EE was walking a man lift down a pit road when the tires hit a 3-4 inch deep washed out rut caused by rain while traveling 2-3 feet off the ground. The rut caused the basket to bounce causing EE to be lifted into the air, when EE came down EE's right foot landed on the throttle assembly causing EE's right knee to twist.
Operator was making a hard left turn up a ramp when the right front tire caught traction and pulled the cab over on the right side of machine.
Loading a shot, was struck in the head by the moving Auger Tube. (recordable after the appointment on 2/13/24)
At approximately 3:05 PM EE was walking from the plant to the C-9 Belt area when EE passed the Oversize chute a 1 1/2 -2 inch rock bounced out of the screen 2 chute and hit EE on the left thumb. A splint was placed on EE's left thumb to minimize movement. Restrictions given from clinic were no typing, pushing, pulling and to wear splint.
Employee was reversing down from the 1/4' pea gravel stockpile ramp in the JD844L loader with an empty bucket. The bucket was about midway up. When the rear tires contacted the grade change at the bottom of the ramp it jolted the loader causing the air ride seat to bottom out causing pain in the left hip area.
While hauling stripping equipment, the haul truck driver contacted a dip in the road which caused the seat to bottom out. The driver experienced back pain, and was diagnosed with back spasms.
While loading a rib line hole using an anfo hose, the employee felt a sharp pain in their upper back/neck area.

Installing a saddle in conveyor and the boom truck block hook struck the employees foot causing crushing injury without fracture.
While loading haul trucks, a rock slid down from the pile and landed in the loader bucket, causing the rear to lift off the ground and return with force. The operator seat bottomed out, causing a back sprain.
Employee received an eye injury when a hydraulic hose fitting came loose/broke and sprayed hydraulic fluid into the eye. EE was wearing safety glasses at the time but they were not preventive. Prescription eye medication was administered.
Employee struck a swale cut into the haul road while returning to the pit and suffered a sprain to EE's left ribs and middle back.
Employee strained back while operating loader. This claim is questionable but currently meets the definition of a reportable injury.
While driving haul truck loaded with rock, miner engaged manual brake retarder which resulted in locking up rear wheels and put truck in uncontrollable slide. Truck came to stop at end of haul road and ran into a pile of material.
Employee strained EE's back while operating mobile equipment through the day. The exact cause of the strain is undetermined. EE reported discomfort on 03/29/2018 but did not request medical treatment until April 2, 2018.

ON 1-6-17 AROUND 7:00 AM, IT WAS SNOWING AND THE ROADS GOT SLICK AND 2 DUMP TRUCKS SLID OFF THE ROAD INTO THE DITCH AND HAD ROAD BLOCKED AT FRONT ENTRANCE, EMPLOYEE WAS GOING DOWN THE ENTRANCE ROAD, APPLIED BRAKES AND STARTED SLIDING AND COULDN'T STOP. LEFT FRONT TIRE HIT THE REAR TIRE ON DUMP TRUCK, THEN LEFT MIRROR HIT THE BED AND BROKE IT OFF. STEERING DAMAGED.
Scaler working a loose brow was using maximum pressure on the pick when slab broke loose. Slab hit the boom of scaler, lifted the rear of machine appx 5 feet off the ground. Scaler slammed back to the ground jarring the operator.
This report stems from an incident on 08-10-2016 not reported until later - first doctor visit 09/07/2016. Haul truck backed into the loader. Truck avoiding spillage contacted loader dipper.
Technicians were in the process of removing the transmission on a 988G. They had installed 4 lifting links with bolts. The chain being used was a four legged spreader chain. While pulling on chains with the boom, the lifting bolts were pulled out of their threaded holes allowing the chain, which was under tension to flip up and hit the technician in the face.
Employee using skid steer to clean up around tail section made contact with concrete foundation causing skid steer to stop abruptly and throw operator forward in seat and head to make contact with top of cab causing tightness in neck. 12/07/15 employee returned to medical facility and was put on restrictions and off work.

11/10/14 employee notified mgt they hurt their back 11/7/14 while driving haul truck in yard. Employee was taken to medical facility diagnosed with sprain/strain of shoulder/upper arm. Employee could still perform duties of driving truck. 1/15/15 mgt was notified employee had herniated disc and given restrictions making this a reportable injury
Employee arrived at work on 3/27/12 and complained their back was hurting. Employee had been loading production haul trucks from muck pile the previous day using a Kamotsu 600 wheel loader.
Employee stated his hip began to go numb and his leg also. He told the track hoe operator that he needed to park the truck and seek medical attention. Employee was taken to a walk-in clinic by another employee. The employee was driving haul truck at the time the employee told track hoe operator.
Employee legs been hurting from running 988B loader.Clinic-possible strain.Employee said from seat, seat replaced original was found in good working order.Continued working 10 weeks but due to reduction in work force laid off 10/22/10.Additional clinic visits determined torn right medial meniscus.
Employee driving haul truck told fellow employee seat was hurting his back but did not report to foreman. Fellow employee told forman at end of shift. A new seat was put in haul truck. When employee returned to work on 7/12 he told foreman his back was hurting due to the seat in haul truck. Employee was taken for medical exam.
Truck was backed up grade to place material in a washed area of the road. When operator raised the truck bed, the truck began to roll and operator lost control. The truck traveled over an embankment and rolled. As a result of the incident the operator received a cut to the head and broken knee.
Employee operating a Cat 613 Water Wagon hit a dip in road and felt pain in back/rib area. Employee taken to clinic, x-rays taken-negative. Overhead restrictions did not impede employee from doing normal job. Cat Scan negative 6/30/09 it was determined that the employee had a soft tissue injury and physical therapy required, injury then reportable to MSHA. No lost work days.
Employee was working in a loader for entire 10 hour shift. Next day at beginning of shift, employee reported that his eyes were stinging and red. Took employee to Promptcare and reported that eyes had an infection in both eyes due to diesel smoke. Employee was given antibotics and steroids for treatment. No work was missed or restriction of duty.

Jarred in pit loader. Did not hurt day of accident; went to Dr. 5/23/2007. Restricted duty.
An employee was driving a water truck to water the lower part of the haul road. He lost control of the truck and ran the truck up on the berm on the drivers side turning the truck over on the passenter side where it came to rest. The truck was inspected and it was found an airline had ruptured during the operation of the truck. The employee was not wearing his seatbelt.
EE states back pain caused by operating end loader in a stockpile/yard application. Deemed work related after EE's return to work, by insurance carrier.
An employee with 18 months experience was hauling shot rock to crusher. He came around right handed curve got off left shoulder, over corrected causing truck to turn over. Employee's left elbow cut and scratched. Employee was transported to hospital where he received 7 stitches to left arm. X-rays revealed bruised left shoulder and arm.
Employee trying to turn off ac in cab of loader-stuck hand thru vent hole and hit finger came in contact with fan blade. EE was attempting this while sitting on loader's not looking up at ac controls direction cover was missing off of the ac vent.
At 7:45 a.m. on October 11,2004, employee with 18 months experience, was operating a 988 Loader moving shot rock to a dry area. Employee hit a pot hole causing him to get a catch in his back. Employee taken to the doctor and was given mediction and told to take 2 days off work.
OPEARTOR WAS DUMPING OFF ON STOCKPILE WHEN THE BACK OF THE PILE SLUMPED OFF. THE END DUMP SLID BACKWARDS DOWN STOCKPILE WITH END DUMP COMING TO REST WITH ITS. FRONT WHEELS IN THE AIR. UNKNOWN TO THE OPERATOR A LOADER HAD LOADED A CUSTOMER FROM THE BACK OF THE PILE, CAUSING THE PILE TO SLIDE WHEN HE BACKED UP.
EMPLOYEE WAS MOVING GRAVEL FROM PRODUCTION AREA TO STOCKPILE AREA. AS HE BEGAN MOVING FORWARD WHILE LEVELING AND LOWERING HIS BUCKET HE LOOKED OUT HIS SIDE WINDOW TO CHECK ON ANOTHER PILE. HIS CUTTING EDGE STRUCK THE GROUND. ALTHOUGH HE WASWEARING HIS SEAT BELT HE STRUCK HIS HARD HAT AGAINST THE WINDSHIELD. THE HAT FORCED HIS SAFETY GLASSES FORWARD CUTTING HIS NOSE, REQUIRE 3 STITC
