Bar came loose striking employee in the right leg.

Was sitting in truck while being loaded and struck head on the steering wheel.

Twisted knee while carrying a box of explosives

Employee was walking up side bank to get to upper level. 980F loader bucket came into contact with employee. No visible life threatening injuries, called 911 per company policy. Employee was taken to hospital where they found a small puncture in lung, broken ribs, and bruising on chest and back.

Employee was changing screens on screening plant. When the screen was set into place it landed on left thumb. Employee was wearing gloves. Left thumb was broken and lacerated.

EE was walking and stepped on rock and twisted his ankle. Went to DR on 7/30/08 and was told to take the rest of the week off. EE will miss 3 days & is expected to return 8/4/08.

Greasing stacker belt while it was moving. Standing between drive tires and transport tires. Fell and was run over, probably by drive tires. Left foot was crushed, right knee was lacerated.

WHILE CONDUCTING DRILLING OPERATIONS EMPLOYEE WAS STRUCK BY A ROCK FALLING FROM THE HIGHWAY.

EE WAS WORKING FROM WALKWAY AROUND BREAKER FEEDER USING A BAR TO DISLODGE ROCKS. A PIECE OF ROCK SHOT OUT OF BREAKER STRIKING EE ABOVE THE LEFT EYE. ROCK SHOT OUT THROUGH THE PROTECTIVE CHAINS . EE WAS WEARING SAFETY GLASSES.

EE WAW WORKING WITH A BAR REMOVING FROZEN MUD FROM BELOW THE OVERHEAD CONVEYOR OF THE CEDAR RAPIDS SCREEN UNIT WHEN THE MUD CAME LOOSE SUDDENLY HE FELL BACK AGAINST FRAME HITTING HIS SIDE & BA CK.
