(3) MINERS WERE WORKING TO INSTALL & MOVE A NEW CONVEYOR BELT. PILAR WAS FEEDING THE BELT OFF THE ROLL TOWARD THE HEAD PULLEY. VICTIM WAS PULLING THE BELTING TOWARDS THE TAIL PULLEY WITH A 3/4"" ROPE ATTACHED TO THE LACING HOLES W/3 STRANDS OF WIRE. THE PULL ROPE DETACHED FROM THE WIRE & HE FELL BACKWARDS ONTO A CONCRETE SLAB. HE WAS 12'ABOVE THE GROUND, FALL PROTECTION NOT WORN.
