FALL OCCURED AT XCUT 120 #4 ENTRY IN THE 3RD MAIN WEST BELTLINE. THE ROOF WAS BOLTED WITH 10' DOUBLE LOCK BOLTS. L 22' W 30' BY H 12'.
A ROOF FALL OCCURRED IN 1-C PANEL (1ST SOUTH SUBMAIN OFF OF 2ND WEST PARALLELS) BETWEEN #4 & #5 ENTRIES XCUT 33. L 12' X W 17' X T 12'. THE ROOF WAS BOLTED WITH 6' & 10' DOUBLE LOCK BOLTS WITH MONSTER PLATES.
THE SLOPE CAR WAS IN PROCESS OF LOWERING INTO THE MINE WHEN THE HOIST KICKED OUT SETTING THE BRAKES UP APPROX 800' DOWN, IT WAS DETERMINED THAT THIS PROBLEM WAS CREATED DUE TO THE POWER SUPPLY FROM WHICH WE RECEIVE OUR POWER.
A ROOF FALL OCCURRED AT X CUT 113 1ST MAIN, WEST #6 ENTRY L 20'X W30' X T 10' THE ROOF WAS BOLTED WITH 10' POINT ANCHOR BOLTS.
THE BRAKE CAR FAILED THE OVERSPEED TEST. WE DID HAVE BACK UP SYSTEM. CAR WAS TAKEN OUT OF SERVICE SO THE PROCESSOR COULD BE REPROGRAMMED TO CHECK SYSTEM. THIS TOOK APPROX. 2 1/2 HOURS.

A ROOF FALL OCCURRED IN THE 1ST MAIN WEST #9 ENTRY #60 X-CUT. L 25' X W 18' X T 10'. THE ROOF WAS BOLTED WITH 10' POINT ANCHOR BOLTS. PROBABLE CAUSE OF FALL: LAMINATED ROOF.
A ROOF FALL OCCURRED IN THE 1ST MAIN WEST #8 ENTRY BETWEEN 59 & 60 XCUTS. L 45' X W 18' X T 8'. THE ROOF WAS BOLTED WITH 6' POINT ANCHOR BOLTS. PROBABLE CAUSE OF FALL: LAMINATED ROOF.
EMPLOYEE WAS IN THE PROCESS OF INSTALLING A 10'ROOF BOLT, HE TIED A PIECE OF WARNING FLAGGING AROUND THE BOLT. IT CAME OFF AND HE ATTEMPTED TO PLACE IT BACK ABOVE THE PLATE CATCHING HIS FINGER BETWEEN THE ROOF AND THE PLATE. WHEN HE PULLED BACK HE TORE THE TIP END OF HIS FINGER OFF.
EMPLOYEE WAS IN THE PROCESS OF MOVING THE SUPPLY FORKS ON A MINI TRACK WHEN A FORK SLIPPED AND CAUGHT THE EMPLOYEES FINGER BETWEEN THE FORK AND THE MOUNTING FRAME.
EE WAS STRUCK BY A ROCK THAT FELL OUT BETWEEN ROOF BOLTS AND THE RIB. HE WAS STRUCK ON THE LEFT FOOT RESULTING IN SWELLING AND A CONTUSION. HE WAS IN THE FACE AREA ON #1 UNIT, RUNNING A CONT INUOUS MINER. ROCK WAS APPROXIMATELY 31"" X 31"" X 3 1/2"".
EMPLOYEE WAS OPERATING A CONTINUOUS MINER UNDER SUPPORTED ROOF. HE WAS IN THE PROCESS OF CLEANING UP A CROSS CUT RIGHT WHEN ROOF ROCK FELL OUT AND HIT THE MINING MACHINE. A PIECE OF THE ROCK B ROKE OFF AND STRUCK EMPLOYEE IN THE LOWER RIGHT LEG RESULTING IN A COMPOUND FRACTURE.
EE WAS RIDING TO HIS UNIT IN A MANTRIP WHEN THE MANTRIP STRUCK A ROUGH SPOT IN THE ROAD CAUSING THE EE TO BOUNCE IN HIS SEAT & SPRAIN HIS LOWER BACK. EE DID NOT START MISSING WORK UNTIL 10/24.
EE WAS OPERATING A BATTERY, HE EXITED THE SCOOP TO HANG A CABLE AND WATERLINE WHEN THE SCOOP ROLLED FORWARD AND FRACTURED HIS FIBULA IN HIS RIGHT LEG.
EMPLOYEE WAS INSPECTING A FIRE VALVE OUTLET THAT WAS LEAKING WHEN THE VALVE & PIPE NIPPLES BLEW AWAY FROM THE LINE & STRUCK HIS FACE CAUSING SWELLING & TROUBLE OPENING HIS MOUTH.
EE WAS IN THE PROCESS OF MOVING MINER CABLE OVER WHEN A PIECE OF DRAW ROCK FELL FROM THE ROOF AND STRUCK THE EE IN HIS LOWER BACK. FIRST DAY AWAY FROM WORK WAS 7-31-03.
EE WAS IN THE PROCESS OF BUILDING A PERMANENT VENTILATION CONTROL (BRATTICE) WHEN A SHUTTLE CAR ENTERED THE CROSSCUT WHERE SHE WAS WORKING AND STRUCK HER IN THE LEFT LEG CAUSING A STRESS FRACT URE TO THE FIBULA.
EMPLOYEE WAS STANDING ALONGSIDE A FLETCHER ROOF BOLTING MACHINE IN THE INTERSECTION OF #5 ENTRY IN THE LAST OPEN XCUT WHEN A SHUTTLE CAR CAME THROUGH THE WING CURTAIN AND STRUCK THE EMPLOYEES RIGHT LEG, MASHING IT BETWEEN THE SHUTTLE CAR AND BOLTING MACHINE, CAUSING A CRUSHING INJURY TO HIS RIGHT FEMUR AND KNEE CAP.
EMPLOYEE WAS PICKING UP TRASH AND STEPPED IN A HOLE. HIS KNEE BUCKLED, TWISTED AND THEN POPPED. EMPLOYEE'S FIRST DAY MISSED WAS 5/8/03.
EE WAS CARRYING 2 SMALL PIECES OF METAL TO PLACE IN THE METAL RACK WHEN HE SLIPPED ON A PATCH OF ICE AND INJURED HIS KNEE. EE CONTINUED WORKING UNTIL HE UNDERWENT SURGERY AND STARTED MISSING W ORK ON 03-21-03.
EE WAS IN THE PROCESS OF SPLICING A SHUTTLE CAR CABLE ON #1 UNIT. HE TRIMMED PART OF THE CABLE AND IN DOING SO CUT INTO HIS RIGHT THUMB WITH A BELT KNIFE. HE RECEIVED SUTURES.
EMPLOYEE WAS IN THE PROCESS OF HANGING A VENTILATION CURTAIN. WHEN HE ROSE TO HANG THE CURTAIN, A PIECE OF WIRE HANGING FROM THE MINE ROOF WENT INTO HIS EAR CAUSING A LACERATION.
EE WAS IN THE PROCESS OF CUTTING A METAL BAND WITH BAND CUTTERS WHEN THE BAND THAT WAS AROUND A BUNDLE OF ROOF BOLTS POPPED UP AND STRUCK HIM IN THE UPPER LIP, HE RECEIVED 4 SUTURES.
EMPLOYEE WAS INSTALLING A SECOND STEEL INTO THE HOLE HE WAS DRILLING. HE PLACED THE SECOND STEEL IN THE CHUCK AND RAISED THE BOOM, AT THIS TIME THE STEEL BENT AND STRUCK THE EMPLOYEE IN THE LE FT HAND CAUSING A LACERATION THAT REQUIRED 12 SUTURES.
EMPLOYEE WAS IN THE PROCESS OF INSPECTING A CHECK VALVE THAT WAS ON A ROCK DUSTING MACHINE, AS HE KNELT DOWN TO LOOK AT THE VALVE A COAL RIB ROLLED OFF AND STRUCK THE EMPLOYEE.
EMPLOYEE WAS OPERATING A A.L.LEE MINI TRACK IN THE PROCESS OF MOVING SUPPLIES WHEN A FIRE EXTINGUISHER THAT WAS HANGING FROM THE ROOF CAME IN CONTACT WITH THE FORKS OF THE MINI TRACK AND FELL STRIKING THE EMPLOYEE ON THE WRIST AND HE NEEDED SUTURES.
EE WAS IN THE PROCESS OF WALKING DOWN THE BELT SLOPE TO APPLY CHLORIDE TO THE ICE THAT WAS ON THE FLOOR, WHEN HE SLIPPED ON THE ICE AND STRUCK HIS HEAD, ARM AND HAND CAUSING A HAIRLINE FRACTUR E TO HIS RIGHT HAND.
EE WAS INSTALLING 10'ROOFBOLTS WHEN HE FELT A SHARP PAIN IN HIS LEFT SHOULDER. HE HAS CONTINUED TO WORK UNTIL 12/8/03, HIS FIRST DAY LOST WAS 12/09/03.
EMPLOYEE WAS WALKING TOWARD THE FACE OF #3 ENTRY ON #3 UNIT WHEN HE STEPPED ON A LUMP OF COAL AND FELL LANDING AWKWARDLY ON HIS LEFT FOOT CAUSING A FRACTURE.
