Accident type, without injuries

November 12, 2014 at approx. 8 pm, hoist motor failed. Electricians were called and found bad motor. MGset motor had to be replaced. Repairs were completed on 11/13/2014 at 8:19 pm. Hoist/shaft inspection were made and put back in service at that time.

Plant power kicked off at 7:15 am on 2-6-14. Plant power was restored at 7:20 am but hoist power remained tripped. electricians worked on problem with main breaker, Called MSHA to report hoist power outage. Main breaker was repaired and hoist was running at 8:17 am. Second call made to MSHA to report problem resolved at 8:27 am on 2-6-14

Struck by falling object

Rock fell from back and hit employee on hard hat.

Fall from ladders

EE was loading a concentrate truck with a crane. The cable on the crane tipped the ladder over that she was on causing her to fall to the scale below. EE received an open fracture of the left patella.

EE grabbed the hose for the 30W oil to unwind the holes off the reel when the hose ruptured in his left hand. Treatment of injection injury was given. EE was released to restricted duty following procedure.

Over-exertion in wielding or throwing objects

EE was swinging a sledge hammer to tighten a hitch nut on a truck when he felt sharp pain in stomach. EE was taken to company dr and placed on restricted duty until seen by a surgeon. Surgery was scheduled by surgeon for 11.20.14. EE ""should"" be able to return to work on restricted duty the following Monday.

EE felt pain in stomach while driving on bumpy road. No Doctor trip was needed at that time. EE went to Dr on 3.25.15 and was diagnosed with a hernia. Further treatment with is pending.

EE was wrapping a pallet with shrink wrap and felt pain right arm, elbow, and wrist. Initial ER visit released back to work full duty. Follow up visit on 11.13.14 put her on restricted duty.

EE was exiting boulder breaker, his foot slipped off the step and he caught himself with his arms, he felt pain in his back. EE was placed on restricted duty.

EE stepped on water hose and twisted right ankle. EE was placed on restricted duty.

EE slipped in shower. Restricted duty for concussion symptoms

EE was operating a grader, when put into reverse the grader jerked and EE felt pain in neck. EE received restricted duty per company Dr.

EE tripped and fell down 3 steps. EE strained shoulder, back, and ankle.

EE fell from step on a 4' step ladder - felt pain in low back. Restricted duty for 13 days.

EE was turning off water hose. Torn tendon in left thumb requiring surgical procedure to repair.

EE was walking out of lunchroom when he lost his balance and twisted his right knee.

Employee was clearing out a shaker using an eight foot pry bar to loosen the rocks from the chains. One of the rocks fell from around four feet from the shaker striking the pry bar. The bar then smashed the upper area of his right foot causing a broken metatarsal.

Employee was loading tools into pickup truck and jumped from tailgate to ground and injured right knee

EE was exiting drill when his foot slipped, felt pain in Right shoulder. Dr prescribed pain medication.

EE slipped on snow covered ice, felt pain in right shoulder, EE had surgery 5.8.14.

EE was driving a haul truck underground when his light belt hit the bed lever, the bed raised and hit the back suddenly stopping the truck. When the truck stopped suddenly, EE hit his mouth on the steering wheel causing a laceration. EE received stitches and was returned to work at full duty.

EE was moving office furniture when the leg broke on the desk tipping it. When it tipped the glass top slid sideways, when he reached to grab the glass it lacerated his left index finger requiring stitches.

EE was walking to mill when something blew in his eye. EE was taken to eye dr. and prescribed eye drop to prevent infection to scratch on eye. Right eye.

Absorption of radiations, caustics, toxic and noxious substances

EE was changing a pop off valve on the SO2 tank. The gas leaked around the rupture disc assembly. The EE received only first aid treatment at that time. On 11-4-14 EE still having irritation in eye, EE taken to eye dr. Eye dr. prescribed eye drop to alleviate discomfort and returned to work.

Imbedded debris in left eye. EE was under a truck removing bolts when debris fell into eye. EE was taken to eye dr and a piece of metal was removed from eye.

Employee was carrying a piece of plate steel with assistance from a co-worker. They lost their grip and dropped the plate. It came down on the tip EE's right ring finger. He received 1st aid treatment and returned to work. He was not taken to the doctor until 8/26 because he did not make himself available for treatment. Finger broken and light duty status.

EE was moving a water hose and lost his footing falling. EE received fracture to left middle finger.

Employee was not wearing gloves picking up pipe and received cut on right hand.

EE was changing a dart valve on the CU rougher, the cap on the dart actuator was not secured and flew off hitting EE in the mouth. EE broke a tooth and chipped another.

While EE was blowing dust and debris with air hose, EE got something in left eye. Dr removed small piece of metal imbedded in left eye.

EE got foreign body imbedded in left eye. Piece was removed and prescription eye drop was given to EE.

Employee shut his hand in a haul truck door, laceration to left thumb requiring stitches.

EE was opening a box with a box cutter when the box cutter slipped, cutting employees left thumb, requiring stitches.

Alleged Carpal Tunnel Syndrome. Remains on full duty at this time and has not yet been determined as work related (3.31.15).

EE alleged carpal tunnel syndrome. Dr diagnosed EE with carpal tunnel and set up surgery for 1/15/2015. EE will be released to Light duty following procedure on 1/15/2015.

EE diagnosed with Carpal Tunnel Syndrome on 12.08.14. EE will remain on full duty until after surgery. Surgery has not been scheduled at this time.

EE alleged carpal tunnel. EE treated with conservative care. Dr recommended surgery on cubital tunnel syndrome with ular nerve. Surgery is scheduled for 2.6.15, EE will return on restrictive duty following surgery.

EE alleged carpal tunnel on 5/13/2014. EE treated with conservative care until dr recommended surgery. Surgery is not yet scheduled. EE will have light duty restrictions after surgery.

EE was breaking bolts loose on a transmission when he felt pain in right elbow. EE has been on full duty since that date. Dr has determined surgery is needed and will is scheduled on 4/2/2015. EE should return on 4/6/15 on restricted duty.

Carpal Tunnel Syndrome in both hands.
