Stepped out of cab to get off of machine that EE was measuring. Turned to position self to step down backwards & grab grab-iron. This is the motion that tweaked EE's back. EE was in the parking lot not in the mine itself. EE has prior back issues from past injuries.
Employee was going down 3 steps from a trailer after a rain. EE's boots were muddy and EE slipped and fractured bones in left ankle.
Twisted knee while carrying a box of explosives
Miner slipped on a rock pile while catching a sample. This caused a pain in the back.
This is an accident that happened on July 8, 2016 and didn't become reportable until 8/24/2016. ***Per MM this accident occurred on 7/8/2014 determined by workers comp conference***
Miner was getting out of the Terex truck and missed the bottom step causing a sore back and leg.
Employee was getting into his truck when he slipped and hit his ribs on the hand rail.
Miner twisted his right knee while going to his truck. Icy patch where he was walking.
Employee was moving explosives from magazine to pickup and slipped on condensation from the humidity causing pain in lower back and leg. Did not go to Dr. until 7/18; doctor released him to work and recommended physical therapy. Physical therapist recommended he be taken off work; first missed day was 7/21. Will be off work until follow up appointment on 7/28.
Employee was climbing out of 6 wheel articulated truck and slipped off of the step causing a ligament sprain in his right knee. Did not report to supervisor until late that day and it was reported to the office on 4/26/2011. Employee went to the doctor on 4/26 and was taken off work for an estimated 2 weeks and is thought to be released 5/9/2011.
Employee began missing 1/24/2011 for surgery on left knee. On November 8, 2010 employee twisted knee, but did not seek treatment at that time. This is the first lost time with this injury. Has not been released for work since surgery.
Employee scooping snow, twisted and hurt left knee. Went to doc 1/8/10, no treatment. On 1/21/10 he went in for physical therapy. On 1/27/10 he went in for surgery. He missed part of day 1/27/10, and is expected to be off until 2/1/10.

Fell from rock bin. Shirt caught on guard, lost footing and fell to ground. Landed on back, hit head on ground. Went to dr to get checked out. Released to come back to work 9/24 with 10 lb restrictions.

EE WAS STEPPING DOWN FROM LADDER OF PLANT (SCREEN) AND TWISTED HIS LEFT ANKLE. HE CONTINUED WORKING FOR THE REMAINDER OF THE SHIFT. AFTER GOING HOME, DUE TO PAIN & SWELLING, EE WENT TO ER AT LOCAL HOSPITAL AND FOUND HIS ANKLE WAS BROKEN.
Employee was exiting haul truck and slipped. He tried to right himself and in the process twisted his back.

EE WAS WORKING ON QUARRY DRILL. THE BOOM WAS OUT AND HE WAS MANUALLY PUSHING BOOM BACK INTO PLACE. HE WAS STANDING ON SOME CAT TRACK NEXT TO DRILL AND WHEN HE STEPPED DOWN HE TWISTED ANKLE CAU SING HIM TO FALL INTO DRILL FRAME.

EE WAS CONDUCTING CONT. & RESISTO TEST ON LEAD OUT BINE. WHILE CLIMBING DOWN HIS LEFT FOOT SLIPPED FROM LADDER RUNG CAUSING HIS ANKLE SPRAIN.
WHILE CLIMBING INTO LOADER EE SLIPPED ON STEP & BROKE HIS HEEL.
EE WAW WORKING WITH A BAR REMOVING FROZEN MUD FROM BELOW THE OVERHEAD CONVEYOR OF THE CEDAR RAPIDS SCREEN UNIT WHEN THE MUD CAME LOOSE SUDDENLY HE FELL BACK AGAINST FRAME HITTING HIS SIDE & BA CK.
EMPLOYEE WAS STEPPING OUT OF PICKUP & TWISTED HIS BACK CAUSING MUSCLE SPASMS IN UPPER MIDDLE BACK.
