Accident type, without injuries

1:00 pm #29 Mine hoist went down. After evaluation it was determined to be a skip hung in shaft. MSHA was called and evacuation procedures were in process. Evacuation process completed at 4:45 pm. Maintenance was completed to put shaft back into service on 12/30/17.

Accident type, without injuries

Upon inspection it was determined that a 10 foot guide beam in the vertical service shaft was loose. The decision was made to completely replace the beam and the service shaft will be taken out of service for more than 30 minutes to complete this repair.

Over-exertion in lifting objects

EE lifting water lines and felt pain in abdominal area. EE taken to local clinic and sent to ER for diagnosis and treatment. EE then transferred to Mercy Hospital in Festus Mo for surgery of hernia.

Over-exertion in lifting objects

EE lifting drill steel onto tractor and felt pain in groin area. EE did not seek medical treatment at the time of incident. Medical treatment on 7/11/17 placed employee on restricted duty pending evaluation and treatment for hernia.

Fall to the walkway or working surface

EE walking around piece of equipment underground when EE slipped. EE tried to catch self and felt pain in right shoulder. Testing is ongoing. EE placed on restricted duty while testing is in progress.

Fall onto or against objects

I was washing raise in wet conditions when rock slipped out from under footing. Fell forward into safety line that was attached to harness swung into rock pile on opposite side of raise. Then fell onto rock surface of raise. Broke number 11 rib on left side and small scratch on head.

Struck by falling object

EE moving a steel box when the plate clamp slipped and the steel box fell onto EE right foot fracturing a bone in right foot. EE released with light duty restrictions following ortho specialist exam.

EE hooking up bottle rack on crane, another EE bumped the crane control to go up a little bit and caught EE left index finger between crane hook and bottle rack. EE was taken to ER and received 9 sutures in left index finger. EE released to restricted duty.

Struck against a moving object

EE operating haul truck underground, bed raised while driving and stuck in back causing EE to hit steering wheel. EE was taken for medical treatment and cleared for light duty until seen by company doctor. EE was seen at company doctor on the first shift back, 11/13/17 and released to full duty.

EE lost left footing while standing on battery box. Right knee slid down between tire and loader twisting right knee. EE received medical treatment and was placed on restricted duty until follow up appointment on 11/10/17 and was returned to full duty.

EE felt pain in low back when lifting a 40 lb box of resin onto the roof bolter. EE went to the doctor on 8.14.17 and was placed on restricted duty at that time.

EE struck in arm with loader bucket. EE placed on restricted duty for arm contusion.

EE stepped onto bottom rung of ladder and felt pain in right knee. EE placed on restricted duty for knee sprain.

Over-exertion in pulling or pushing objects

Cleaning a tank and upon completion the laborer-miner was pulling a shop-vac out of the tank. It became snagged on the rim and the worker attempted to pull harder to release the vacuum from the edge of the tank, it did not release and EE strained lower back.

Fall to the walkway or working surface

EE attempting to clean at #5 crusher train dump when EE lost footing and fell into open hole. EE received sutures in a laceration on bottom lip and was placed on restricted duty for knee pain.

Struck against stationary object

EE was pulling on a wrench to loosen a cartridge when the cartridge broke loose causing EE to hit elbow on the fender. EE was placed on restricted duty on 6/14/2017.

Struck by flying object

EE removing rock from under coarse ore bin when rock came in cab and hit ee on the right arm and hand. EE received laceration and sutures and placed on restricted duty.

Over-exertion in pulling or pushing objects

EE pulling on a water hose to wash off entrance to truck wash and felt pain in low back area. EE went to local clinic on 3.22.17 and was placed on restricted duty at that time.

Fall to the walkway or working surface

EE WAS PULLING A ROPE ON A TROLLEY BEAM WHEN THE ROPE BROKE CAUSING EE TO TWIST RIGHT ANKLE. EE WAS PLACED ON RESTRICTED DUTY WHILE TESTING WAS PERFORMED, DIAGNOSED WITH A FRACTURE OF RIGHT ANKLE AND EE REMAINS ON RESTRICTED DUTY.

EE prying with a scaling bar to unplug a boulder from a chute and felt pain in right shoulder. EE did not seek any medical treatment at that time. EE sent for medical treatment on 2.21.18 and was released with no restrictions. A follow up appointment on 3.22.18, the EE was placed on restricted duty at that time.

Over-exertion in lifting objects

EE lifting 50 lb bags of chemicals and felt pain in abdomen. CT scan revealed a hernia. EE being referred for treatment. EE placed on light duty pending treatment.

Struck by falling object

EE letting clothes basket down in changeroom when deris fell off basket and landed in EE left eye. Debris was imbedded and was removed by dr. and prescribed antibiotic eye drops. EE was released back to full duty following medical care.

Struck by falling object

Contractor came on site to update Ethernet switch. Contractor was hit in head by an Ethernet switch requiring medical treatment. Contractor received 5 sutures in back of head and returned to finish job. This is not our employee and not required to have an MSHA ID number.

Struck by flying object

EE removing a pinion from Cat Loader when the part hit employee on the face causing a laceration to left eyebrow. EE received 4 sutures and was released back to full duty.

EE using a utility knife to cut a tie wrap on a power cable when the cable moved causing a laceration to EE right hand requiring sutures. No restricted duty.

Struck by flying object

EE got a foreign object in eye while welding. Nothing was imbedded in eye, however, EE was given a prescription eye medication to prevent infection.

Received email (1/18/2018) with diagnosis of Occupational Carpal Tunnel Syndrome on 12/8/2017. Treatment is pending at this time. EE will be released with restricted duty following surgery.
