Employee had replaced hydraulic lines on Cat dozier. As he was dismounting the dozier he lost his balance. He attempted to break his fall by throwing out his hand, causing him to twist his wrist backward.
Employee was removing bolts in wear plates of Hazamag crusher. As a bolt was freed from the plate, it dropped out of worn plate spinning. It then caught employees fingers with the worn sharp edge of the head of the bolt.
Stitches were required to close a laceration to the upper right leg caused by a metal fragment from the track pin.
EE WAS WALKING AROUND DRILL. AS HE STEPPED WITH HIS LEFT FOOT, THE GROUND SHIFTED, CAUSING HIS RIGHT KNEE TO TWIST.
EMMPLOYEE AND 2 CO-WORKERS WERE MOVING SHEM TO BE ADDED BACK IN PLACE. WHILE IN MOTION EMPLOYEE LOST HIS BALANCE, KEEPING ONE HOLDING SHEM AND THE OTHER HAND AND FOREARM TO BREAK FALL. EMPLOYE E'S FOREARM MAKE CONTACT WITH METAL GUARDING. EMPLOYEE'S ARM BROKE THE FALL, BUT SLID DOWN GUARDING RESULTING IN FOREARM LACERATION. (7 STICHES).
EE WAS THROWING SWITCH IN 100 AMP MAIN FUSE BOX AFTER HE HAD COMPLETED THE WIRING UP OF DUST SUPPRESSION PUMP & MAGNET THAT WAS INSTALLED ABOVE CONVEYOR.
