#34 HOIST WENT DOWN AT 7:20 AM. MSHA WAS CALLED AT 7:35 AM. SURFACE MAINTENANCE REPLACED 3 BUSHING IN GEAR BOX THAT ROTATES MECHANICALLY AND TELLS THE HOSITMAN THE CAGE LOCATION. HOIST WAS RUNNING, INSPECTED, AND PLACED BACK INTO SERVICE AT 08:00 PM. MSHA WAS CALLED AT 08:12 PM REPORTING THE HOIST IN OPERATION.

EE reported back and hip pain from operating a fuel buggy on bumpy roads. EE received a medical evaluation and was placed off work on 5/19/2020, EE currently remains off work pending treatments.

EE STEPPED ON ROCK WHILE WALKING AND TWISTED RIGHT ANKLE. EE RECIEVED NO MEDICAL TREATMENT AT THAT TIME. EE REQUESTED MEDICAL TREATMENT AND WAS DEEMED WORK RELATED PER EMAIL ON 4.27.2020. EE WAS PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE removing a fan from a bulkhead. EE felt pain in neck and upper back from cutting through cables. EE placed on restricted duty on 8/6/2019 after medical treatment.

EE lifting drill steel and felt pain in groin and lower abdomen. EE went for medical treatment on 7/16/2019 and was placed on restricted duty for hernia. Treatment pending at this time.

EE lifting 50lb bags of anfo to dump into charging rig hopper and felt pain in neck and shoulder. EE received medical treatment on 7/08/2019 and was placed on restricted duty at that time.

EE reaching to stop a piece of drill steel from rolling off steel holder and felt pain in right shoulder. EE was placed on restricted duty following medical treatment on 3/28/2019.

As a 20' section of NQ rods was being lifted up along the slide to be added to the drill string, the top of the water swivel caught the edge of a tower strut leveraging the bottom of the rods to kick up striking the miner on the left elbow and shoulder forcing the miner to the deck.

EE felt pain in abdomen when lifting a 16' drill steel. EE placed on restricted duty on 8/22/2019 while testing was performed. Testing was negative for hernia and EE was released back to full duty.

Wheel came off truck causing truck to hit rib. EE placed on restricted duty after medical treatment on 8/3/2019

EE climbing onto machine, EE slipped and fell onto right side. EE placed on restricted duty on 8.1.19.

Vibrating screen lid broke (the eyelet holding the screen lid) and fell on employee. EE was placed on restricted duty on 7/29/2019 due to left shoulder strain.

EE USING SLEDGE HAMMER TO REMOVE A BOLT AND FELT PAIN IN RIGHT ARM AND ELBOW. EE DID NOT RECIEVE MEDICAL TREATMENT UNTIL 8/28/2019 AND PLACED ON RESTRICTED DUTY AT THAT TIME.

EE operating a walk behind loader. EE slipped off platform, while slipping off EE grabbed control and moved loader backwards pinning lower leg between loader and concrete. EE placed on restricted duty on 7/8/2019.

EE driving haul truck, hit a bump in the road, EE felt pain in low back area. EE was placed on restricted duty for strain after medical treatment the following day.

EE using a pry bar to move a stuck rock, pry bar slipped and mashed left ring finger. EE received 5 sutures in finger.

EE bending over to pick up a piece of drill steel and felt pain in low back area. EE went for medical treatment on 6/7/2019 and was placed on restricted duty at that time.

EE FELT PAIN IN LOW BACK WHILE BENDING OVER TO PLACE A WHEEL CHOCK UNDER WHEEL OF LOADER. EE PLACED ON RESTRICTED DUTY AFTER MEDICAL TREATMENT ON 5/1/2019.

EE LACERATED RIGHT RING FINGER BETWEEN HOIST ROPE AND DRUM. EE RECEIVED SUTURES AND WAS PLACED ON RESTRICTED DUTY AT THAT TIME.

EE carrying 5 gallon bucket of oil, switched hands and felt pain in low back. EE was placed on restricted duty after medical treatment on 4/2/2019.

Went into dog house to fill out paperwork. Finished paperwork, went to walk out of trailer and hit head on the low hanging cross beam. Happened in dog house.

EE stepped on drain in floor and fell injuring right shoulder. EE placed on restricted duty on 4/29/2019. Treatment is ongoing at this time.

EE dismounting grader and slipped on step, falling into grader on left side torso. EE taken for medical examination on 3/4/2019 and was placed on restricted duty for pain and contusions. EE had a follow up medical exam on 3/8/19 and was returned to full duty at that time.

EE pulling on blown hyd hose and slipped and fell. EE felt pain in right shoulder and arm. EE placed on restricted duty and released following week back to full duty.

EE lifting and pulling on a pump and felt pain in low back area. EE was placed on restricted duty on 2.6.19 and remains on restricted duty at this time.

EE opening a gate, right thumb smashed between the gate and metal brace fracturing right thumb. EE will require surgical procedure to repair fracture and will be able to remain on full duty after surgery due to scope of job requirements.

EE cutting a cylinder stem with torch, the stored energy cause the cylinder to kick back striking EE on forehead, lacerating forehead. EE received sutures to forehead and released back to full duty following that medical treatment.

6.24.2019 EE diagnosed with occupational carpal tunnel syndrome. Surgery dates are pending at this time. EE will be released to restricted duty after surgery.

EE was diagnosed with occupational carpal tunnel syndrome on 5/31/19. EE treatment / surgery is pending at this time. EE will remain full duty until surgery and will be released on restricted duty following surgery.

EE climbing down ladder on dam builder and felt pain in left knee. No medical treatment until 6/20/19 and EE was placed on restricted duty.

EE was diagnosed with Carpal Tunnel Syndrome on 5/20/2019. Treatment includes surgery and date of surgery is pending at this time. EE remains full duty and will be placed on restricted duty following surgery.

EE diagnosed with bilateral carpal tunnel syndrome on 1/31/2019. Treatment and/or surgery is pending at this time.

EE diagnosed with carpal tunnel syndrome 2.21.19. Treatment and/or surgery is pending at this time.

EE diagnosed with occupational carpal tunnel syndrome on 2.21.19. Treatment and/or surgery is pending at this time.

EE diagnosed with Occupational Carpal Tunnel Syndrome on 2.2.19. Treatment and/or surgery is pending at this time.
