Employee suffered heat exhaustion during work. He missed work starting 7/1/2011. He was released back to work on 7/8/2011.

Employee began missing 1/24/2011 for surgery on left knee. On November 8, 2010 employee twisted knee, but did not seek treatment at that time. This is the first lost time with this injury. Has not been released for work since surgery.

Struck by rolling or sliding object

Employee was removing a receiver hitch from a pickup. The tie bolt rolled off of bumper and cut his thumb. Employee received stitches and was released back to work the same day with no restrictions.

Employee was digging rock out of the breaker when a rock broke loose and fell on his hand causing bruising. Went to doctor & had x-ray, nothing broken. Did not receive report in office until 7/14/2008

While working on hammermill, got cut on right hand. No accident report was filed. When injury healed, he realized there was metal stuck in his hand. He had surgery to remove it on 6/25/08 and was off work until 6/30/08. Missed 3 days of work.

EMPLOYEE WAS SCRAPING LIME DUST THAT WAS STUCK ON THE INSIDE OF THE MAIN SCREEN BIN. HE STATED THAT HE TWISTED HIS BODY TO REACH A CORNER AND STRAINED HIS BACK.
