Accident type, without injuries

Power went out causing hoist to go down at 8:40 A.M. Problem was with electric company (Amerun UE). Hoist was back in service at 11:30 A.M. MSHA was notified of power outage and when power was restored.

Main breaker in sub-station tripped. When they first tried to reset breaker, it wouldn't stay in. Hoist went down at 9am. Msha was notified at 9:30am. Hoist was down and came back up at 3:30pm

Loss of power to 30 & 31 hoist at 8:30 pm. Called MSHA at 8:55 pm. Electricians came in & checked power & they found problem & they reset power. Hoist called MSHA back at 10:55 pm to report hoist back in service.

EE was operating a mechanical scaler when he heard a loud boom. Scaler caught on fire and mine was evacuated at that time. It is beleived that this was an electrical flash fire & self-extinguised in less that 30 minutes. We notified MSHA at the time because it was thought we had a reportable mine fire. Upon further investigation, it revealed it probably was not.

Struck by flying object

Driver dumped load and turned on hose to wash out truck bed. Hose was not shut off at hozle and pressure made hose fly up and hit driver in cheek under right eye.

Fall onto or against objects

Two employees were attaching 8"" cement hose to surface cement hole. They were in basket of JCB to make hook up. Boom was extended out approx. 20' to reach pipe at cement hole when the basket boom tipped over & fell to ground approx. 25'. They were taken to Dr. & released to work. Injured EE continued to have pain in his rib/back area. Prescribed a cortizone shot & therapy.

Employee was dumping wet pb over rail into pb high grade sump. too much weight in basket causing it to tip forward into sump. He was taken to co. dr. and after continuous treatment he has surgery on august 28, 2006 making this a lost time incident.

While driving to Casteel with road marked as an escape route. Road had been shot out and not properly barricaded, allowing Ee to drive off of an 8' bluff in chest deep water. EE hurt leg and knee. He was taken to county Dr and to an orthopedic specialist. He is now off work beginning 7/3/06 waiting for knee surgery.

Struck against a moving object

Employee was operating small Bobcat mucking shaft bottom. Rear end of bobcat came off the ground and dropped back down jarring employees neck and upper shoulder. Employee was seen by doctor and released to come back to work. Employee had neck surgery on 10/04/2010 and began losing time.

Employee was getting on cage when door of cage went through shoe right behind steel toe hitting left foot. Foot instantly began to swell. Employee was taken to company dr. on 05-13-06 and was diagnosed with a broken toe. Employee is off work.

Boulder hung in load chute in skip pocket. Boulder was too big to go thru the load out system getting thru grizzly. While trying to move cable, hook slipped off boulder hitting the employee across the nose. Employee was taken to emergency room where he was diagnosed with a crushed bridge and would need surgery. Employee is currently off work.

Contact with hot objects or substances

Employee was scaling heading when he heard a loud boom. He realized machine was on fire and got out. He received burns to face, hands and right thigh. Employee was taken to ER and transported on to St. Louis. Employee remains off work.

Over-exertion in pulling or pushing objects

A number of hole plugs had been driven over by drill & compacted very tight. While pulling on one with pliers, he felt a strain to his left elbow. He continued working & noticed swelling, as the swelling continued the pain set in & increased. Employee was taken to the E.R. where he was given 2 injections. One for pain & the other was an antibiotic.

Caught in, under or between a moving and a stationary object

Employee was pulling a piece of loose off of a pillar. The loose gave way causing the mining bar to move and smash his left ring finger between the mining bar and multicrane basket. Employee was taken to Salem ER. Employee fractured tip of left ring finger.

Absorption of radiations, caustics, toxic and noxious substances

Compressed air line to shotcrete nozzle appeared to be clogged when air was turned on. The employee noticed air line started to come off nipple, he tried to shut the compressor off & the line came off & squirted accelerant in his eye. EE was taken to the company Dr. & prescribed an antibiotic.

Two employees were attaching 8"" cement hose to surface cement hole. They were in basket of JCB to make hook up. Boom was extended out approx. 20' to reach pipe at cement hole when the basket boom tipped over & fell to ground approx. 25'. EEs were taken to Dr. & released to work. Injured EE chipped his 2 front teeth & on 10/9/06 he received 2 temp. caps making this reportable.

Flash burns (electric)

Employee went to remove lock out and turn breaker on. When he turned on the rod mill breaker it blew up blowing breaker doors open. He burnt his right arm and bruised right forefinger. He was taken to co dr and prescribed an antibiotic ointment for the burn.

Employee was loading powder in down holes. While cutting blasting wire toward body, the force he was using caused the knife to continue to his forearm causing a laceration. He was taken to salem emergency room where he received 11 sutures to inside of left forearm.

Fall to the walkway or working surface

Employee was walking across slick floor when he fell down and hurt his back. EE has had continuous treatment with company Dr. and specialist. Employee had an epidural injection on 11/3/06 and will began physicial therapy 11/14/06.

Employee was removing worn gear from ZN filter when the sharp edge of the gear cut his right thumb. He was taken to company dr. where he received 7 sutures.

Absorption of radiations, caustics, toxic and noxious substances

Grout mix was too thick causing hose to rupture under pressure. Grout got in left eye. Employee was taken to Salem E.R. where he was given an antibiotic. He was then taken to an eye doctor where he was given additional antibiotic.

Employee was hand mining when the tip broke on the mining bar and had a sudden jar to right side. Employee has been treating with company doctor and then was referred to an orthopedic specialist. He was given a cortisone shot on 06-20-06 and was also prescribed physical therapy.(ee shoulder did not get better, surgery on 3/27/07 to make reportable)

Struck against stationary object

Employee was climbing down off of back fill and rocks. When rock gave away, He lost footing and caused him to twist right knee. He was taken to co. dr and after continuous treatment, He had knee surgery on august 25,06. He will return to work on next scheduled shift august 30, 06.

Struck by flying object

Employee was operating a loader when company co-worker tossed a rock at loader and it hit the right side loader window. Window shattered and glass went into the employee's palms. Employee was taken to company doctor on 3/7/06 and a fragment was removed from the left palm.

Fall to the walkway or working surface

EMPLOYEE WAS CARRYING A BOX OF POWDER TO CHARGE BLUFF HOES WHEN HE TRIPPED ON ROCK, FELL FORWARD & CAUGHT HIMSELF. WHEN HE FELL HE CUT PALM ON RT. HAND. EE WAS TAKEN TO E.R. WHERE HE HAD 3 SUTURES MAKING THIS REPORTABLE.

EMPLOYEE WAS HOLDING THE 500 MM CABLE WHILE HIS PARTNER WAS CUTTING THE SHEETING WITH A UTILITY KNIFE. KNIFE CUT THROUGH THE SHEETING CUTTING EMPLOYEE'S RIGHT HAND BETWEEN KNUCKLE OF INDEX & MIDDLE FINGER. EE WAS TAKEN TO E.R. WHERE HE WAS GIVEN 7 SUTURES.

EMPLOYEE WAS HAULING 2 INCH PLASTIC PIPE TIED WITH A ROPE APPROXIMATELY 3 FEET FROM THE END. PIPE WAS IN A BIND AGAINST SIDE OF PETTIBONE, WHILE HE WAS HOLDING PIPE WITH RIGHT HAND LOWER THAN WHERE PIPE WAS TIED TO KEEP PIPE FROM SPRINGING BACK TOWARD FACE OR BODY. KNIFE CUT THROUGH ROPE EASIER THAN EXPECTED AND WENT DOWNWARD INTO THE RIGHT HAND REQUIRING 4 STITCHES.

Over-exertion (Not Elsewhere Classified)

EE reported alledged carpal tunnel syndrome on 12-19-06. He was seen by specialist on 2/14/07 that determined he needed surgery. He had surgery for the right hand on 4/12/07 and will have the left hand done on 4/26/07.
