Employee was stepping from work boat to deck of dredge when foot slipped on the wet deck and landed on EE's face. Employee received 13 stitches to lower lip, and minor cuts and scraps to the facial area. Employee released to full duty, reported due to receiving stitches.

Employee was reinstalling a impeller on pump and was pushing it with employee's foot to line it up so they could get it on the shaft and employee felt a popping sensation in employee's lower left back area.
THIS WAS A CUSTOMERS TRUCK THAT WAS DRIVING TOO FAST FOR CURVE AND TURNED OVER ON RIGHT SIDE. DRIVER RECEIVED LACERATION TO RIGHT EYEBROW. SPEED LIMIT SIGNS, 25 MPH, CURVE SIGNS AND ARROWS WERE ALL IN PLACE, Date of birth is unknown, SSN is unknown.

Employee was closing storage door on the A-frame of a 28 foot trailer and the steel lid slipped out of EE's hand, pinching off the soft tissue of EE's left ring finger.
EE was grinding a piece of metal and a piece of metal went into left eye. Ths claim was reported due to the fact the pieces had to be removed from the eye. There was no lost time
