EE was making electrical repairs to the load side loop on a deenergized pole mounted solid blade disconnect switch. He had finished the repair work and was starting to come down the pole. Some how he received an electrical shock and fell down the pole.

EMPLOYEE HAD A MOTOR VEHICLE ACCIDENT. HE WAS DRIVING ON A DIRT AND GRAVEL ROAD. HIS VEHICLE OVERTURNED AND HE WAS THROWN FROM THE VEHICLE. THE VEHICLE THEN ROLLED ONTO EMPLOYEE.
THE EMPLOYEE POSITIONED A PICK-UP TRUCK APPROXIMATELY 10 FT. IN FRONT OF A 170 WABCO. HE APPARENTLY DID NOT MAKE HIMSELF KNOWN TO THE DRIVER. THE DRIVER PULLED FORWARD FATALLY INJURING THE VIC TIM.
USING A 110V AC SINGLE PHASE IMPACT WRENCH WHEN HE RECEIVED AN ELECTRICAL SHOCK.
