#31 HOIST LOST POWER ON LOWER CONTACTOR. ELECTRICIANS CHANGED CONTACTS AND FUSES AND RETURNED HOIST TO SERVICE. THE HOISTMAN TRIED TO CALL MSHA AND COULDN'T MAKE CONTACT OVER PHONE. HE DID CON TACT DOE RUN SAFETY DEPT AT VIBURNUM AND THEY CALLED MSHA. HOIST WAS BACK IN SERVICE AT 8:40 AM.THE WEST FORK HOIST WAS PUT INTO SERVICE DURING THIS TIME.
31 HOIST LOST POWER IN THE LOWER MODE. ELECTRICIANS WERE CALLED AND THEY REPLACED ALL SIX CONTACTS IN THE MAIN LOWER CONTROL CABINET AND RETURNED THE HOIST TO SERVICE. MSHA WAS CALLED AT 10:55 A.M. AND TOLD HOIST WAS DOWN AND THEN CALLED AT 11:43 AM SAME DAY AND TOLD THAT HOIST WAS BACK IN SERVICE.
#31 HOIST LOST POWER, ELECTRICIANS FOUND A LOWER MAIN CONTACTOR BURNED UP. THEY REPLACED THE CONTACTS AND RETURNED THE HOIST TO SERVICE. MSHA WAS CALLED AT 12:20 P.M. AND TOLD HOIST WAS OUT OF SERVICE. THEN HOISTMAN CALLED MSHA BACK AT 12:50 P.M. SAME DAY AND SAID HOIST WAS BACK IN SERVICE.
HAD A TERMINAL BAD ON 1C CONTACTOR THAT WOULDN'T ALLOW HOIST TO POWER UP TO FULL SPEED. NO ONE WAS ON THE HOIST. MSHA WAS NOTIFIED AT 6:10 A.M., WHEN HOIST WENT DOWN, AND AGAIN AT 6:50 A.M. WHEN HOIST WAS BACK IN SERVICE.
HOIST LOST POWER AT 3:10 PM AND ELECTRICIANS WERE CALLED TO REPAIR IT. MSHA WAS CALLED AT 3:23PM AND TOLD THAT #31 HOIST AT FLETCHER WAS OUT OF SERVICE. THE ELECTRICIANS FOUND ALL AUXILLARY C ONTACT THAT WAS BAD ON THE MAIN LOWER CONTACTS AND CAHNGED IT OUT. HOIST WAS RETURNED TO SERVICE AT 5:29 PM SAME DAY AND MSHA WAS CALLED AND TOLD HOIST WAS BACK IN SERVICE.
HOISTMAN HAD BEEN TAKING SUPPLIES UNDERGROUND. HE BROUGHT CAGE TO SURFACE LANDING AND SPOTTED IT. AT THAT POINT HE HEARD A POPPING NOISE IN MOTOR CONTROL. HE CALLED ELECTRICIANS AND THEY CH ECK IT OUT AND FOUND HOIST MOTOR BD. CALLED MSHA AT 1400.
HAD ELECTRICAL RELAY GO BAD ON #33 HOIST. HOIST WOULD NOT MOVE. HAD TO REPLACE RELAY TO PUT IN SERVICE. MSHA WAS CALLED AT 10 00 P.M. WHEN HOIST WENT DOWN AND CALLED AGAIN AT 11:15 PM WHEN HOI ST WAS RETURNED TO SERVICE.
A MINE FIRE OCCURRED IN AN AREA ADJACENT TO THE UNDERGROUND SHOP. THE MINE WAS EVACUATED WITH 17 MINERS BEING EVACUATED FROM THE WEST FORK SHAFT AND 7 MINERS, INCLUDING 5 CONTRACT MINERS, WER E EVACUATED LATER THRU THE FLETCHER PRODUCTION SHAFT AFTER MINE RESCUE TEAMS EXTINGUISHED THE FIRE. MSHA WAS NOTIFIED IMMEDIATELY.
HOIST STARTED PULLING HIGH AMPS AND CONTACTOR BURNED UP ON THE LOWER SIDE IN THE MOTOR CONTROL CENTER. ELECTRICIANS WERE CALLED TO REPAIR HOIST AND HOISTMAN CALLED MSHA 11:40 AM, HOIST WAS REP AIRED AND RETURNED TO SERVICE SAME DAY.
DURING AN INSPECTION OF HOIST BY SURFACE SHOP, THEY TRIPPED A SLACK ROPE SWITCH TO TEST IT & IT WOULD NOT RESET. MSHA WAS CALLED AT 4 P.M. & ELECTRICIANS CHECKED THE SWITCH, REPAIRS WERE MADE & MSHA WAS CALLED BACK AT 4:40 P.M. ON SAME DAY & TOLD HOIST WAS BACK IN SERVICE.
HOISTMAN LOST BRAKE PRESSURE ON AUTOMATIC, BUT COULD RUN ON MANUAL SURFACE SHOP THE HYDRAULIC TANK & CLEANED THE SCREENS. MSHA WAS CALLED AT 9:40 A.M. AND TOLD HOIST WAS DOWN. AFTER MAINTENA NCE WAS COMPLETED, HOIST WAS RETURNED TO SERVICE & MSHA WAS CALLED & TOLD HOIST WAS BACK IN OPERATION AT 1:25 P.M.
TWO EMPLOYEES SENT A NEW MUCK BUCKET UNDERGROUNDON NORTH SIDE OF #30 SHAFT. THE BUCKET HUNG UP ABOUT 70' DOWN & CAUSED HOIST TO KICK OUT SLACK ROPE SWITCH. TWO CENTRAL SERVICE MAINTENANCE MEN WENT DOWN TO FREE UP BUCKET WHICH WAS BROUGHT UPTO LANDING AT 4:25 PM AND SHAFT INSPECTION WAS MADE. MSHA WAS CALLED AT 4:30 THE SAME DAY.

AT 12:40 PM ELECTRICIANS LOCKED OUT #31 HOIST SO MAINTENANCE COULD PEFORM A SCHEDULED GEAR OIL CHANGE AND TH EHOISTMAN CALLED MSHA AT 12:55 AND TOL DTHEM HOIST WAS DOWN. MAINTENANCE WAS COMPL ETED AND HOIST RETURNED TO SERVICE AT 5:45 PM AND MSHA WAS CALLED AND TOLD HOIST WAS BACK IN SERVICE. WEST FORK HOISTMAN WAS NOTIFIED AT START AND END OF THIS SERVICE.
A NON-DESTRUCTIVE ROPE TEST WAS PERFORMED. PRELIMANARY FINDINGS INDICATED A BETTER THEN 10% LOSS OF LMA. THE LAST TEST WAS 4.2 IN SEPTEMBER 99. THE HALKINS MACHINE AHD DETECTED CORROSION OF THE INNER WIRES. THE CORROSION WAS OBSERVED WHEN THE ROPE WA DISASSEMBLED.
OVERTRAVEL SWITCH IN HEADFRAME ON #34 HOIST FAILED IN THE SAFE POSITION. MSHA WAS NOTIFIED AT 10:30 AM AND AGAIN AT 11:00 AM WHEN HOIST WAS BACK IN SERVICE.
#30 HOIST WENT DOWN AND MSHA WAS CALLED AT 10:08AM. ELECTRICIANS AND MECHANICS REPLACED CHECK VALVES BETWEEN BRAKE PUMPS AND RETURNED HOIST TO SERVICE AT 2:18 PM SAME DAY. MSHA WAS CALLED AT T HAT TIME AND TOLD HOIST WAS BACK IN SERVICE. WEST FORK MINE WAS CALLED AT 10:10 AND AGAIN AT 2:22 PM SAME DAY TO LET THEM KNOW STATIS OF HOIST.

DURING INSPECTION ON #2 SHAFT, WE DISCOVERED THE BRACKET BROKE ON SOUTHEAST DOG ON SOUTHWEST CORNER. BRACKET APPEARS TO BE BROKE DUE TO YEARS OF SERVICE, METAL WEAR AND METAL FATIGUE. MSHA-ROL LA WAS NOTIFIED AT ABOUT 3:00 PM.

A CLARK FORKLIFT WITH FORK PUSHED TOGETHER WAS BEING USEDTO HELP POSITION 3RD MEMBER & SUSPENSION PARTS A S EE POSITIONED BOLT INTO RUBBER SPRING MOUNTINGSUPPORTS THE FORKS ON THE FORKLIFT SPREAD ALLOWING 3RD MEMBERS TO FALL PINCHING EE'S LEFT HAND EE'S INDEX FINGER HAD TO BE AMPUTATED AT THE MET
REPORTABLE: EMPLOYEE STATED ALLEGED LOSS OF CONSCIOUSNESS. EMPLOYEE WAS UNROLLING STEEL FENCE BY KICKING THE ROLL. HE FELT A SHARP PAIN IN LOWER BACK.
TWO EMPLOYEES WERE USING THE UTILITY TRUCK TO HANG VENT BAG IN 696 N/E-850. ONE EMPLOYEE RAISED THE BASKET UP WHILE HOLDING VENT BAG IN OTHER HAND. HE HEARD INJURED CALL FOR HELP, TURNED AND SAW HIM PINNED BETWEEN THE WORK BASKET RAILING &THE BACK.
WHILE HAND MINING HE PUT PRESSURE ON SCALING BAR AND FELT PAIN IN HIS LOWER BACK. X-RAYS WERE NEGATIVE, BU EE BEGAN PHYSICAL THERAPY ON 10/11/00.
EMPLOYEE WAS COMPLETING AN ELECTRICAL CONNECTION WHEN A CONDUCTOR TO GROUND SHORT OCCURRED IN A SEPARATE ENCLOSED JUNCTION BOX. A SMALL SCREW HAD ABRADED THE CONDUCTOR INSULATION. A GROUNDED C ONDUCTOR CAUSING AN ELECTRICAL FLASH BURN IN HISLEFT HAND AND FOREARM, 2ND AND 3RD DEGREE BURNS.
EE HAD AN ACCIDNE T WALKING ACROSS THE PARKING LOT, TRIPPING OVER A CONCRETE PARKING BLOCK. EE WAS TAKEN TO DR.'S OFC. AND HIS XRAY WAS NEGATIVE , EE RETURNED TO WORK FULL DUTY. EE'S KNEE DID NOT IMPROVE. WAS SENT TO ORTHOPEDIST FOR AN MRI.MRI SHOWED FLOATING CARTILAGE ON RT. KNEE. EE OPERATED ON 11-15-00. LOST TIME BEGAN ON 11-15-00.
EE HAD A TRUCK ACCIDENT HITTING A PILLAR. INJURIES INCLUDED A BRUISED FOREARM AND A BRUISED LEFT KNEE. EE WAS TAKEN TO DR. FOR XRAYS, WHICH WERE NEGATIVE. KNEE DID NOT IMPROVE. EE WAS SENT TO ORTHOPEDIST WHO DID AN MRI, WHICH SHOWED THAT EE NEEDED TO HAVE SURGERY ON THE KNEE FOR SPUR FORMATION AND FLOATING CARTILAGE. SURGERY WAS DONE ON 10-30-00.
EE HAD BEEN SCALING FROM A PETTIBONE BASKET. THE BASKET HAD BEEN LOWERED BY THE PETTIBONE OPERATOR TO MOVE TO ANOTHER LOCATION. EE THAT WAS INJURED HAD HIS LEFT HAND ON TOP RAIL OF BASKET FRAM WORK (ABOUT WAIST HIGH). PETTIBONE OPERATOR WAS INTENDING TO MOVE PETTIBONE IN REVERSE. OPERATOR PLACED CONTROL IN FORWARD INADVERTENTLY. THE BASKET HIT AGAINST A PILLAR, PUSHING THE BASKET R
EE SLIPPED & FELL DOWN STAIRWAY BETWEEN PRODUCTION SHAFT & CHANGE ROOM. SURGERY TO REPAIR RIGHT SHOULDER ON 1/31/01 RESULTING IN LOST TIME DAYS.
EE CLAIMS WHILE SCALING OVERHEAD, HE PULLED MUSCLE IN LOWER BACK. FIRST DAY OF LOST TIME IS 1/24/00. (MODIFIED REPORT BASED ON DOCTOR'S DIAGNOSIS).
FRACTURE OF FIRST JOINT ON LEFT HAND LITTLE FINGER, WHILE HAND MINING WITH SCALING BAR FROM BASKET IN BOOM TRUCK, PINCH POINT BETWEEN BASKET AND SCALING BAR.
EE WAS REMOVING DAMAGED DOOR GLASS FRAME FROM A WRECKED TRUCK, WHEN FRAME SLIPPED IN HIS RIGHT HAND AND A PIECE OF GLASS STRUCK AND CUT HIS RIGHT ARM. INJURY REQUIRED 5 SUTURES.
EMPLOYEE WAS ASSISTING WITH MAINTENANCE ON A LOADER. EMPLOYEE WAS IN A CONFINED AREA AND WHILE MOVING HEAD AND ARMS HE STRUCK HIS MOUTH ON A FUEL TANK CAP RESULTING IN CHIPPING BOTTOM OF RIGH T FRONT TOOTH.
EMPLOYEE WAS USING AIR PRESSURE TO CLEAN OUT DRILL HOLES. MUCK BLEW OUT OF DRILL HOLE AND HIT ENMPLOYEE IN THE FACE, WITH SOME MATERIAL GOING UNDER SAFETY GLASSES INTO EMPLOYEE'S EYES. NO IMME DIATE DISCOMFORT NOTICED. LATER IN THE EVENING, HIS EYE BECAME VERY IRRITATED. EMPLOYEE WENT TO E.R. AND A SMALL PIECE OF PARTICLE DUST WAS REMOVED. ANTIBIOTIC DROPS PRESCRIBED.
EMPLOYEE HAD BEEN CHARGING DURING SHIFT. DURING MEAL BREAK EMPLOYEE'S RIGHT EYE STARTED ITCHING.EMPLOYEE RUBBED EYE, WHICH THEN BECAME IRRITATED. EMPLOYEE FLUSHED EYE. WAS TAKEN FOR MEDICAL TR EATMENT AT NEARBY HOSPITAL ER. ANTIBIOTIC EYE DROPS PRESCRIBED.
EMPLOYEE WAS REMOVING GUARD OVER DRIVE COUPLING WHILE THE SHAFT WAS STILL COASTING. HE LIFTED GUARD; IT CAUGHT ON DRIVE COUPLING AND WAS THROWN AGAINST HIS LEFT FOREARM. THIS RESULTED IN A L ACERATION REQUIRING APPROX. 15 SUTURES. HE RETURNED TO WORK THE SAME DAY.
THE EMPLOYEE REPORTED STEPPING ON AN UNSECURED 2' BY 3' SECTION OF METAL GRATING AND FALLING APPROX TWO FEET TO THE GROUND BELOW. THE EMPLOYEE'S RIGHT LEG CONTACTED PROTRUSIONS OF THE SECURED GRATING THAT WAS ADJACENT TO THE UNSECURED SECTION RESULTING IN A LACERATION. MULTIPLE SUTURES REQUIRED. EMPLOYEE RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT.
EE WAS WELDING ON HAND RAIL ON VOLVO LOADER. EE STRUCK ARC BEFORE HOOD WAS FLIPPED DOWN. AS EE FLIPPED HOOD DOWN, IT HIT A PIPE NEAR HIS HEAD, AND THEREFORE DID NOT COMPLETELY CLOSE. EE HAD TO MOVE HEAD BACK TO CLOSE HOOD. THE EVENT PROBABLY TOOK 2-3 SECONDS. A FLASH BURN TO BOTH EYES RESULTED. EE RETURNED TO WORK ON HIS NEXT SCHEDULED SHIFT.
EMPLOYEE WAS STEPPIN GOFF OF EQUIMENT AND FELL TO GROUND HITTING AND TWISTING HSI LEFT KNEE. EMPLOYEE WENT TO THE DR ON 4-27-00. FLUID WAS TAKEN OFF OF KNEE.
EMPLOYEE REPORTED TO HIS SUPERVISOR THAT HE HAD EXPERIENCED PAIN IN BOTH HANDS, WRIST, AND ELBOW. BOTH ARMS GOING NUMB AT TIMES TOO. EMPLOYEE WENT TO DOCTOR AND DIAGNOSED WITH CARPAL TUNNEL SY NDROME.
E WAS INSTRUCTED TO TEST AND REMOVE WIRING COMING OUT BOTTOM OF ELECTRICAL FAN BOX. EE STATED HE TESTED ALL WIRES AND TEST SHOWED THEY WERE NO HOT. EE REMOVED BOTTOM WIRE AS INSTRUCTED. HE STA RTED TOREMOVE TOP WIRE (WHICH HE WASNOT INSTRUCTED TO DO) AND RECEIVED FLASH BURNS TO RIGHT FOREARM AND LEFT WRIST AREAS. DIAGNOSED WITH FIRST DEGREE BURNS TO RIGHT FOREARM.
