Struck against a moving object

EE was riding in a buddy seat of a haul truck and felt pain in the upper back. EE was taken for medical attention at that time. EE remains on restricted duty.

Struck by flying object

Apx. 760' HPDE pipe was being pulled down an access road. Around a 100 degree bend, the pipe developed resistance from unidentified debris (presumed rock). The resistance gave way and the tension applied from pulling the pipe caused it to straighten striking and employee inspecting for holes.

Struck by flying object

Spring bolt sheared causing employees wrist to hit hand rail behind work area. Received a fracture to left forearm.

Struck by concussion

EE noticed the vent tube waving and EE's ears began popping and the door on the #5044 haul truck slung open while EE brought EE's truck to a stop. A planned blast took place and the haul route was affected. There were no injuries observed. EE has had multiple doctor visits to diagnose issues with eye and ear discomfort and to explore if there was any damage present.

Over-exertion in lifting objects

EE MOVING BATTERIES ON A PALLET AND FELT PAIN IN LEFT SHOULDER. EE SOUGHT MEDICAL TREATMENT, TESTING SHOWED TEAR IN ROTATOR CUFF THAT WILL REQUIRE SX. EE REMAINS ON FULL DUTY AT THIS TIME. SX SCHEDULED 4.21.21 AND WILL BE RELEASED TO LIGHT DUTY FOLLOWING SX.

EE CLIMBING LADDER ON HAUL TRUCK TO INSPECT FIRE EXTINGUISHER FOR MONTHLY INSPECTION. EE FOOT SLIPPED ON LADDER, STRAINING LEFT ARM. EE PLACED ON RESTRICTED DUTY ON 11/3/2021.

Over-exertion in lifting objects

EE using a mining bar to pry a loose rock to access a drill hole in the floor and felt pain in right wrist. EE was placed on restricted duty on 10/4/2021.

EE CHANGING BOOM ROLLERS, SLIPPED ON THE BOOM ROLLER COVER AND FELL ON LEFT SHOULDER AND RIGHT KNEE. EE PLACED ON RESTRICTED DUTY ON 8/16/2021

Struck against stationary object

EE twisted left knee stepping on a loose rock on the ground. EE placed on restricted duty on 07/12/2021 after requesting a medical evaluation.

EE STOOD UP TO STEP FORWARD AND RIGHT KNEE HYPER-EXTENDED. EE WAS PLACED ON RESTRICTED DUTY AFTER MEDICAL EVALUATION.

EE placed on restricted duty following a strain in groin area from lifting gates on the cage to put it back together after hoisting equipment.

EE PULLING ON A WRENCH TO BREAK LOOSE A BOLT ON A SEAT BELT AND FELT PAIN IN LEFT WRIST. EE TAKEN FOR MEDICAL EVALUATION AND WAS DIAGNOSED WITH A LEFT WRIST SPRAIN. EE PLACED ON RESTRICTED DUTY. EE FOLLOW UP ON MONDAY AND RELEASED BACK TO FULL DUTY.

EE STEPPED OFF DRILL AND TWISTED RIGHT KNEE. EE PLACED ON RESTRICTED DUTY ON 04/05/2021.

EE walking in park area doing a pre-operational equipment inspection and tripped on a rock falling and injuring right elbow. EE placed on restricted duty following medical examination.

EE got out of pick-up truck to open a gate on gravel road and slipped and fell on icy roadway. EE felt pain in left knee. EE seen at emergency room and released to restricted duty at that time.

EE pulling on wrench to tighten boom roller and felt pain in right shoulder. EE was seen for medical attention on 02/01/2021 and placed on restricted duty at that time.

EE WAS REACHING TO FLIP A VALVE, HAND SLIPPED OFF VALVE AND FELT PAIN IN LEFT SHOULDER. EE PLACED ON RESTRICTED DUTY AFTER MEDICAL TREATMENT ON 01/15/2021. EE WAS RELEASED TO FULL DUTY AFTER MEDICAL FOLLOW UP ON 01/18/021.

EE twisted the left knee while walking. EE sought medical treatment on 1.20.2021 and was placed in restricted duty at that time.

EE walking down stairs, fell down 2-3 steps. EE was placed on restricted duty following medical treatment on 01/26/2021. EE will begin physical therapy as well.

EMPLOYEE WAS USING A SCALING BAR TO PRY A MIDDLE DRIVELINE ON AN ST8 AND FELT PAIN IN RIGHT ELBOW. EE WAS PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

Struck by flying object

EE was was leaning over adjusting a portable air compressor hose when the quick connect ruptured striking the EE in the left ear. EE received sutures to the left ear and was released back to full duty.

EE ADJUSTING THE FORKS ON A FORKLIFT WITH A WRENCH, WRENCH HIT EE ON THE CORNER OF THE RIGHT EYE RESULTING IN SCRATCH TO RIGHT EYE. EE RECEIVED PRESCRIPTION EYE DROP AND RELEASED TO FULL DUTY WITH NO RESTRICTIONS.

When removing metal bands from roof bolts, metal band lacerated employees right thumb requiring stitches. Employee returned to full duty after seeking medical attention.

A sub-contractor hired by Rail-Veyor was listening to a hazard training presentation when they moved backwards and was un-aware of a mounted electrical motor framing in which they tripped over, resulting in hitting hard hat on the wall and falling to the floor causing a lower back injury.

EE was performing final pre-operational checks on EE's #0533 Volvo 40D haul truck. As EE was entering the truck cab, EE shut the door with EE's left hand from behind while facing forward going into the truck. The door caught EE's thumb causing it to split open in which EE received 10 stitches in treatment.

Caught in, under or between a moving and a stationary object

EE used jack to lift up forklift for service. EE putting blocks under forklift to secure load when the jack mechanically failed and smashed pinky finger on right hand between the board and forklift. EE received 12 sutures and was released back to full duty.

EE testing knife valve, blade came into contact with left index finger. Laceration to left index finger requiring 5 sutures. EE was released back to Full Duty following medical treatment.

EE diagnosed with occupational carpal tunnel syndrome. Surgery is recommended. EE will be released to restricted duty following surgery.

EE diagnosed with carpal tunnel syndrome in left hand/wrist. EE will have surgery on 2/11/2021 and be released to restricted duty following surgery.

EE attempting to sit on bench in men's change room and collapsed onto floor. Witness to event began emergency procedures. EE taken by ambulance to Hospital and pronounced deceased.
