Accident type, without injuries

#2 Service hoist went down at 2:30 am on 8/27/16 due to a burnt coil. Electrical was called and coil was replaced. Hoist was placed back into service at 4:47 am on 8/27/16.

Caught in, under or between a moving and a stationary object

EE was performing maintenance on a liner in the mill building, the liner shifted and mashed three fingers on right hand. EE currently seeking treatment.

Struck against a moving object

EE received fractured ribs when EE failed to negotiate a curve in a haul truck underground.

Contact with hot objects or substances

A crack in a fuel tank on a dump truck was being repaired by a welder when the tank exploded. The nearby driver suffered 2nd and 3rd degree burns. The driver is at St. Johns Mercy Hospital in St. Louis, MO.

Struck against a moving object

EE claimed a personal illness beginning on 8.22.16. EE missed time for illness. On 5.1.17 an email from the company outside attorney came claiming the illness is a possible work related illness or aggravation of an illness. EE is currently on restricted duty. Will amend when more information on case is known.

Struck against stationary object

EE getting out of a Kawasaki Mule when EE stepped on a rock and twisted right knee. EE placed on restricted duty pending treatment.

EE foot slipped while exiting tractor, twisting right knee. EE placed on restricted duty.

Struck against a moving object

EE was scaling the back when the tooth slipped and jarred the cab of the mechanical scaler. EE was placed on restricted duty on 6.17.16. Further testing is pending.

EE slipped on pavement at truck scale and strained hamstring in left leg. EE placed on restricted duty.

Struck by falling object

EE training another employee in the skip pocket when EE was hit in the head with a rock, causing a cervical strain. EE was placed off work pending further testing.

Over-exertion in wielding or throwing objects

EE felt pain in low back area while swinging a hammer to drive out a swing cylinder pin. EE placed on restricted duty for muscle strain and was released on 11.1.16.

Caught in, under or between a moving and a stationary object

EE got thumb on right hand stuck between a box of powder and the railing on the platform. EE placed on restricted duty.

EE was wiping the trunnion bearing on the ball mill when ee felt a pain in middle back. EE was placed on restricted duty pending treatment.

Fall to the walkway or working surface

EE reattaching a guard when the lifting hook came loose and hit EE in the chin. While exiting ladder, EE missed a step and fell to the ground. EE placed on restricted duty for knee pain.

Struck against a moving object

EE operating a truck underground when the truck bed hit the back. The EE hit the steering wheel. Minor injuries at the time. Incident became reportable when EE was placed on restrictive duty on 8/18/16 with neck pain.

Over-exertion in pulling or pushing objects

EE was tamping powder into a drill hole when EE felt pain in shoulder. EE was placed on restricted duty on 7/20/2016, pending further treatment.

Fall onto or against objects

EE cleaning rock out of plugged chute, employee lost footing when the splitter car moved causing employee to fall into a handrail, resulting in 3 fractured ribs. Employee currently on restricted duty.

Fall from machine

EE fell from steps when exiting mechanical scaler. EE was placed on restricted duty for pain in left hand/wrist.

EE climbing into haul truck cab, strained right shoulder. EE was placed on restricted duty on 6/10/16.

Over-exertion in pulling or pushing objects

EE was diagnosing a maintenance issue with a dozer when EE felt a pop and pain in EE's right shoulder. No medical treatment at that time. 8.26.16 EE went to company dr. and was placed on restrictive duty. Treatment is pending.

Struck against stationary object

EE tripped on a rock in a water hole underground. EE did not seek medical treatment until 6/6/16 and was placed on restricted duty at that appt.

EE performing maintenance on a drill. When exiting the drill, EE felt pain in knee stepping off the ladder. EE placed on restricted duty pending further testing.

Over-exertion in lifting objects

EE lifting a piece of steel and felt pain in left shoulder. EE placed on restricted duty pending testing on 5.11.16.

Struck against stationary object

EE was walking to the park to get a haul truck to drive to shop to make repairs when EE stepped on a rock and twisted knee. EE was placed on restricted duty on 5.5.16 and will require surgery. Surgery is pending for 5.16.16.

Struck against a moving object

EE felt pain in low back region while driving haul truck underground after hitting a pot hole. EE went to the physician on 4-15-16 and was placed on restricted duty.

EE climbing in and out of rod mill, twisted right knee. Surgery pending on 5.23.16

EE exiting drill when EE missed the step and fell to the ground. EE felt pain in lower back and was placed on restricted duty for 2 days following incident.

Struck against a moving object

EE operating a haul truck when the operators seat malfunctioned. The seat dropped down causing EE to feel pain in back. EE placed on light duty restrictions for one week.

EE stepped off getman scaler and twisted right ankle. EE placed on restricted duty on 2/05/16 and returned to full duty on 2/12/16.

EE received lacerations requiring stitches on two fingers of left hand after reaching in to adjust an engine and cut fingertips on fan blade.

EE received a laceration on left ring finger while installing belts on a shaker motor. EE received stitches and was released back to work.

EE hand scaling from a basket, a rock fell striking EE in the head causing a laceration. EE received stitches and was returned back to full duty.

EE stung by bee on the arm. No allergic reaction, no medical treatment necessary.

EE alleges feeling pain in low back while operating a loader. Investigation and treatment are pending at this time.

EE in a heading when a piece of rock fell striking EE in the head. EE received sutures and released back to full duty.

A crack in a fuel tank on a dump truck was being repaired by the welder when the tank exploded. The welder suffered 1st degree burns. EE was taken to St. Johns Mercy Hospital in St. Louis, Missouri for treatment.

EE stepped up on the bottom step to climb up into a loader when the step broke and a piece of metal lacerated EE's left leg requiring stitches.

Struck by flying object

EE hit a chute liner with a hammer to loosen it, EE felt something in EE's eye. Medical personnel did not find anything in eye, but was written a script for eye medication.

Caught in, under or between a moving and a stationary object

EE was shutting metal doors. EE had closed the left hand door when the air pressure caught the right hand door and caught EE hand between the door. This caused a laceration on right hand requiring 4 stitches. EE released back to work all full capacity same day.

Struck by falling object

EE moving tables, dropped a table hitting EE's right hand/wrist. EE was released to full duty. EE received a splint to keep on EE's wrist to partially immobilize.
