When removing the plate underneath an excavator, the bolt freed jolting the employees shoulder and arm backwards. The incident occurred on 9/3, but became reportable on 9/9.

Over-exertion in pulling or pushing objects

Back strain: Employee tried taking pain pills and therapy but it did not work. Doctor told her not to work until they find out what it is. Shots have been helping the pain. No results have been given.

Struck against stationary object

Employee trying to turn off ac in cab of loader-stuck hand thru vent hole and hit finger came in contact with fan blade. EE was attempting this while sitting on loader's not looking up at ac controls direction cover was missing off of the ac vent.

EE WAS CLIMBING UP TO SECONDARY MILL AND WAS STRUCK IN EYE BY FLYING ROCK. EE WAS NOT WEARING SAFETY GLASSES AS REQUIRED BY SAFETY POLICY OF COMPANY. EE WAS NOT ASSIGNED TO THIS AREA AND HAD L EFT HIS JOB POST WITHOUT PERMISSION.
