Cutting little finger on left hand. Shutting cab door on drill.

Employee was moving a shaft, when it slipped from hand. Trying to catch with the other hand and mashed finger on hard surface.

Holding onto the door frame when the second employee shut the door and cut EE's finger.

Taking brace off of plant, bolt was stuck and the brace smashed left hand

Changing drill steel on drill.

Replacing cylinder on loader. Strap on cylinder slipped mashing hand between cylinder and loader frame.

Was closing the property gate at the entrance and the gate swung striking the employee in the face.

Employee strained back during plant set up.

Employee was grabbing a spade from the dozer operator and didn't keep mind on task. The spade stuck his lower lip and had to have 2 stitches. Returned to work with no lost time.

Miner was replacing a belt on a conveyor when his finger was pinched. Miner then went to hospital and had his finger stitched and returned to work 9-15-2014 with no restrictions.

Replacing belt on lime bin and got some dirt in his eye.

Miner went against training and stuck head in pinch point between counterweight and frame of excavator while installing counterweight.

Employee rolled a rock off haul road to remove from travel area, strained a muscle, did not seek treatment at that time. That night he went to the dr. for pain & was released with instructions to use ice and Tylenol as needed. He went to the dr. night of 6/21, referred to Des Moines for further treatment, diagnosed torn bicep muscle 6/22 first missed day Scheduled for surgery 6/28

Truck driver was helping excavator operator remove a rock from tracks of excavator.

Employee was loading scrap iron into a loader bucket, felt pain in abdomen. Doctor determined it was just a strain and released him back to work on the 26th.

Removing a belly pan off of a truck, there was a block in place to prevent pan from falling to ground. When pan was released, landed on injured's hand. Went to the doctor, X-rays were negative for a break. Doc told him to ice it and keep it elevated, and come back the 12th. Oct 12th was released back to work with a 10lb lifting restriction for one week. Missed one day of work.

Employee was removing a receiver hitch from a pickup. The tie bolt rolled off of bumper and cut his thumb. Employee received stitches and was released back to work the same day with no restrictions.

Employee was helping to change overhead belt on the screening plant. When the clamp was released, it hit employee in the arm and cut his wrist. Employee received stitches and was released back to work no restrictions. Employee had some pain and bleeding overnight and went back to the doctor on 3/26/2009. Wound was cleaned and dressed, released back to work no restrictions.

Employee was changing screens on screening plant. When the screen was set into place it landed on left thumb. Employee was wearing gloves. Left thumb was broken and lacerated.

Working on fuel tank and got a metal flake in his eye. Flushed it out and kept working. Was still irritated the next day, went to dr to have object removed and wash released back to work with no restrictions.

Cleaning rock from belt, hit right thumb on guard and needed stitches.

Crew was changing plant screens. A screen fell over striking employee and cut eyebrow. Required stitches.

Loading a pickup with 50lb bags of ANFO. Lower back strain. Went to the doctor and has a 10lb lifting restriction.

Employee was working on a dozer, lifting gear sets and got a hernia on his right side. He did not seek treatment until 12/10. He is scheduled for surgery on 12/21/07.

Getting out of truck, twisted back wrong and has pain in back. There were discrepancies in the doctors notes, so we waited to hear back from work comp, that is why the time difference for the report.

Picking up rocks, he felt back snap. Did not report to supervisor or seek treatment until 10/1/07. Doctor reported T2 compression fracture. Will return 10/5/07 with 6 weeks restricted duty.

EE was unloading parts from truck, a trunion and shaft, when the shaft slipped . He reached out to hold it and pulled a muscle in his upper back.

The employee was changing oil in trucks at the mine site. He had opened the hood of the Volvo truck. He let the hood down easy and went on about his business. He returned to finish the truck and did not realize that the hood had latched and gave a strong tug to open it, straining his left shoulder.

Employee was helping move a 6" water line. While pulling on hose, he felt back pop.

EE was assisting drill operator and while adding drill steel got his left hand pinched between steel and coupling.

Employee was lifting a 4" pump discharge hose. He pulled muscles in left shoulder. (Surgery required - 1/17/06)

Employee was loading a portable power washer into his maintenance vehicle and strained his lower back.

EE WAS HAULING ROCK FROM QUARRY WHEN HE SAW A ROCK ON THE HAUL ROAD. EE GOT OUT OF HIS TRUCK TO REMOVE THE ROCK. WHILE LIFTING THE ROCK, EE SLIPPED AND TWISTED HIS BACK.

Employees' were lifting an 8'brace up to position it to be chained to loader bucket. Ground was wet & muddy, brace slipped & fell on EE's arm.

EMPLOYEE EXITED VEHICLE & CLOSED DOOR, DOOR DID NOT LATCH SO HE HELD LATCH WITH RT. HAND & SLAMMED THE DOOR WITH HIS LEFT. THE DOOR CLOSED ON RT. 3RD MIDDLE FINGER CUTTING THE TIP REQUIRING STITCHES.

Employee was working an un-plugging the hammermill and was int he process of opening the cover. the cover pivole up on hinges. the cever went over center and struck the employee on the right thight causing extersive and severe bruising.

EMPLOYEES WERE WORKING ON THE ROLLER FRAME OF A QUARRY DRILL AND USED HEAT TO REMOVE A BOLT. THE INJURED EE CAME ALONG TO HELP AND SAW A LARGE NUT LAYING ON THE FLOOR AND PROCEEDED TO PICK IT UP BURNING HIS RIGHT PALM & FINGERS.

EE WAS TAKING APART A 2" WATERHOSE QUICK CONNECT WHEN IT SUDDEN TO LET GO THE END STRUCK HIM IN THE MOUTH, CUTTING HIS LIP AND CHIPPING HIS TOOTH.

EE WAS MOVING A PIECE OF STEEL IN THE SHOP AND IN THE PROCESS HE STEPPED ON THE FLOOR GRATING OVER THE DRAIN AND TWISTED HIS ANKLE CAUSING HIM TO TWIST HIS LOWER BACK. AT THIS TIME HE WENT T O MEDICAL CENTER AND RETURNED TO WORK.

EMPLOYEE WAS HELPING MOUNT A HEADACHE RACK ON A CAT D400 ARTICULATING TRUCK WHEN A CHAIN CAME LOOSE AND STRUCK THE EMPLOYEE IN THE BACK OF THE HEAD. EMPLOYEE WENT TO DOCTOR AND GOT X-RAYS (NEG ). COMPLAINED OF SORE NECK & HEADACHE.

EE WAS ATTEMPTING TO SLIDE A TRANSMISSION ONTO THE LIFT GATE OF A SHOP TRUCK AND THRU OUT HIS BACK. INJURY OCCURRED IN TRUCK SHOP.

APPARENTLY THE EE WAS CLEANING A PAIN SPILL AND REACHED ABOVE HIM TO GET SOME MORE PAPER TOWELS HE KNOCKED OVER A CAN OF DENATURED ALCOHOL. THE CHEMICAL SPILLED ON HIS HEAD AND SHOYELERS. HE W ENT DIRECTLY TO RESTROOM AND WASHED HIS FACE ANDEYES. HE CALLED 911.

EMPLOYEE'S WERE IN THE PROCESS OF CHANGING SCREENS IN THE SCREEN PLANT AND EMPLOYEE CUT HIS RIGHT LITTLE FINGER ON A STEEL FLANGE. THIS CUT REQUIRED 10 STITCHES.

EE WAS LIFTING THE TAILGATE OF A DUMP TRUCK AND HURT HIS BACK.

EMPLOYEE WAS MOVING EXPLOSIVE BOXES AND TWISTED HIS BACK RESULTING IN LOWER BACK PAIN.

EE WAS WORKING ON A POWER TAKE OFF WHEN A SPRING CAME OFF AND CUT THROUGH HIS RIGHT INDEX FINGER.

EE WAS REMOVING A BROKEN AXLE FROM A TEREX 3305 QUARRY TRUCK AND THE AXLE SLIPPED AND DROPPED ON HIS LEFT FINGER CUTTING IT.

EMPLOYEE WAS PULLING OLD CABLE FROM PULL SCRAPERSPOOL AND PULLED A MUSCLE IN HIS BACK.

EMPLOYEE WAS HELPING WITH REPLACING THE CONVEYER BELT ON THE AUTOMATIC STACKER CONVEYER WHEN ANOTHER EMPLOYEE PULLED ON BELT CATCHING HIS FINGER BETWEEN THE BELT AND THE STACKED FRAME.

EMPLOYEE WAS HELPING TO REMOVE A TIRE FROM HAUL TRUCK AND WHILE PUSHING ON TIRE HIS BACK POPPED.EMPLOYEE RECEIVED MEDICINE FROM DOCTOR.
