/07, an employee was driving to a Mine Rescue Contest, when the vehicle he was driving was hit in the rear by another vehicle. The accident caused a fracture to the right hand. Another employee was also injured in the accident. He received a laceration above his eyes that required sutures to close.

On 5/23/07 Section Foreman on 002 unit was sitting on gob pile talking to his roof bolter operators. As he was talking, a rock fell from between roof bolts striking him on the right knee. The falling rock caused a laceration which required sutures to close. The rock was approx. 2' X 2' X 3"" thick. First aid was administered and he was transported out of the mine.

While drilling, employee struck the rig of the shaft with the boom of the drill jumbo, dislodging a small rock. The rock struck the employee on the hand causing a small laceration requiring eight stitches.

On 04/18/07, employee was cutting rubber for belt skirting with a utility knife. When he cut through the rubber, he also cut his left leg. The puncture was closed with sutures.

EE was roof bolting between #8 and #9 entry on #1 unit when a piece of rock fell from the roof striking him on the back causing a laceration. The laceration was closed with surtures.

EE was roof bolting in #6 entry on #1 unit, when a piece of rock fell from the roof striking him in the front of his neck, causing a 2"" laceration . The laceratin was closed with sutures.

Struck by falling object

EE was roof bolting on #1 unit, when the suction on the roof bolter became stopped up. EE lowered the pot, leaving drill steel in roof. When he put his left hand on the drill pot, the drill steel fell from the drill hole in the roof. The steel landed on his left ring finger and little finger, causing a fracture to the ring finger.
