Right pinky finger got pinched between the spacer and the housing of the bearing unit. While trying to remove the lower shaft and bearing on a double screw material washer. Received 1 stitch at ER to close up laceration.

The screen needed more oil and to be greased. The injured employees added oil. The employee that got injured decided to put their left hand between the timing belt and sheave to check if the bearing was "locked up," the counter weight fell freely pulling their glove and left hand between the timing belt and sheave. Injured employee had 3 fractured fingers.

Employee was adjusting the primary jaw crusher. The pitman got hung on the cheek plate and did not come back with the toggle plate like it should have. The employee placed their hand on the spring bolt to observe what was catching the pitman and the toggle plate fell on the employee's right hand, causing a laceration that needed stitches.

Employees were replacing a tail pulley. It dropped into place unexpectedly and the shaft fell onto the tip of the employee's ring finger on their right hand severely mashing it. The fingertip was later amputated at the first joint behind the fingernail, at the ER.

The employee was cutting 2" x 2" x 1/4" square tubing for handrail supports and catwalk structure. The employee had cut three pieces 3'6" long. Each stick weighed approximately 19 pounds. The employee grabbed three sticks of the tubing and carried them up the stairs to the catwalk. As EE was walking up the steps EE felt a pain in EE's lower back.

Employee was attempting to adjust seat in a PC490-11 excavator with right hand when weight shifted forward pinching right little finger in the frame of the seat. Resultant was a cut to the tip of right little finger. The employee was treated with surgical adhesive and a tetanus shot. Employee returned to work that morning with no restrictions.

Employees lifting cab on Bobcat. Front door left open. Door slammed, smashing one employee's right little finger. Employee taken to ER. Xrays revealed no broken bones. Finger nail was removed by ER doctor, wound clenaed; bandaged. Pain meds prescribed; tetnus shot given. Employee released to return to work.

On 07/11/2018 while doing routine shop maintenance the employee realized something was in EE's eye. EE attempted to wash EE's eye out but did not report the incident. On 07/12/2018 EE went to the doctor who removed a piece of dirt from EE's eye. The employee was not wearing EE's safety glasses.

On 3/20/18, employee strained their back while lifting 40 / 50 lb. bags of ANFO. EE reported the strain on 3/21/18 but choose not to seek medical treatment until 3/28/18 at which time it was deemed reportable. This is being reported within the 10-day reporting requirement of the injury being deemed reportable.

A haul truck driver claimed ee hurt back after removing several "cantaloupe" size rocks from the haul road. EE did not inform supervisor until Friday the 11th. An investigation was begun involving safety, risk management and insurance providers to determine cause of injury being work related or not.

Employee was washing out the surge tunnel, their eye started bothering them, might be debris in eye. Employee worked till 6.30pm did not report incident till 9.00pm. Next morning their eye was red irritated and swollen half way shut. Employee went to dr couldn't find anything was given a pres. for drops and returned to work.

was pulling up the guard with his right hand and reaching across his right arm to put a bolt in, something popped in his right shoulder, started to hurt. went to doctor to get checked out, DR. said looked like strained tendon or tear in the tendon. DR.wants to see patient again. scripts Norco for the pain at night and Etodolac for inflammatory.

Employee clocked out after shift. Was standing outside break room leaning against door frame wind caught door and smashed finger. Employee had laceration and fracture to right little finger. Employee received sutures and was released for regular duty with an antibiotic and pain medication and no restrictions.

Customer truck driver was trimming their load of rip rap. Rip rap was preventing the tailgate from closing. When driver dislodged the rip rap the tailgate closed on the middle right finger resulting in amputation at the first knuckle.

Two employees lifting printer enclosure from a pallet into the back of a pickup truck with a topper. While placing the enclosure on the tailgate one employee felt a twinge in their back. Employee was diagnosed with tilted down S1 vertebrae. Employee is a support person and not an actual "miner".

EE was washing and cleaning up a primary impactor to paint on 6/5/14. Removed a wedge from side access door, set it on frame to do more cleaning, wedge dislodged falling onto main frame then glancing off onto his foot and ankle. He had pain but continued to work. 6th he came into work with foot swollen and having a hard time walking. Went dr 6/9/14, major contusion, off 3 weeks.

Mechanic used rag to check for debris on matting surface of yoke. Metal splinter entered left ring finger. Splinter pulled out mechanic said was fine. On Monday 5/19/14 mechanic reported to plant manager that his finger was hurting and requested medical treatment. Metal splinter was removed by doctor, there was abscess so prescription antibiotic was prescribed

Repairing rip rap deflector plate on scalper screen. Angle iron was laid on roll away cover which is at a slant. He was bending over to pick up another piece of metal for repairs then piece slid down chute & smashed index finger against the cover he was using as a hand hold.

Work boat positioned by dredge. Employee lifted lid of tool box located on work boat to get clamp. Lid was resting against dredge cable which did not allow the lid to open completely. When employee was done in tool box the lid slammed down quicker than normal because of the pressure from the cable. Tip of the left middle finger was cut.

Mechanic removing bin out of top shelf of service truck tool box. Had both hands on each side of bin while pulling it out. When the back portion of the bin came out it dropped and smashed his left hand pinky finger between the bin and the shelf of the tool box. Received 10 stitches to his left pinky finger.

Employee was shoveling material from under the surge tunnel conveyor when he felt pain in his arm and hand. *The reason for the late reporting is the doctor has been evaluating this injury to see if it was new or a reoccurence of an injury to the same area in the previous year(2009).

Employee lifted the hood on his loader and did not lock it. As he bent down to pick up the jumper cables, his hard hat fell off his head. He picked his hard hat up and hit his head on the cab as he raised up puncturing a hole in the back of his head. He went to the hospital and did receive stitches. He returned to work that afternoon.

Changing a return roller. The employees thought that the brackets that hold the roller in place were on top of the frame of the convoyer but were not, they were on the bottom. One employee let his side loose first, letting the roller free to swing into the other employees finger, smashing it into the frame.

Employee was throwing welding leads over a highwall for work that had to be done on the water line that came from the pit pump. When he threw the welding leads, he felt a pain in his shoulder. He did not wish to see a dr at the time. On 3/30/09 he requested to see a dr. On 4/27/09 he was prescribed four rounds of physcial therapy making this a reportable injury.
