Employee was using a pry bar on the screen deck motor when the bar slipped hitting EE in the face.

Holding onto the door frame when the second employee shut the door and cut EE's finger.

Taking brace off of plant, bolt was stuck and the brace smashed left hand

Changing drill steel on drill.

Removing a bolt with air gun, bolt spun in the other hand causing the injury.

Miner dropped an electric drill on his hand Thursday 5/24/2012. Results of his injury was a break to his hand. He did not tell his supervisor or seek treatment until he returned to work on 5/25/2012. Office was notified on 5/29/2012 that he will be off work until the hand is healed.

Employee rolled a rock off haul road to remove from travel area, strained a muscle, did not seek treatment at that time. That night he went to the dr. for pain & was released with instructions to use ice and Tylenol as needed. He went to the dr. night of 6/21, referred to Des Moines for further treatment, diagnosed torn bicep muscle 6/22 first missed day Scheduled for surgery 6/28

Loader/scale operator running loader experienced pain in lower back. Did not report to office untill 3/30/2009. Went to doctor on 3/31/09, was released back to work with a 25 lb weight restriction.

Employee was lifting drive shaft from loader with chain hoist. Weight shifted and caught finger between shaft and loader.

Running levers five years ago lead to carpal tunnel. *Please see attachment* EE had surgery on 7/24/07 & returned to work on 8/3/07.

Employee was dismounting his truck and caught his arm in the hand rail and twisted his left shoulder.

Employees' were lifting an 8'brace up to position it to be chained to loader bucket. Ground was wet & muddy, brace slipped & fell on EE's arm.

EMPLOYEE'S WERE IN THE PROCESS OF CHANGING SCREENS IN THE SCREEN PLANT AND EMPLOYEE CUT HIS RIGHT LITTLE FINGER ON A STEEL FLANGE. THIS CUT REQUIRED 10 STITCHES.
