Employee was moving explosives from magazine to pickup and slipped on condensation from the humidity causing pain in lower back and leg. Did not go to Dr. until 7/18; doctor released him to work and recommended physical therapy. Physical therapist recommended he be taken off work; first missed day was 7/21. Will be off work until follow up appointment on 7/28.

Loader was backing away from pile. While he was preparing to turn and move forward, a pickup drove in front of him. Person in pickup was taken to hospital for back pain and released. Missed 3 days of work, released with no restrictions.

Struck by falling object

Removing a belly pan off of a truck, there was a block in place to prevent pan from falling to ground. When pan was released, landed on injured's hand. Went to the doctor, X-rays were negative for a break. Doc told him to ice it and keep it elevated, and come back the 12th. Oct 12th was released back to work with a 10lb lifting restriction for one week. Missed one day of work.

Employee was cutting boxes, the knife slipped and cut his right index finger. Required 3 stitches. No restrictions, no lost time.

EE was in quarry truck and reached for something, causing a pain in his back. The doctor has removed him from work until 11/21/08.

Working on fuel tank and got a metal flake in his eye. Flushed it out and kept working. Was still irritated the next day, went to dr to have object removed and wash released back to work with no restrictions.

Cleaning rock from belt, hit right thumb on guard and needed stitches.

Fell from rock bin. Shirt caught on guard, lost footing and fell to ground. Landed on back, hit head on ground. Went to dr to get checked out. Released to come back to work 9/24 with 10 lb restrictions.

Absorption of radiations, caustics, toxic and noxious substances

There are hoods covering the padlocks on the caphouse. When he went to unlock it he was bitten by a bat. There were no lost days, he had to have rabies shots.

He is a loader operator and has had problems with his back before. On July 26, 2007 he had back pain from using levers in loader. Missed work July 27th for Dr appointment. Returned July 30th for an hour and a half, then went home because of pain. Has not been released to full duty yet.

Operating loader, extending back to reach levers and steering, and noticed a sharp pain in his lower back. He missed 11 days of work and was released to come back 5/8/07 with 30 lbs. Lifting restriction. He had elective non-work related surgery on 5/8/07 and did not return to work until 5/15/07. He was released from lifting restrictions on 5/18/07.

Employee was changing oil in a haul truck when he slipped and caught himself pulling and tearing his left shoulder.

The employee was changing oil in trucks at the mine site. He had opened the hood of the Volvo truck. He let the hood down easy and went on about his business. He returned to finish the truck and did not realize that the hood had latched and gave a strong tug to open it, straining his left shoulder.

Employee was helping move a 6"" water line. While pulling on hose, he felt back pop.
