Accident type, without injuries

#34 service hoist went down at 6:55 am. Contacted electricians at 6:55 am. Called MSHA at 7:25 am. Electricians found a bad contact. Hoist was back up and running again at 10:20 am. Called MSHA at 10:36 to report hoist was in service.

EE climbing into cab of Wagner ST8 and felt pain in abdomen. EE diagnosed with hernia on 1.9.19, treatment/surgery is pending at this time. EE placed on restricted duty on 1.9.19.

Over-exertion in pulling or pushing objects

EE pulling on concrete pumping hose and felt pain in groin area. EE diagnosed with hernia.

Over-exertion in lifting objects

EE straining while lifting liners and felt pain in groin area. EE diagnosed with hernia. EE placed on restricted duty while treatment is pending.

EE climbing up steps to get into loader when EE twisted left knee. EE placed on restricted duty following medical treatment.

EE felt pain in back during repetitive bending during the drilling process. EE went for medical treatment on 2.26.18 and was placed off work at that time.

EE twisted right knee when exiting piece of equipment on 11/26/2018. EE remained on full duty following medical evaluation and testing. EE will receive surgical treatment on 5/24/2019 and will be released to restricted duty on 5/25/2019.

EE stepping onto train and felt pain in left knee. EE remains full duty at this time but will have surgical ""scope"" procedure on 2.1.19 and will be placed on restricted duty following procedure.

EE operating mechanical scaler, rock rolled down boom jarring operators cab. EE felt pain in right shoulder. EE seen for medical treatment on 10.22.18 and placed on restricted duty at that time.

Over-exertion in pulling or pushing objects

EE pulling on wrench to replace the sheave on the south tails pump when EE felt pain in right shoulder. EE treated at medical clinic on 10.8.2018 and paced on restricted duty at that time.

Over-exertion in lifting objects

Consultant contract employee standing at core rack and felt pain in back. Taken to clinic and diagnosed with muscle strain and placed on restricted duty by physician.

EE PULLED A PALLET FROM UNDER A TRUCK AND FELT PAIN IN LOW BACK. EE TREATED AT ER ON 9/23/18 AND WAS PLACED ON RESTRICTED DUTY AT THAT TIME. EE RETURNED TO FULL DUTY ON 10/3/18

EE lifting a piece of plate steel and felt pain in right elbow. EE received medical treatment and was placed on restricted duty.

EE reported on 7/24/2018 that on 7/20/2018 EE was shoveling concentrate out of the bed of a concentrate truck feeling pain in left arm and chest area. EE was sent for medical evaluation on 8/14/2018 and released with no restrictions. Follow up medical evaluation on 8/17/2018 EE was placed on restricted duty.

Over-exertion in lifting objects

EE lifting cases of powder onto charging rig basket when EE hit edge of basket with box and caused pain in left shoulder. EE was placed on restricted duty on 7/5/2018.

Struck against a moving object

EE operating loader and caught the bucket on a rock jarring EE in cab. EE felt pain in shoulder. EE was placed on restricted duty on 7/17/2018 following medical treatment.

Over-exertion in lifting objects

EE lifting a tire from the side of a truck bed when the tire slipped overextending left arm straining left bicep. EE placed on restricted duty.

Caught in, under or between a moving and a stationary object

EE stepping off tire on charging rig, stepped onto rock, rock rolled and caught toe under edge of tire bending right foot backwards, fracturing distal fibula of right leg. EE was placed on restricted duty after medical evaluation.

EE moving pipe. EE unchained the end of the pipe to move its location when the pipe swung and struck EE on side of head and chest. EE placed on restricted duty.

Over-exertion in lifting objects

EE shoveling ore in skip pocket and felt pain in lower back. EE was seen by physician and was placed on restricted duty on 4/12/18.

EE was loading glue on bolter when EE hip popped out of place and back into place pinching nerve. EE was released to full duty following medical examination on 4.10.18. A follow up appointment on 5.1.18 resulted in EE placed on restricted duty at that time.

EE performing hand scaling underground with a scaling bar and felt pain in right elbow. EE was sent for medical treatment on 3/14/18 and was placed on restricted duty at that time for elbow sprain.

Fall down stairs

EE slipped going down a step into the bathroom and fell, twisting left knee. EE remains full duty but will have a surgical procedure on 3.26.18. EE is expected to return to full duty following procedure on 3.26.18.

EE smashed thumb between steering cylinder and frame resulting in small chip fracture of left thumb. EE was released to full duty following medical evalulation.

Fall to the walkway or working surface

EE slipped in shower in changeroom on surface. EE received laceration on head requiring sutures. EE released back to work following medical treatment.

Absorption of radiations, caustics, toxic and noxious substances

Possible unconfirmed insect bite/sting. EE working on a pump and noticed a small blister spot on side of right hand. Hand/arm began swelling. EE treated at local clinic but could not confirm diagnosis of insect bite sting. EE released to Full Duty.

EE using a scaling bar to pry open a valve, bar slipped and pinched EE left pinky finger between bar and thickener tank lacerating finger requiring 6 sutures. EE released back to full duty following medical treatment.

EE bitten/stung by insect. Took EE for medical treatment and release back to work full duty.

EE installing grout in hole when grout came out above hole and got into left eye. EE was taken for medical attention and was prescribed a prescription medication for irritation. EE remained full duty.

EE received a laceration on left forearm when the utility knife slipped while EE was cutting a zip tie. EE received sutures and was released back to work.

Struck by flying object

EE got foreign material in right eye while picking up safety glasses. EE received an abrasion on eye and given a prescription eye medication for treatment. EE released back to work on full duty following medical treatment.

EE using utility knife to splice a cable and tool slipped lacerating EE's left hand. EE taken to physician and received sutures and released back to work with no restrictions.

EE removing a broken door glass from the door frame with a scraper tool when the tool slipped lacerating right hand. EE received 5 sutures on the back of the right hand. EE was placed back to work full duty following medical treatment.

EE moving a starter box when the box shifted and right hand was caught between box and handrail. EE taken to emergency room and received sutures in right hand.
