29 MINE MAN HOIST BEGAN MAKING A NOISE ON 7/2/2020. MAINTENANCE ISSUE OF BULL GEAR KEY WELD HAD BROKEN LOOSE. MSHA: 10:15 AM - EMPLOYEES EVACUATED OUT OF THE MINE FOR REPAIRS. REPAIRS MADE AND MSHA WAS CALLED BACK TO LET THEM KNOW THAT SITUATION WAS COMPLETE. EMPLOYEES WERE RELEASED BACK TO WORK AT 12:00 PM

29 Mine Main Hoist went down with an electrical issue while skipping ore. MSHA was called and everyone evacuated out of the mine through the escape shaft. Unfound Electrical Issue - hoist back in operation at 7:51 PM.

Fuel buggy lunged forward pinning employee against pillar. EE was taken by ambulance to University of Missouri a Columbia and remains hospitalized currently. Amputation of left leg below the knee. EE remains off work.

EE going underground on cage and felt ear pop due to elevation change. EE remained full duty until surgery on 9/22/2020 and was placed off work at that time.

EE flushing out a ZnSo4 line with an air hose, the hose came loose and hit employee in the face. EE received a scratch on right eye. Antibiotic eye drops were prescribed to employee.

EE ATTEMPTING TO MOVE A PIECE OF EQUIPMENT INTO THE MENS CHANGE ROOM WHEN THE PIECE OF EQUIPMENT FELL STRIKING EE. EE RECEIVED FRACTURE TO LEFT FOOT AND SMALL SCRATCH BY RIGHT EYE. EE PLACED ON LIGHT DUTY WHILE UNDER MEDICAL TREATMENT FOR FRACTURED FOOT.

EE fell from truck while cleaning lights. EE placed off work after surgery to repair broken bones in leg.

EE STEPPING OFF BOTTOM STEP OF TRUCK AND STEPPED ON ROCK TWISTING RIGHT ANKLE. EE PLACED ON RESTRICTED DUTY FOLLOWING MEDICAL TREATMENT.

EE stepped on ballast rock and felt pain in left foot. EE placed on restricted duty on 9/22/2020 for pain and further medical evaluation.

EE removing a rock from the south shaker with a mining bar and felt pain in right shoulder/arm. EE received medical treatment and placed on restricted duty. Treatment is on-going.

EE slipped on bottom step of scaler. Fracture to left foot. EE released to restricted duty following medical treatment.

EE grabbed another EE that lost balance to keep EE from falling. EE felt pain in right bicep. EE placed on restricted duty pending testing.

EE CUT RIGHT HAND WORKING ON DRIVESHAFT ON SCALER. EE RECEIVED STITCHES AND PLACED ON RESTRICTED DUTY UNTIL STITCH REMOVAL.

EE leaned out the window of a truck to adjust the mirror and felt pain in right knee. EE received medical treatment and was placed on restricted duty at that time. Treatment on-going.

EE attempting to loosen a frozen clevise. EE hit clevise with hammer and clevis came loose and hit EE on right forearm. Resulting in fracture to right ulna. EE placed on restricted duty after medical evaluation.

Lid of emulsion hopper fell striking the EE right thumb causing severe laceration. EE required surgical repair to right thumb, EE remains on restricted duty following treatment.

EE climbing up into loader and felt pain in shoulder. EE placed on restricted duty after medical attention on 2/3/2020.

EE closing an air door - door caught air and came back injuring EE right wrist. EE remained full duty until surgery on 3.5.2020 and returned to work on restricted duty following surgery.

EE WALKING THROUGH THE WOODS AND FELL UP A STEEP SLOPE INJURING LEFT SHOULDER. EE REMAINED FULL DUTY FOLLOWING MEDICAL TREATMENT. EE WILL RECEIVE SURGERY ON 4/28/2020 AND WILL BE PLACED ON RESTRICTED DUTY FOLLOWING SURGERY.

EE pulling on 30 horse pump and felt pain in low back area. EE placed on restricted duty on 1/6/2020 and released on 1/13/2020 to full duty.

EE WELDING/CUTTING ON HAUL TRUCK AND GOT FOREIGN OBJECT IN LEFT EYE. EE TAKEN FOR MEDICAL TREATMENT TO REMOVE FOREIGN OBJECT AND RECEIVED PRESCRIPTION EYE MEDICATION AND RELEASED BACK TO FULL DUTY.

EE BLEW INTO A/C FILTER TO REMOVE DUST FROM FILTER, PIECE OF DEBRIS BLEW INTO LEFT EYE. EE RECIVED MEDICAL TREATMENT, IMBEDDED DEBRIS WAS REMOVED FROM LEFT EYE. EE RELEASED BACK TO FULL DUTY.

EE lifting a lid on the emulsion tank, lid slipped out of EE hand smashing right thumb. EE received laceration to right thumb requiring sutures. EE released back to full duty.
