Employee was operating 982 CAT Loader at the South Plant when EE went to get a load of concrete sand to load a customer truck EE felt pain in EE's lower back. Operator was wearing seatbelt when incident occurred.

Employee was stepping from work boat to deck of dredge when foot slipped on the wet deck and landed on EE's face. Employee received 13 stitches to lower lip, and minor cuts and scraps to the facial area. Employee released to full duty, reported due to receiving stitches.

Employee had a loader bucket on a goose neck trailer, and threw a chain and binder over the bucket to chain it down. When employee threw the chain employee felt a share pain in employee's right elbow and forearm.

Employee was reinstalling a impeller on pump and was pushing it with employee's foot to line it up so they could get it on the shaft and employee felt a popping sensation in employee's lower left back area.

THIS WAS A CUSTOMERS TRUCK THAT WAS DRIVING TOO FAST FOR CURVE AND TURNED OVER ON RIGHT SIDE. DRIVER RECEIVED LACERATION TO RIGHT EYEBROW. SPEED LIMIT SIGNS, 25 MPH, CURVE SIGNS AND ARROWS WERE ALL IN PLACE, Date of birth is unknown, SSN is unknown.

Employee was closing storage door on the A-frame of a 28 foot trailer and the steel lid slipped out of EE's hand, pinching off the soft tissue of EE's left ring finger.

Operator was shoveling sand so they could move the stacker when felt a sharp pain in lower back.

While operating a forklift unloading pipe from a truck, a spider bit the operator on the upper right arm.

Pipeline slipped fell on employees left pinky finger causing laceration and fracture to finger Received 7 stitches restricted duty with left hand

HITTING BEARING WITH PUNCH AND HAMMER, PIECE OF BEARING CHIPPED AND EMBEDDED IN HIS BELLY, SENT TO HOSPITAL TO BE REMOVED.

EE was grinding a piece of metal and a piece of metal went into left eye. Ths claim was reported due to the fact the pieces had to be removed from the eye. There was no lost time

EMPLOYEE WAS WORKING ON PIPELINE AND WIND BLEW FOREIGN ARTICLES INTO HIS LEFT EYE, PROBABLY SAND. Reportable due to Doctor removing foreign particals from eye

EE was drilling a road sign post, when the bit caught, twisting his hand and breaking the bones in right hand to the little and ring fingers. Released to light duty, transferred to light duty position until further medical evaluation.

FLIPPING A SIGN HOLDER OVER TO MOVE OUT OF WAY DROPPED ON FOOT CUTTING TOP OF FOOT.

Employee was trying to connect water hose flanges together and pipe shifted, catching his left ring finger between flanges, There was no lost time, became reportable on 01/19/2007 when the doctor put two pins in knuckle at end of finger

EMPLOYEE STEPPED OVER A PIPE LINE, FELL, TWISTING ANKLE. THIS WAS NOT AN REPORTABLE INCIDENT FOR THE ANKLE. THE EMPLOYEE LATER DEVELOPED A HERNIA PROBLEM FROM THE FALL THAT REQUIRED SURGERY ON 01/11/2007.

Employee was working in work boat when bank caved in and threw him out of boat, landing on suction pipe of dredge, injuring left knee. Restricted duty only.

Operator was opening boxes with a razor knife, when a box fell hitting his arm and jabbed the knife into right leg. Operator received stitiches only

Employee was walking on top of the V-box on the walkway and slipped and fell, landing on backside,Injuring lower back

While carring a pump shell up a hill, the sand gave way, and the pump shell fell on right ring finger. STICHES ONLY

Employee was cleaning roots out of suction, bank caved in and hit end of boat throwing operator into water. Operator stated he did not want medical treatment at time of incident. Took a weeks vacation came back and went to Doctor on 10/11/2005. Was put on light duty, but paper work was not misplaced until 10/25/05. Employee is still on light duty, reason report filed late.

Employee was moving plywood and got a splinter in his right index finger just behind the finger nail. This report is for stitches only medical.

Operator was pulling rear winch cable thru cable trough when a piece of broken wire punctured and stuck in his neck behind right ear. Operator received stitches only.

Operator was stating boat motor, put motor in reverse and it jumped, cutting operator on left elbow

Injured worker stated he twisted his knee during the performance of his duties.

UNROLLING CABLE, CABLE SPRUNG UP AND HIT HIS FOREHEAD ABOVE HIS LEFT EYE, BREAKING HIS SAFETY GLASSES.

LIFTING UP 2 PIECES OF 18" PIPE WITH LINE UP BAR AND GOT A SHARP PAIN IN BACK.

WORKING ON STARTER CABLE DIRTY WITH PIN HOLE IN IT GOT TOO CLOSE TO PIN HOLE AND GOT SHOCKED, HIT HEAD ON CONTACTOR.

WHILE UNLOADING EMPTY DRUMS, GETTING OFF TRAILERHE STEPPED DOWN AND TWISTED HIS LEFT ANKLE.

TAKING WORK BOAT GOING TO DREDGE, HE GOT BETWEEN PONTOON PUMP AND A-FRAME OF A SUCTION WINCH. HE WENT TO GET OFF BOAT, FELL IN MIDDLE OF A-FRAME UNDER WATER, HIT HIS RIBS, PULLED HIMSELF UP & GOT ON DREDGE AND CALLED FOR HELP.

WHILE CLEANING BACK OF SCALE HOUSE, SHE SLIPPED AND FELL WHILE PULLING ON THE HOSE. WATER HAD BEEN STANDING ON CONCRETE WITH ALGAE ON IT.

LOADING A TRAILER, HE MOVED OUT OF THE WAY OF A PIPE IN THE AIR AND TRIPPED OVER A PIECE OF PIPE ON THE GROUND.

HOOKED TRAILER UP & PUT BINDERS IN BOX COMING OFF OF LOWBOY GOT FOOT HUNG UP IN AIR BRAKE HOSE.

WAS GETTING OFF OF LOADER MISSED TOP STEP, GRABBED HAND RAIL TO KEEP FROM FALLING - PULLED STOMACH

PUTTINGON SEAL PUMP WHILE TOTING DOWN THE HILL TRYING TO STOP SELF KNEE STARTED HURTING
