Employee was moving a shaft, when it slipped from hand. Trying to catch with the other hand and mashed finger on hard surface.

Miner went against training and stuck head in pinch point between counterweight and frame of excavator while installing counterweight.

Struck by falling object

Employee was working in the hopper of the breaker when the skid loader approached with a drag attachment on the front, the attachment dropped off and struck him.

Door slammed shut on hand. Fractured left ring finger. He was loading truck on trailer, truck shifted and door slammed shut.

He stepped on a rock and twisted his ankle. Missed work 11/30 & 12/3. Returned to work on 12/4.

Employee was working an un-plugging the hammermill and was int he process of opening the cover. the cover pivole up on hinges. the cever went over center and struck the employee on the right thight causing extersive and severe bruising.

EMPLOYEE EXITED VEHICLE & CLOSED DOOR, DOOR DID NOT LATCH SO HE HELD LATCH WITH RT. HAND & SLAMMED THE DOOR WITH HIS LEFT. THE DOOR CLOSED ON RT. 3RD MIDDLE FINGER CUTTING THE TIP REQUIRING STITCHES.

Inhalation of radiations, caustics, toxic and noxious substances

APPARENTLY THE EE WAS CLEANING A PAIN SPILL AND REACHED ABOVE HIM TO GET SOME MORE PAPER TOWELS HE KNOCKED OVER A CAN OF DENATURED ALCOHOL. THE CHEMICAL SPILLED ON HIS HEAD AND SHOYELERS. HE W ENT DIRECTLY TO RESTROOM AND WASHED HIS FACE ANDEYES. HE CALLED 911.

Fall from ladders

EE WAS CONDUCTING CONT. & RESISTO TEST ON LEAD OUT BINE. WHILE CLIMBING DOWN HIS LEFT FOOT SLIPPED FROM LADDER RUNG CAUSING HIS ANKLE SPRAIN.
