While Traveling to the section on a 2 man Johnson. A drop of water fell from the mine roof and struck the employee in the right eye. The employee was wearing EE's safety glasses at the time of the incident.

Employee picked up the continuous miner cable and waterline to move it over to the rib out of the roadway. When employee picked the cable and waterlines up they felt a burning sensation in lower back.
EE sat down on the operator seat of a 4 wheel personnel carrier. When EE raised a leg to swing it into the 4 wheeler EE felt a sharp pain across lower back.
Making a splice on shuttle car cable, employee was pulling the outer jacket off the cable, felt a pop in right shoulder. Went to the doctor on 5/19/2020. The doctor ordered an MRI. Off work waiting for results of MRI.
IE and another EE were throwing miner cable and waterline against rib in crosscut between #3 and #4. When slinging the cable and waterline IE felt a pain in lower back. IE worked on Jan. 3rd and 4th and did not seek medical attention until Jan. 6th.
Setting arches when co worker went to put a crib under support and let IE take the weight of the steel set.
EMPLOYEE WAS PICKING UP MINER CABLE TO HANG FELT BURNING PAIN IN LOWER LEFT BACK
Employee stated that while coming out of the mine they pushed airlock door open and large amount of dust blew up under glasses filling their eyes up. Employee was treated and released w/medicated eye drops.
Employee states they were double booming a six foot glue bolt and while bending roof bolt straight to install it rolled out and caused them to lose balance, hyper extending their left arm causing pain from bicep to shoulder in elbow area.
During workday EE used a sledge hammer, carried boxes of blasting caps and other tasks. When shift was over they were experiencing pain in neck and shoulders. EE said it felt like a pinched nerve. EE did not know which task caused the injury. MRI revealed fracture of C& and C6 spinous process. Sent home with cervical collar. Will see a neurosurgeon. (Edited to fit 384 characters)
Employee states while working under #4 shuttle car on #2 unit a large amount of material fell on EE (coal, Dirt) fell landing in EE's eyes and in EE's left ear. Took employee to ER at Baptist health in Madisonville and received antibiotic eye drops. Employee back to work next shift.
Stacking continuous miner cable. When EE bent over and picked up cable EE twisted EE's back.
The Employee was changing out a belt top stand on North A belt. As they lifted the top stand into place, they felt pain in the lower back.

Employee was working along the Section E beltline. Employee was dragging water/pipe line and tripped over other pipe line in the work area.
When throwing a crib block under a shuttle car ee smashed right ring finger between the crib block and the shuttle car frame. This accident caused a laceration to finger and broken bone in the finger also.
While taking the front hood off a D11 tractor EE bent EE's head sideways looking under the hood and felt a pop in EE's neck.

Working on D11R Dozer E-Bar center pin repair with customers job mechanic. Pin was frozen to correct temperature and both were holding each side of the 180 lb. pin when customers job mechanic stumbled. EE took the whole weight of the pin to keep it from falling on self. Felt burning sensation in stomach.
Employee was operating a 785D rock truck on Valley Fill #9. Employee stated they were cleaning the mirror on the offside of the truck and when they raised up they struck their left shoulder on the handrail on the truck.
Unloading miner cable off head of miner, swung cable to the rib and felt pain in right shoulder. Continued working at regular duties until 11/13/2017 when ee was examined by a physician.
Employee was carrying a box of resin glue to put on the roof bolter, when employee twisted to put the glue in the glue tray employee felt pain in employee's lower back and pain going down employee's left leg.

EE was working on a 785 truck at Grape Vine South Surface removing the main pump with another EE and a field person from JP Tech. Pump was almost out, EE moved to the side and was waiting for pump to come out to the ground. When it did, it struck the pry bar EE had been using. When the pump fell to the ground, it hit the pry bar and the bar slammed into EE's lower leg.
Employee states that while picking up belt framing on 1B belt, stepped and turned and felt discomfort in neck. At time of report it was file only, but after Employee went to Dr. found that needed surgery. Employee didn't miss work or restricted until 4-21-2017 for surgery on Neck.
Injured was hooking up a reclaim trailer to a diesel mantrip. The trailer shifted and the trailer tongue hit injured on right shin and caused a laceration that required 11 stitches.
The employee got left wrist pinched between the foot jack assembly and the frame of a roof bolter trying to put a keeper pin in the foot jack.
Injured was moving over a 3 foot section of miner cable and felt a pain in lower back.
Removing cover from control panel on continuous miner. Cover slipped on smashed finger.

Was cutting down a travel pad, reaching above head. Felt a sharp pain in right shoulder. Incident occurred on 8/9/2016, but did not seek medical attention until 8/17/2016 and was put on restricted duty.
