Cutting left hand equalizer bar end pin out of roller frame and slag popped off went into right ear causing a thermal burn.
Torqueing mount bolts for 992G wheel loader unit and ratchet gear stripped causing employee to fall backwards straining lower back
Installing roof strap, while installing roof strap it slipped down striking employee in left cheek and ear. He received fifteen stitches. (15 stitches).
The employee was located at the off-side rear of the Fletcher roof bolting machine, and a roof bolt and plate had fallen off the machine. The employee bent down to pick it up and the operator began tramming the machine at this time and backed the machine up onto the employee's right foot.
Drill steel fell out of hole mashing the end of the left pinky finger.
Pushing on a shot and backed over a big rock. Jarred the victim. Had pain running from right hip to right ankle. Was x-rayed and released.
Operator was tramming a dozer to the pit and alleges he pulled a muscle in his lower back. This was not reported until the 27th.
Pushing shot to loader and backed over a big rock. Dozer went backward over rock then forward throwing employee forward. Pulled muscle in stomach area.
Operator was backing up the dozer when he hit a rock with one track. Said his neck was stiff and went to hospital the next day. Said he had a pulled muscle.
EE was drilling a hole for his roof bolt when the drill steel broke, throwing a piece of steel, striking him in the mouth. He required 8 stitches on his lip and he broke a tooth, which will require dental work.
Employee was backing the roof bolter up to reposition and roof bolt machine came into contact with the employee when it slid to the side, pushing him into the rib.
Hood on a D8T Cat Dozer stuck open and EE moved it causing the hood to slam shut. It caught his middle and ring finger between the frame and the hood.
Victim was assisting with the install of the final drive in D-10 dozer. They were using alignment bars and hoist on boom truck. The weight shifted it caught his hand between drive and undercarriage. Transported to Highland Regional ARH then to UK Medical Ctr. Two fractures and laceration. Treated with cast and stitches & released. Still off work to date.
Employee was mounting his drill, stepped onto the first step started to lift to the second step ,pulled a muscle in his lower left leg.
While installing a roof bolt, a spur on the bolt caught the EE's glove as he started his rotation causing his thumb to twist. He finished his shift and worked the next 8 shifts before consulting a doctor. He told his foreman that he was fine and did not require attention. On 5/2/11 he informed us that he had consulted a doctor.
While running Jay Miner softball sized rock was thrown by chain and struck person on right shoulder.
Loading side post jack fo Fletcher Bolter onto a two man personal buggy caught right hand cutting middle finger requiring 6 stitches.
Victim was pushing shot rock with a 475 Komatsu dozer. He backed over a rock and his right hip started hurting. Bruised right hip. He went to_Pikeville Medical Center ER on Feb. 22nd. Employee returned to work on February 24, 2011
Drill steel fell out of drill hole and hit his left finger.
Employee was roof bolting. He felt pain in his chest and left arm, taken to hospital. Admitted for possible heart attack, released next day and told there was no cardiac event. He was diagnosed with a possible pulled muscle and given two days off. Returned to Dr. complaining of left arm pain and was told to get an MRI, employee off until given an MRI.
EE was helping another ee repair the suction on the drill head of a roof bolter where the repair were completed ee inadvertently lowered the drill head on ee leg causing swelling or bruising to the upper right leg & ankle.
Employee was roof bolting. He stated that he twisted around to retrieve a roof bolt when he felt a pain in his back. He worked the rest of his shift and went to the emergency room on 12-21-2010 at 10:00 a.m. His was told that he had a pulled muscle.
He was helping the mechanic weld on a 988F loader. He was in the process of welding and apparently got flash burns in his eyes.
Rock wheel on grinder busted sending a chip of rock into ee chin, cutting a small gash in the bottom.
While preparing to take down a material stacking conveyor on impoundment, located on K7 2009 belonging to Infinity Coal Co, EE was standing on tail end of stacker cutting holes in sideboards for chain hook points. A backhoe operator was removing material from around support legs and turned conveyor over, causing cuts on 3 fingers on right hand of inured EE, requiring stitches.
Employee was operating pinner and was retracting a piece of starter steel from the top as the steel cleared the top he was struck by the steel hitting his right arm.
While bolting, employee had his right hand on the pizza pan. While he was trying to get the rotation hose out of the way, the lever was accidentally pushed causing the bolt to raise up, catching his right fingers between the pizza pan and the top.
EE was taking suction hose off near the pinner head. Another EE moved the pinner catching EE's middle finger in a pinch point. This mashed the finger requiring stitches. Note: Paper work got misplaced.
Employee was moving the roof bolter when he was caught between the roof bolter and the mine rib. This caused a compound fracture to the femur bone of the left leg. This required surgery for placement of a pin in the femur bone.
Changing out bad production car. Man cleaning coal off the front car and got in the way of pusher jack. He did not realize pusher jack was coming in contact with his leg.
Operating a D-11 dozer, backed up hitting a rock that gave him a strong jolt, caused pain in lower back.
EE felt sick with back / side hurting and decided to go to the doctor. Did not inform the Foreman that he had an accident. Foreman thought he was sick.
Employee was preparing to start drilling the hole for a roof bolt when he slipped and hit the rotation lever, causing him to twist his wrist.
Injured was hanging a roof bolter cable, as the roof bolter was tramming tension came into cable causing him to cut his finger on the insulated hanger, requiring 2 stitches.
Miner was cutting sandstone roll. A piece of cutting bit came off miner & hit employee in upper left arm.
Bolt men were suppling bolter. Injured EE was on offside reaching across bolter to place bolt supply. The operator thought he was out of the way and trammed bolter forward onto injured's left foot.
Employee was loading a truck when dust and fine pieces of rock came back on him hitting him in his right eye. His right eye lid was bruised but no apparent damage to his right eye.
Employee was greasing equipment when the hose of the grease gun came loose from the fitting. The end of the grease hose struck employee in left eye.
Employee was dropping loaded railroad cars when his foot was caught between the railroad car wheel "Truck" and the rail.(Foot crushed with all toes amputated)
Employee had entered cab of haul truck and was about to close the door when a gust of wind blew the door shut and caught the employees little finger against the door jamb.
EE states he was washing down first floor of the plant, when the clean up pump stopped working. The EE pushed the stop button, opened the cover and pulled belt to see if motor was locked up. A co-worker the reset button in the control room, causing pump to start. Injured EE's (R) hand was pinched between teh belt and pulley.
Grinding cutter drum preparing to weld bit lugs to drum. Metal got into eyes. Employee did have safety glasses on at time of accident.
While greasing a 992 loader, the grease plug came out and struck employee on the cheek, below eye.
Removal of an empty counterweight carrier from a D11 rcd tractor was found to be unbalanced which caused the front to be heavy when pins were removed. Anticipating the rear was heavy, a restraint chain had been installed on rear to prevent tipping but should have been on the front. Victim's right lower leg was momentarily pinched between crawler track & the brace of the carrier.
Employee ran his dozer into a hole and hit his leg against the door. He refused medical treatment at this time, stating he would be o.k. On Aug. 25, 2008, he went to the doctor with swelling on the knee and received 5 days light duty. He returned to work 9/3/08.
EE was changing a crawler on a Jow 14-15 miner. He was setting a jack when it slipped and mashed his thumb.
Employee was attempting to put a strap in place, when it slipped, striking him on the chin.
Employee left a message on voice mail 10/3/07 that he may not work the next day. A message was left on 10/4/07 that he had been scheduled for an MRI on his back. Emoloyee operates a D-10 Cat Dozer.
Employee fell between the miner power head and a push beam "car." The push beam was knocked forward causing employee's leg to be caught between powerhead and push beam.
Putting pusher steel in starter-starter hung in hole, steel came out and hit him on left hand and cut his palm. Sent to Welch ER.
