LOST POWER ON NO.33 PRODUCTION HOIST MOTOR GENERATOR. REPLACED CPACITOR AND SET MOTOR STARTER. MSHA WAS CALLED TO REPORT HOIST DOWN AT 4:35 AM ON 9-19-01 AND CALLED AGAIN TO REPORT HOIST WAS RUNNING AT 6:00 AM.
#31 HOIST BRAKE WAS SET AND WOULD NOT RELEASE. ELECTRICIAN WAS CALLED AND RETURNED THE HOIST TO SERVICE. MSHA WAS CALLED AT 2140 AND TOLD THE HOISTR WA OUT OF SERVICE AND THE HOISTMAN CALLED MASH BACK AT 2335 AND TOLD THEM THE HOIST WAS BACK IN SERVICE.
WE HAD REPEATED POWER FAILURE DUE TO INCOMING VOLTAGE LOW. THIS CAUSED THE DYNA-BRAKE TO LOOSE ITS MEMORY. RE-PROGRAMED DYNA-BRAKE. MSHA WAS NOTIFIED AT 12:15 PM THAT THE HOIST WAS DOWN AND WA S CALLED AT 1:00 PM WHEN BACK IN SERVICE.
OPERATOR HEARD LOUD POP. LOOKED BACK AND SAW FLAMES COMING OUT OF EXHAUST PIPE CAUSING SMOKE TO FILL AREA, RESULTING TURBO CHARGER ON FIRE. OPERATOR CHOOSE TO LEAVE AREA AND START EVACUATION O F MINE.
HOISTMAN STARTED AT 5:50 A.M. TO TEST #31 HOIST AND COULD ONLY LOWER HOIST. WHEN HE TRIED TO RESET POWER IT WOULDN'T RESET. MSHA WAS CALLED AT 6:25 A.M. AND TOLD THE HOIST WAS DOWN. WE DIDNT H AVE ANYONE UNDERFOND AT THE TIME. THE ELECTRICIANS FOUND A FAULT IN THE UPPER HOIST LIMIT SWITCH AND REPAIRED IT. THE HOIST WAS RETURNED TO SERVICE AT 7:05 AM SAME DAY AND MSHA WAS CALLED AND
HOIST WAS PULLING A LOADED SKIP OUT AND LOST POWER. HOISTMAN CALLED AN ELECTRICIAN IN TO CHECK IT. THE ELECTRICIANS FOUND A BAD BRAKE SET SOLENOID COIL AND REPLACED IT. MSHA WAS CALLED AT 3;50 A.M. AND REPORTED HOIST DOWN. MSHA WAS CALLED BACK AT 9:00 A.M. SAME DAY AND TOLD HOIST WAS BACK IN OPERATION.
THE EMPLOYEE REPORTED WHILE STANDING IN THE CARGO AREA OF A FORD TRACTOR, THE OPERATOR DROVE OVER A ROUGH AREA OF THE ROADWAY. THE EMPLOYEE LOST HIS BALANCE STRIKING THE SIDE OF THE CARGO AREA WITH HIS SELF-RESCUER. CONTUSIONS TO THE UPPER HIPS AND A SPRAINED LOWER BACK RESULTED. THE EE SAW THE COMPANY DOCTOR ON 1-16-02 AND AGAIN ON 2-14-02 AND PLACED ON RESTRICTED DUTY.
EE WAS STACKING GLUE INTO GLUE RACK ON BACK OF #31-08 SECOMA PINNING RIG WHEN HIS FOOT SLIPPED ON SURFACE OF PINNING RIG CAUSING HIM TO FALL TO GROUND LANDING ON HIS FEET. EE WAS TAKEN TO LOCA L HOSPITAL AND XRAY REVEALED A FRACTURE TO THE DISTAL END OF THE LEFT LEG FIBULA.
WHEN GOING BACK TO MINE AREA 65 BC 50 TO GET LOAD OF ORE, EE HIT A HOLE IN ROADWAY. THIS AREA OF ROAD WAS COVERED WITH WATER RESULTING IN STRAINED MUSCLES IN LOWER BACK AROUND HIPS. THERAPY ST ARTED ON 2-20-02.
THREE EE WERE GROUTING PILLARS IN A-4 PILLAR AREA, WHEN THE 3/4"" GROUT HOSE BECAME PLUGGED WITH HARD CEMENT. THE HOSE BLEW APART INT HE EE'S RIGHT HAND SPRAYING CEMENT IN HIS RIGHT AND LEFT EY ES. EE WAS TAKEN TO DR WHERE THEY WASHED OUT HISEYES DR REFERRED EE TO DR AN OPTOMETRIST IN FARMINGTON. EE WAS DIAGNOSED WITH SEVERE IRRITATION TO BOTH EYES. WHICH WERE BANDAGED A FOLLOW UP V
EE REPORTED WHILE GRADING AN UNDERGROUND ROADWAY WITH A GALION 850 B GRADER, THE BLADE STRUCK A SOLID LEDGE PROPELLING HIM FORWARD AND ONTO THE STEERING WHEEL. EE UNDERWENT RIGHT SHOULDER SURG ERY TO REPAIR A TORN ROTATOR CUFF ON 2/20/02.
THE EE REPORTED WHILE DRAGGING A WATER HOSE BACK TOITS HANGER IN THE SOUTH END OF THE MILL BASEMENT NEAR THE ROD MILL DISCHARGE. HE STEPPED INTO A 12"" DIAMETER BY 6"" DEEP HOLE IN THE CONCRETE. HE SPRAINED HIS KNEE AND FELL TO THE FLOOR ON HIS LEFT SHOULDERT. THE EE SAW DR. ON 10-12-01. THE EE BEGAN PHYSICAL THERAPY ON 10-17-01, TWO TIMES A WEEK FOR THREE WEEKS.
THE EMPLOYEE WAS CLIMBING DOWN OFF OF A 980 CAT LOADER WITHOUT MAINTAINING A 3 POINT CONTACT WHEN HE MISSED THE BOTTOM STEP. THIS CAUSED HIM TO LAND ON THE GROUND HURTING HIS TAIL BONE. EMPLOY EE ASK TO GO TO DOCTOR ON 10-22-01.
THREE WORKERS WERE TRYING TO DUMP A TRUCK BED WITH A LHD & A LOADER. THEY LIFTED THE TRUCK BED ~2/3 UP & COULDN'T MOVE IT FURTHER. THEY DECIDED THEY WOULD BE ABLE TO GET THE TRUCK TO DUMP IF T HE DUMP LEVER WAS MOVED OUT OF NEUTRAL. AN EE CLIMBED ONTO THE TRUCK BUMPER TOWARD THE CAB AREA.THE LHD BUCKET SLIPPED OFF THE TRUCK BED & EE'S LEG WAS PINCHED BETWEEN LOADER TIRE & TRUCK CAB
EE WAS IN A HURRY JUMPING FROM FORKLIFT LEFT FOOT LANDING ON A LOOSE ROCK. ROCK ROLLED UNDER FOOT TURNING ANKLE RESULTS ARE FRACTURE TO SMALL BONE ON SIDE OF LEFT FOOT.
EE WAS CLEANING FOGGY WINDSHIELD ON WA500. HIS FOOT SLIPPED ON WET STEP OF THE LOADER & HE FELL TO THE FLOOR OF THE PB HOUSE APPROXIMATELY 6 1/2FEET. X-RAY SHOWS A FRACTURE TO THE RIGHT WRIST.
EE WAS GETTING ON #14 GETMAN. EQUIPMENT HAS A SWING TYPE LADDER. OLADDER SUNG UNDER BASKET CAUSING EE TO HYPEREXTEND RIGHT KNEE RESULTING IN ARTHROSCOPY SURGERY ON 10-03-01.
EMPLOYEE WAS LIFTING A FULL 5 GALLON WATER BOTTLE WHEN HER WRIST POPPED AND STARTED HURTING. AFTER SEVERAL DAYS, SHE STATED THERE WAS STILL MUCHDISCOMFORT, AND SHE WAS SEEN BY DR. WHO TREATED HER FOR A SPRAINED WRIST. THE EMPLOYEE STATED HER WRIST GOT BETTER, BUT WENT BACK TO HURTING AT NIGHT. THE DR. RECOMMENDED A SPECIALIST.SPECIALIST DIAGNOSED SPRAINED WRIST AND WRIST BRACE.
EMPLOYEE WAS LOWERING A LINER WITH A ROPE AND FELT PAIN IN LOWER BACK. LINER WEIGHED 150 LBS. AND WAS BEING LOWERED APPROXIMATELY 2 FEET INTO TRIPPER CHUTE ABOVE FINE ORE BINS. PHYSICAL THERAP Y BEGAN ON JULY 2, 2001.
EMPLOYEE WAS WALKING FROM CRUSHER BACK TO LOADER WHEN HE STEPPED ON SIDE OF SIX INCH ROCK, FOOT SLIPPED OFF ROCK, CAUSING PAIN IN LOWER BACK. AFTER EXAMINATION BY DR ON MONDAY, HE RECOMMENDE D THAT EMPLOYEE BE KEPT OFF DUTY STATUS UNTIL ALL ISSUES OF EMPLOYEE'S CONDITION ARE RESOLVED.
EMPLOYEE STARTED SCALING BACK AND TOP OF RIBS WHILE WORKING THEIR WAY DOWN THE PILLAR, EE WAS STRUCK ON TOP OF THE HEAD BY A PIECE OF LOOSE 8"" BY 10"" WEIGHING AROUND 20 LBS. AND IT FELL ABOUT 10' TO 15' BEFORE IT STRUCK HIM. BEGAN LOST TIME6-01-01.
EE WAS DRIVING A 30T MOXY TRUCK ON THE EXIT HILL OF THE #28 QUARRY. THE HILL HAD ROCK ON IT AND EE ATTEMPTED TO STRADDLE SOME ROCKS THAT HAD BEEN LOST ON THE PREVIOUS LOAD. THE FRONT FRAME OF TRUCK CAUGHT THE LOOSE ROCK AND THREW IT UNDER FRONT TIRE CAUSING CAB & BED TO TWIST. EE EXPERIENCED BACK/HIP PAIN & TAKEN TO CO. DR. THEN WENT TO OWN DR & TOLD TO BE OFF WORK 2 WEEKS & THERAP
EE WAS HAULING ANFO TO POWDER MAGAZINE. HIT HOLE IN ROADWAY, JARRING BACK. CONTINUATION OF ACCIDENT ON 10-27-00. FIRST TRIP TO DR WAS 4-13-01. SCHEDULED BACK AFTER LONG CHANGE RELEASE 4-27-01.
EMPLOYEE REPORTED WHILE WORKING ON THE SEEP POND PIPELINE, HE BUMPED HIS RIGHT ELBOW ON A PIPE WELDER.HE THEN EXPERIENCED NUMBNESS AND TINGLING FROM HIS ELBOW TO HIS FINGERS. DR. DIAGNOSED POS SIBLE NERVE DAMAGE AND REFERRED THE EMPLOYEE TO ANOTHER DR.. TESTS REVEALED NERVE DAMAGE. SURGERY IS SCHEDULED FOR 7-30-01. EMPLOYEE IS EXPECTED TO BE OFF WORK APPROX. 6-8 WEEKS.
EE WAS HAND MINING WALLS. AS EE WAS BACKING AWAY FROM PIECE KNOCKED LOOSE WITH HIS SCALING BAR, HE TRIPPED OVER A ROCK AND FELL BACKWARD. HE TRIED TO CUSHION HIS FALL WITH HIS LEFT WRIST/HAND. XRAY REVEALED A FRACTURED LEFT WRIST.
WHILE THROWING CHAIN OVER A LOADED TRAILER AND STANDING IN A POOR POSITION, THE EE STRAINED HIS RIGHT SHOULDER WITH RE-OCCURRING INCIDENTS ON 2--6-01 AND 5-16-01. LOST TIME BEGAN 5-18-01.
EE WAS TRYING TO HAMMER ROCKS THROUGH TAILS SHAKER SCREEN WITH A 8 LB. SLEDGE HAMMER WHEN HE FELT A PAIN IN HIS RIGHT ELBOW.
EE WAS OPERATING #20 ST8 LOADER IN REMOTE. HE WAS MOVING A LARGE BOULDER OUT OF LOADING AREA. HE WAS STANDING ABOUT 10' FROM PILLAR AND ABOUT 10' FEET FROM LOADER. HE BACKED LOADER OUT WITH BO ULDER IN DIPPER AND STARTED FORWARD WHEN THE REAR OF THE LOADER CAME OFF THE GROUND AND SWUNG AROUND AND HIT HIM, KNOCKING HIM UP AGAINST A PILLAR.
OPERATOR WAS DRIVING A-40 VOLVO TRUCK TRAVELING DOWN STRAIGHT ROADWAY WHEN HE STRUCK PILLARS ON LEFT SIDE OF ROADWAY. EMPLOYEE STATES HE DOES NOT KNOW WHAT HAPPENED.
EE GOT OFF #73 JUMBO TO TIGHTEN A HOSE ON THE BOOM. HE LOST HIS FOOTING ON THE ROUGH FLOOR & BEGAN TO FALL, CATCHING HIMSELF WITH HIS HANDS. HISHANDS CAME IN CONTACT WITH SOME SHARP ROCKS, CUT TING BOTH HANDS WHICH REQUIRED STITCHES.
THE EMPLOYEE REPORTED WHILE CHANGING A STRIKER BAR ON A ROCK BOLTER, HE USED A HARD METAL HAMMER TO LOOSEN IT. A METAL CHIP BROKE OFF OF THE STRIKER BAR IMBEDDING IN HIS RIGHT FOREARM. A DR. REMOVED THE OBJECT AND CLOSED THE WOUND W/2 STITCHES. THE EMPLOYEE RETURNED TO FULL DUTY WORK ON THE SAME DAY.
THE EMPLOYEE REPORTED WHILE GETTING READY TO TAKE A TRANSMISSION OIL SAMPLE ON THE #05-31 CATERPILLAR D400E HAUL TRUCK AT THE UNDERGROUND PM PIT, THE TRANSMISSION COVER FELL ON HIS LEFT FOREAR M. A LACERATION TO THE LEFT FOREARM REQUIRING 19 STAPLES RESULTED.
EMPLOYEE WAS INSTALLING ROOF BOLTS FROM A GETMAN BASKET. IN THE PROCESS THE EMPLOYEE MASHED HIS LEFT INDEX FINGER BETWEEN THE BASKET TOP GUARD RAIL AND THE ""STOPER DRILL"". THE INJURY REQUIRED 3 STICHES.
EE WAS USING A GRINDER TO SMOOTH THE EDGES OFF A PIECE OF THE STEEL. LATER DURING SHIFT THE EE'S RIGHT EYE BEGAN TO FEEL ITTITATED WITH SOME PAIN, BUT THE EE DID NOT FEEL MEDICAL CARE WAS NECE SSARY. AT THE BEGINNING OF HIS NEXT SHIFT ON THE FOLLOWING AFTERNOON THE IRRITATION & PAIN STILL CONTINUED. HE WAS THEN TAKEN TO THE DOCTOR & A SMALL SLIVER OF STEEL WAS REMOVED FROM RIGHT EY
EMPLOYEES WERE REMOVING A BAD SECTION OF PIPE USING HYSTER FORKLIFT TO SUPPORT ONE END OF THE PIPE THAT HAD TO BE CUT OFF. INJURED WAS SUPPORTING THE OTHER END OF THE PIPE WHILE EE REMOVED T HE CLAMP, WHEN CLAMP WAS MOVED, THE PIPE ROLLED OFF THE FORKLIFT AND SMASHED FINGER BETWEEN PIPEAND ROPES ON FORKLIFT.
OTHER SOUTH MINE TRAIN OPERATOR WAS RETURNING SOUTH TO GET ANOTHER LOAD. TRAIN WAS TO SLOW DOWN TO A CRAWL AT #2 SWITCH CURVE HEADED FOR #4 RAISE TO LOAD ORE. TRAIN WENT PAST 32E/713 FUEL BAY THE LOADER OPERATOR PULLED ACROSS THE TRACK HEADED TO FUEL BAY AT QUITTING TIME AND WAS STRUCK BY EMPTY RAILROAD CAR.
EE WAS UNDERGROUND IN AN AREA DUSTY FROM HAUL TRUCK TRAFFIC. HE FELT SOMETHING IN HIS RIGHT EYE & RUBBED IT THINKING IT WAS ONLY DUST. OVER THE NEXT 3 TO 4 HOURS, THE EYE WAS FLUSHED NUMEROUS TIMES BUT CONTINUED TO BECOME MORE IRRITATED. HEWAS TAKEN TO A DOCTOR WHO REMOVED A SMALL PARTICLE & APPLIED ANTIBIOTIC TO THE EYE & PRESCRIBED ANTI-INFLAMMATORY EYE DROP.
EE WAS DOING ROUTINE MAINTENANCE ON TRUCK AT UNDERGROUND SHOP BY FILLING DIFFERENTIALS WITH 90 WT OIL. IN THE PROCESS PRESSURE FROM WITHIN DIFFERENTIAL CAUSED THE OIL TO SPRAY OUT INTO THE EE S FACE AND TO RUN INTO EE EYES AND EARS. EE S EYES AND EARS WERE THOROUGHLY CLEANED, WITH THE EYES BEING FLUSHED OUT SEVERALS TIMES AS PRECAUTIONARY MEASURE, THE EE WAS SENT TO DR. EXAMING AND
A CONTRACTOR HAD WASHED A 980 LOADER IN SHOP AND LEFT WITHOUT CLEANING THE FLOOR. A TRACTOR WAS PULLED IN WITH A FLAT & WHILE EE WAS CHANGING TIRE HE SLIPPED ON GREASE ON CONCRETE & FELL LANDI NG ON HIS RIGHT SHOULDER AREA. EE WAS TAKEN TO DR THAT DAY. STILL EXPERIENCING PAIN & HAD MRI 5/11. SAW NEW DR ON 5/22 & WAS INSTRUCTED TO GO TO PHYSICAL THERAPY. 7000-1/THERAPY. NO LOST TIME
EMPLOYEE STEPPED ON A ROCK TWISTING HER RIGHT ANKLE RESULTING IN A HAIR LINE FRACTURE TO SMALL BONE IN RIGHT ANKLE. ANKLE HAD PREVIOUSLY BEEN FRACTURED RESULTING IN CHRONIC ANKLE SPRAINS. DR R ECOMMENDED NO CAST OR SPLINT AND ISSUED PAIN MEDICATION ONLY. EMPLOYEE WAS RELEASED FOR REGULAR DUTY.
EE WAS ON OVERTIME ASSITING WITH REBUILDING CYCLONE FEED PUMP AT THE MILL BUILDING. EE WAS USING A PIPE WRENCH TO LOOSEN A SHEAVE WHEEL BY EXERTING HEAVY DOWNWARD PRESSURE ON THE WRENCH HANDLE . WHEN WHEEL NUT LOOSENED, THE MOMENTUM OF THE DOWNWARD PRESSURE RESULTED IN EE PINCHING HIS RIGHT INDEX FINGER BETWEEN THE WRENCH HANDLE AND GUARD BRACKET. TREATMENT INCLUDED 3 SUTURES. EE W

FOLLOWING MORNING EE WAS TAKEN BACK TO DR, WAS REC OMMENDED THAT AN ORTHOPEDIC SPECIALIST LOOK AT SITUATION. DETERMINED EE HAD AN AVULSION TO LEFT ANKLE. RECOMMENDED EE WEAR ANKLE BRACE UNDER BOOTS & THERAPY PRESCRIBED. RELEASED FOR NEXT SHIF
EE WAS WORKING ON SCALER IN UG SHOP. EE WAS USING METAL BAR TO POSITION HOOD MOUNT RAILING INTO POSITION TO BE WELDED. THE BAR SLIPPED CAUSING THE EE TO BE THROWN OFF BALANCE AND FOR HIS RIGHT HAND TO SLIDE DOWN METAL FRAME FAR ENOUGH TO CUT RIGHT RING FINGER ON TOP OF LAST JOINT(BELOW FINGER NAIL). LACERATION REQUIRED THREE STITCHES. EE WAS RELEASED TO WORK NEXT SCHEDULED SHIFT.
THE EMPLOYEE REPORTED THAT HE WAS HAVING NUMBNESS AND PAIN IN RIGHT AND LEFT HANDS, WRISTS AND ARMS, ESPECIALLY AT NIGHT WHILE TRYING TO SLEEP. A NERVE CONDUCTION TEST WAS GIVEN AND ON 7/30/01 THE DOCTOR GAVE THE RESULTS TO THE EMPLOYEE, AND RECOMMENDED SURGERY TO THE RIGHT HAND ONLY. SURGERY HAS BEEN SCHEDULED TO 8/22/01. FROM THAT DAY, THE EE WILL BE OFF WORK FOR APPROX 6 WEEKS.

ON MAY 15TH, 2001, EMPLOYEE REPORTED TO AREA REPAN ALLEGED CARPAL TUNNEL IN BOTH HANDS. HE WAS SEEN BY DR. ON 5/17/01. THE DR. USED A BILATERALSPLINT IMMOBILIZATION AND ANTI-INFLAMMATORY THER APY AT THIS TIME. ON 10/09/01, EMPLOYEE RETURNEDTO DR., WHERE HE DID A NERVE CONDUCTION TEST ON EMPLOYEE. SEVERE CARPAL TUNNEL SYNDROME ON THE R. WRIST, MODERATE ON L. WRIST. SURGERY ON 10/31
THE EE REPORTED THAT HE WAS HAVING CARPAL TUNNEL SYNDROME SYMPTOMS, NUMBNESS AND PAIN IN BOTH OF HIS RIGHT AND LEFT HANDS, WRISTS AND ARMS. AN APPT. WAS SCHEDULED FOR 3-27. THE DR.
