EE was fatally injured when struck by ice dislodged from a slurry pipeline.
EE was changing tire on 10SC shuttle car, had new tire on and was finishing tightening bolts, when the tire exploded and the 2 safety rings struck the victim in the face, left hand and chest area.
The individual was discovered in a structure designed to settle aggregate fines. His job duties involved keeping the area clean from spillage & debris. The Structure wall is over 4 feet high and there was no reason for the individual to have access to the top of the structure. He was discovered floating in the structure.
CO. MIS-JUDGED AMOUNT OF MILKY LIME IN TANK. 12/8 THEY REMOVED TANK DOOR & HARD SCALE FORMED OVER DOOR. CO. CONTACTED ONYX TO CUT SCALE AT BASE TO REMOVE WHAT CO. SAID WAS 2-3 FT OF LIQUID. ONYX USED HIGH PRESSURE WATER TO CUT INTO SCALE. ONCE TECH STARTED, SCALE GAVE WAY & TECH WAS TRAPPED UNDER LIQUIDE THAT CAME OUT. MORE LIQUID WAS IN TANK THAN THOUGHT. CO DID NOT SOUND TANK.
Highwall collapse resulting in death of loader operator.
EMPLOYEES WERE CHANGING BOLTS ON THE "DUTCHMAN" ON AN ELECTRIC SHOVEL. THE DOOR OF THE DIPPER WAS BLOCKED OPEN WITH A METAL BAR THAT WAS WELDED IN PLACE. FOR REASONS UNDER INVESTIGATION, THE BLOCKING BECAME DISPLACED AND THE DOOR CLOSED, PINNING TWO EMPLOYEES BETWEEN THE DOOR AND THE DIPPER.

Treecutter was cutting a tree that had another tree leaning into it. Cutter was fatally injured when the leaning tree fell on him.
EXCAVATOR OPERATOR WAS LOADING A 785 ROCK TRUCK WHEN THE WALL FELL STRIKING THE OPERATORS COMPARTMENT OF THE MACHINE.
The mine owner received fatal injuries when he was caught between a 980C Cat Front End Loader & a tractor trailer. The coal truck was having trouble driving up the steep haul road. The end loader was moving into position to allow the victim to hook the steel pull rope to both pieces of equipment. The end loader struck the victim resulting in fatal injuries.
VICTIM AND TWO EMPLOYEES WHERE IN THE PROCESS OF CHANGING OUT SURFACE HIGH VOLTAGE TRANSFORMERS. DURING THE PROCESS THE VICTIM CAME INTO CONTACT WITH THE HIGH VOLTAGE CURRENT, RESULTING IN INJURIES INVOLVING BURNS, CUTS AND BRUISES. VICTIM LIVED UNTIL 10/29/04 AT 3:00 P.M.
NO TAG LINE USED ON THE CONVEYOR BEING HOISTED. WORKING WITHIN 10 FEET OF ENERGIZED OVER HEAD LINE.
WHILE TAKING A LOAD OF FINES TO THE STOCK PILE, EMPLOYEE WAS RETURNING TO THE PIT AREA (WITH A LOADED TRUCK). WHILE ROUNDING A CURVE IN THE HAUL ROAD, EMPLOYEE WENT THROUGH A BERM CAUSING THE TRUCK TO ROLL. EMPLOYEE WAS CRUSHED BY THE TRUCK HE WAS DRIVING.
A fire occurred on the front end loader. The operator (victim) was dumping a bucket of refuse rock on a stockpile. A nearby truck driver noticed an oil leak on the loader and radioed the victim to shut down. The victim dumped the loaded bucket, backed up and drove the loader forward. The loader burst into flames. Fire was located between operator's cab and engine compartment.
Not sure yet. Still under investigation. Theory #1: Natural death. Theory #2: A rock fell from conveyor and struck employee at a glancing blow, causing him to strike metal structure fracturing 12 ribs and puncturing right lung. There were no witnesses.
EMPLOYEE WAS HELPING CHANGE OUT A BOTTOM BELT SCRAPER AT 3 EAST MAINS #1 TAILPIECE. EMPLOYEE WAS PULLED INTO THE TAILPIECE RESULTING IN FATAL INJURIES.
HE WAS ASSISTING A CERTIFIED ELECTRICIAN IN INSTALLING A 7200 VOLT PLUG (CATHEAD) WHEN HE APPARENTLY CAME IN CONTACT WITH AN ENERGIZED RECEPTACLE ON THE POWER DISTRIBUTION BOX (VCB). THIS RESULTED IN A FATAL.
OPERATOR WAS RUNNING D4H CAT DOZER AND WAS FOUND LAYING BETWEEN BLADE AND DOZER. INVESTIGATORS ARE NOT SURE AT THIS TIME THE CAUSE OF THIS ACCIDENT.
Employee attempted to replace starter on Hyster H50XM forklift, jacked forklift up with a 10 ton hydraulic floor jack. He did not put any blocks or shorwings under forklift to prevent it from falling. He then got under forklift to repair starter and forlift then fell off jack causing major head injuries. Employee died.
EE was making electrical repairs to the load side loop on a deenergized pole mounted solid blade disconnect switch. He had finished the repair work and was starting to come down the pole. Some how he received an electrical shock and fell down the pole.
The employee had backed under a feed chute to get a load. He dismounted the truck to activate the feed belt. He mounted the truck and moved it to spread the load evenly as it filled. He dismounted the truck to shut off the feeder and the truck rolled forward, pinning him between the truck and a cement pillar.
(3) MINERS WERE WORKING TO INSTALL & MOVE A NEW CONVEYOR BELT. PILAR WAS FEEDING THE BELT OFF THE ROLL TOWARD THE HEAD PULLEY. VICTIM WAS PULLING THE BELTING TOWARDS THE TAIL PULLEY WITH A 3/4" ROPE ATTACHED TO THE LACING HOLES W/3 STRANDS OF WIRE. THE PULL ROPE DETACHED FROM THE WIRE & HE FELL BACKWARDS ONTO A CONCRETE SLAB. HE WAS 12'ABOVE THE GROUND, FALL PROTECTION NOT WORN.
Causes Unknown - Still under investigation. The victim was helping switch rail cars from a siding when he was struck & run over by the moving cars.
5 North face area, right side, cross 5 to 6 at survey station 38 and 82. Coal and rock fell out on right side of miner knocking man's head and neck into side of the continuous miner.
EE had just finished installing timbers and began to observe the top for the miner operator who had begun to cut the end cut in the #2 entry. A section of roof measuring 18' wide 0-60" thick fell on ee causing fatal injuries.
While cleaning material buildup from underneath field conveyor using a Bobcat loader, the employee climbed out of the vehicle to inspect a pin which had worked loose on the boom arm. After climbing back into the Bobcat, the employee bypassed the safety devices and operated the equipment standing up and became pinned between the boom crossmember and the roof of the cab.
EMPLOYEE WORKING NEAR EDGE OF HIGHWALL FELL 90 FEET AFTER SLIPPING OR STEPPING IN WRONG DIRECTION.
BAD BRAKES/BAD STEERING, STEEP GRADE 15%. EMPLOYEE WAS TEST DRIVING THE WATER WAGON ON THE ACCESS ROAD WHICH IS ON THE SOUTH SIDE OF HOMESTED PIT. EMPLOYEE MUST HAVE LOST HIS BRAKES, POPPED OUT OF GEAR, LOST STEERING & TRIED TO JUMP. HE WAS CRUSHED BY THE TIRE.
FELL FEEDING ROCK BOX. THE ROCK BOX WAS PLUGGED W/ A LARGE BOULDER & 3 EMPLOYEE'S ASSESSED THE PROBLEM. LOCKOUT CONVEYOR LINE, CO-WORKER AT JAW TOWER AND FATALY INJURED EE TRIED USING A PRY BAR TO FREE PLUG & HE CRAWLED INTO THE ROCK BOX TWO TIMES, ON THE SECOND TIME MATERIAL ENGULFED HIM.
A "HORSE BACK" (ROOF FAULT) MEASURING 2' X 14' X 38' DISLODGED FROM THE MINE ROOF PULLING 2 ROOF BOLTS FROM THE LAST FULL ROW OF BOLTS, FALLING ON THE VICTIM WHOSE LOCATION WAS NEAR THE 2ND ROW OF BOLTS, CAUSING CRUSHING BLUNT FORCE TRAUMA, RESULTING IN FATAL INJURIES BEING SUSTAINED.
A FATAL FALL OF ROOF ACCIDENT OCCURRED ON THE 003 WORKING SECTION. THE VICTIM WAS WORKING WITH THE SECTION CREW AND HAD BEEN INSTALLING TIMBERS DURING RETREAT MINING. THE CREW HAD TEMPORARILY HALTED PRODUCTION TO OBSERVE THE MINE ROOF, WHEN WITH LITTLE WARNING THE MINE ROOF BEGAN TO COLLAPSE. THE 5 MINERS RAN TO ESCAPE THE ROOF FALL. FOUR OF THE MINERS ESCAPED.
AS THE VICTIM WAS WASHING DOWN THE AREA OF THE BUNKER, HE BECAME PINNED BETWEEN AN UPRIGHT BEAM AND THE RAIL BUNKER CAR. THE RESULT WAS FATAL CRUSHING INJURIES.
WHILE HAULING DIRT DOWN THE IMPOUNDMENT HAUL ROAD, IT APPEARS EMPLOYEE LOST CONTROL OF THE 10 WHEELER TRUCK HE WAS DRIVING. HE JUMPED FROM THE TRUCK AND IT STRUCK HIM CAUSING FATAL INJURIES.
CRANE TIPPED OVER INTO WATER WITH OPERATOR IN MACHINE.
The injured person, an employee of Service Transport, was delivering diesel to the quarry fuel tank. The driver was the only person at the tank when a hose separated from the truck and he was burned by the hot diesel fuel. He was taken to the hospital with burns - described as non life threatening. Driver later died from what is believed to be complications from the injury.
EE's were attempting to raise the crusher discharge conveyer when a support bracket broke, causing the end of conveyer to raise upward striking the victim. The conveyer was being raised with a hoisting boom mounted to the bucket of the front end loader.
Ee had gone to face area to service face equip as part of idle shift on unit #4. It appears from acc.site that ee was moving continuous miner from face of #3 entry outby about 1 & 1/2 crosscuts to service it. A coworker said ee had been alone for about 5 min, when coworker found ee pinched between tail of miner & coal rib about 15 ft outby intersection for crosscut #29, entry #3.
AFTER FAILING TO REPORT OUT, THE VICTIM WAS DISCOVERED ON SURFACE, HAVING TRAVELED BY BELT. INVESTIGATION IS STILL ONGOING AT THIS TIME AS TO WHAT HAPPENED TO CAUSE THIS ACCIDENT.
Truck driver's lid got stuck on safety cage and when he tried to pry it free, it came loose and sprung and hit him. He fell 11 feet. This was told to me by Ralph Rodriguez.
Employee was pinned between the coal rib and the ripper head of the continuous miner as he was moving the miner from the 2 right crosscut to the 2 left crosscut by radio remote.
Apparently the victim attempted to install drill steel with the mast vertical and the drill head rotating. The victim became entangled in the rotating drill steel when the steel cross threaded in the drill collar.
Drop ball on crane fell striking victim in the head.
Cutter was fatally injured when he was struck by a falling tree.
PRELIMINARY INDICATIONS ARE THAT A PORTION OF THE STOCKPILE SLID, COVERING AN EMPLOYEE WHILE HE WAS DIRECTING A LOADER OPERATING DURING ACTIVITIES RELATED TO THE MAINTENANCE ON AN UNDERGROUND RECLAIM FEEDER. THE EE WAS COVERED FOR A SHORT PERIOD OF TIME AND WAS PRONOUNCED DECEASED AT THE HOSPITAL.
EMPLOYEE WAS WORKING ON THE 4TH FLOOR OF SCREEN TOWER. A GUARD RAIL WAS REMOVED TO LOWER A PIECEOF SCRAP TO THE GROUND. AFTER SCRAP WAS LOWERED,EMPLOYEE WAS BELIEVED TO BE THROWING ANOTHER PIE CE TO THE GROUND WHEN HE FELL FROM THE OPENING TO THE GROUND 56 FEET BELOW. FALL PROTECTION WAS PROVIDED BUT NOT USED.
Employee was driving a service truck down the Colony Bay access road. Just past CP-4, employee stated he lost his brakes and was the road clear. The truck apparently hit the bank at the padded pond and went along the pond road. The truck slid into the pond. Employee was under water about 2-3 minutes. He died 2/26/04.
EMPLOYEE WAS OPERATING A BATTERY POWERED, TRACK MOUNTED MANTRIP. UPON ENTERING THE MINE, THE WHEELS OF THE MANTRIP APPARENTLY LOCKED UP AND THE VEHICLE BECAME OUT OF CONTROL. THE MANTRIP STRUC K A METAL DOOR AND THE DOOR STRUCK THE EMPLOYEE.
DRIVER CAME OFF THE TOP OF THE TRAILER STRIKING THE GROUND
ELECTRICIAN WAS FATALLY ELECTROCUTED WHILE REPAIRING A MINER CABLE. THE CABLE WAS NOT LOCKED AND TAGGED OUT BY THE ELECTRICIAN.
THE VICTIM WAS OPERATING AN AERIAL BOOM TRUCK INSIDE THE BUCKET TO DISMANTLE A SHOP BUILDING. THE VICTIM & TWO OTHER EMPLOYEES WERE USING THE BOOM OF THE BUCKET TRUCK TO LIFT UP WOODEN 4X6 POST. THE POST WAS BURIED IN THE GROUND WITH CONCRETE. WHILE LIFTING THE POST, THE VICTIM WAS THROWN OUT OF THE BUCKET WHEN THE POST SUDDENLY RELEASED FROM THE GROUND.
