MINER WAS STRUCK IN THE LOWER BACK BY THE OVERFLOW CHUTE OF A PORTABLE CRUSHER AFTER IT SHIFTED WHILE BEING STOWED ON THE TRAILER FOR TRANSPORT. THE IMPACT KNOCKED MINER TO THE GROUND.

LIFTING 8" PIECE OF STEEL CHANNEL TO REPLACE A LEG/SUPPORT BEING INSTALLED ON AN OVERLAND CONVEYOR. CARRIED ITEM APPROX 30' AND SET IT DOWN. MINER EXPERIENCED RIGHT TESTICULAR PAIN.
TEAM MEMBER WAS LIFTING INSPECTION DOOR TO VSI. WHILE MOVING TO GET A BETTER LEVERAGE ON THE DOOR TO USE TWO HANDS, THE DOOR SLIPPED AND LANDED ON TEAM MEMBER'S LEFT THUMB. THE EVALUATION BY THE HAND SPECIALIST INDICATES NERVE DAMAGE AND BONE IS OFFSET AND WILL NEED SURGERY.
While flipping gates on the screener, bent over to reach for equipment resulting in a low back strain.

WHILE PERFORMING MAINTENANCE ACITVITIES ON A CONVEYOR, EE DISLODGED MATERIAL THAT WENT BETWEEN THEIR FACE AND SAFETY GLASSES CONTACTING THEIR LEFT EYE. EE WAS EVALUATED AS HAVING A CORNEAL ABRASION AND PRESCRIBED EYE DROPS.
Placing cat walk on fuel tank to avoid falls while checking fuel levels. Cat walk slipped falling to ground before secured. Employee fell to ground with cat walk.
Employee was placing angle iron onto a conveyor when EE caught finger between the metal. EE received a laceration to the tip of the finger that required two stitches.
Employee was repairing the decking on a drop deck trailer upon which a generator sits. To access the decking a small transformer needed to be removed. While attempting to remove the transformer it fell striking the employee's left pinky finger lacerating it. Gloves were being worn.
Mechanic was changing a pulley on a motor and attempted to lift it with one hand when felt a "pop" in upper arm. Suffered distal tendon damage requiring surgery.
EE shoveling material to level the ground out to install stairs and platform to allow for storage of fueling tank.
Team Member removed the mantel nut on an HP-400 Cone Crusher while performing regular maintenance. Team member lost grip of the mantel nut while setting it to the side causing there right hand / wrist to be pinch between the cone liner and mantel nut.

A miner was changing a tire on a large piece of off-road equipment when the new tire tipped over as it was being staged in an upright position striking their foot. A groove/run beneath the tire contributed to this incident.
The employee was moving 480 volt power cords during plant set-up and strained left bicep. 1st aid treatment at that time. On 9/10/2017 pain was getting worse, ee was taken back to the doctor where ee received lifting restrictions and set-up with physical therapy appointments.
EE was lifting a hydraulic rock splitter from a rock when EE felt EE's right shoulder give out. EE was asked if EE wanted to do another job, EE said "no" EE was alright. Monday 6/19/17 EE said EE's back felt like it was going out, at that time went to a less strenuous job. EE's on restricted duty not lifting more than 10lb, PT for 4 weeks and then re-evaluation.
Miner climbed up stockpile to observe conditions of head roll on pea rock conveyor. Miner lost footing and right hand went through drive pulley pinch point, slicing three fingers through miners glove.
Employee was putting an eye on the end of cable. Cable slipped out of employee's hands, striking EE on the chin and causing lacerations to chin and breaking tooth.
Employee was taking down safety rails on screen plant while two other employees were holding rail. Rail slipped and hit employees hand, fracturing right hand.

Employee did receive medical treatment and four stitches to close the laceration.

EE was washing debris from walkway. EE was pulled on a hose that had aggregate on it and EE injured lower back.
Employee was helping remove a barbwire fence and cut his finger on one of the barbs. The cut required 5 stitches and the employee was released and could report back to work.
Employee was placing a chain around a rock that was stuck in a Jaw crusher. The rock shifted and pinched his hand between the rock and the sidewall of the crusher. He fractured a bone on his left hand.
LEG PAIN/STIFFNESS IN KNEE FOR ABOUT A WEEK BEFORE FEELING A "POP" AND BURNING SENSATION DOWN KNEE AND MID SHIN
Employee was lifting power cords overhead and felt a "pop" in his neck and shoulder. He was taken to our doctor who referred him to a chiropractor. The chiropractor adjusted his neck and shoulder.
Employee was dragging the fuel hose to the loader to fuel it up. When dragging the hose back to the tank he felt a pop in his hip.
While performing maintenance on pump 904 employee was assisting with re-attaching the discharge hose to the pump when his right thumb was injured when pinched between the pipe and hoist. Injury consisted of fracture and laceration (2 stitches) to tip of right thumb.
Placing guard on conveyor after replacing drive belts, guard slipped and the employee attempted to catch the guard, guard cut through glove and R ring finger, this required 7 stitches.
While setting up portable crusher, employee bumped right knee on leg of plant.
Moving return rollers, pulled muscle in back.
While servicing the cone crusher, operator's left finger tip was crushed when keeper ring fell while being removed. Medical treatment included xray, antibiotic and pain med, and dressing of wound. Distal finger bone is broken and nail was removed.
Miner received steam burns to his right arm while attempting to service a pump. The pump had lost its prime and had run dry for a period of time building up heat and steam. Miner attempted to open the filler plug and did not notice pump body was hot, and when he unscrewed the plug the steam escaped burning his arm.
Employee received minor break in the skin to his left small finger, while not wearing gloves. 6/23/13 the employee was taken to the emergency room due to an infection in his left small finger. The infection worsened and eventually the doctor decided to perform a partial amputation to the second joint on his small finger.
During the lateral adjustment of a set of forks on a Cat TL1255 Telehandler, the worker cut his left pointer finger while he was pulling and another worker was pushing one of the forks. The fork suddenly slid quicker than was expected, catching his finger between one of the hooks and the bar. He was wearing gloves at the time. his finger received nine (9) stitches.
