Employee was going down 3 steps from a trailer after a rain. EE's boots were muddy and EE slipped and fractured bones in left ankle.

This is an accident that happened on July 8, 2016 and didn't become reportable until 8/24/2016. ***Per MM this accident occurred on 7/8/2014 determined by workers comp conference***

Fall from machine

Loader operator climbing down ladder, foot slipped off bottom rung. Employee fell backwards and hit his head.

Struck by falling object

Employee was helping to change overhead belt on the screening plant. When the clamp was released, it hit employee in the arm and cut his wrist. Employee received stitches and was released back to work no restrictions. Employee had some pain and bleeding overnight and went back to the doctor on 3/26/2009. Wound was cleaned and dressed, released back to work no restrictions.

Over-exertion in lifting objects

Loading a pickup with 50lb bags of ANFO. Lower back strain. Went to the doctor and has a 10lb lifting restriction.

Getting out of truck, twisted back wrong and has pain in back. There were discrepancies in the doctors notes, so we waited to hear back from work comp, that is why the time difference for the report.

Over-exertion in lifting objects

Picking up rocks, he felt back snap. Did not report to supervisor or seek treatment until 10/1/07. Doctor reported T2 compression fracture. Will return 10/5/07 with 6 weeks restricted duty.

Employee was helping change hammers in the hammer mill and when they were lowering the cover, he slipped and instinctively reached out to catch himself with his left hand. His fingers were caught between the hammermill lid and liner.

EE WAS STEPPING DOWN FROM LADDER OF PLANT (SCREEN) AND TWISTED HIS LEFT ANKLE. HE CONTINUED WORKING FOR THE REMAINDER OF THE SHIFT. AFTER GOING HOME, DUE TO PAIN & SWELLING, EE WENT TO ER AT LOCAL HOSPITAL AND FOUND HIS ANKLE WAS BROKEN.
