Employee was climbing caged ladder at conveyor 18. EE was on the last rung when EE heard a pop sound and pain on right bicep.
Employee stated that was working with a driller to put the hose for the sump pump into the lake, when EE felt a sprain/strain type feeling in their back. When positioning the hose, body position can become awkward requiring twisting.

EE removed a lifting hook from an autocrine and placed it on a round shaft. A few minutes later the EE slipped in the mud and hard hat fell off. EE bent over under the shaft to retrieve hard hat. Placed a hand on the shaft to balance themselves and knocked the lifting hook off. The hook fell onto the EE's head. EE received stitches. EE returned to work.
An employee was returning from staking out the limits in area when they tripped and fell over a rock. Employee fell face forward and injured chin and lip requiring stitches.

The incident happened while trying to weld a flatbar, 6" by 36", to the inside of the hopper. While placing the flatbar, right before lowering his welding hood to start welding, some material gave away under his feet. He slipped backwards and struck himself with the flatbar right above his right eye causing a cut on his right eyebrow.
While driving a manlift employee hit a small ditch bounced lost balance and his right foot slept out of the basket impacting the toe guardsand cutting his calf

A 64 year old Miner with over 26 years experience was doing a hoist resocket on a dragline. As he raised the sledge hammer to knock out the wedge from the socket he felt a popping sensation in his right bicep. The employee visited several doctors getting a second opinion and missed his first day of work on 11-5-2012

Cut back of hand with utility knife while cutting old conveyor belt.
After changing a phase monitor on a electrical cabinet and engergizing to adjust voltage, an arc flash occurred and injured his hands.

During the process of replacing a backstop on conveyor 3, both employee & co-worker began to prep the motor. EE had not placed his safety harness on while on the catwalk. His co-worker loosened the bolt & took the nut off, at that instance the bolt shot out, the reducer rotated forward and struck EE throwing him off the catwalk 10 ft into the chute.
Was working on pattern at WU. While tying up pattern, I twisted my left knee. At the time it seemed very minimal. Did not go to the Doctor for several days. Diagnosis: Prepatellar Bursitis
Employee was walking toward the plant at the beginning of the shift, approaching electric room. He slipped on some loose rock and landed on his left knee.
Employee was attempting to climb up his loader to start the shift when his lunch bag strap fell off his shoulder. In his attempt to reach for it before it fell he let go of his left hand to catch it. He then lost his balance and twisted his shoulder.

Employee was moving a portable pressure cleaner from the wash rack area for repair and while taking it out, it tilted back, he tried to catch it, causing cuts to 3 of his left fingers from a small caution sign mounted on the equipment.
Employee was opening rail car doors at the loadout area to washout the rail cars and as he was opening the door with a 5 foot bar he slipped and fell backwards. He felt a pain in his left knee. (Employee quit that same day and did not seek treatment for the injury until August 4th, 2005 at which time he was taken to workers comp.)
Employee was pulling a water hose to wash down the area, when the hose twisted in his hands, catching the employees right thumb, with the hose clamp, causing a cut on his thumb. (The employee did not seek medical treatment until the 26th of July, at which time he was taken to workers Compensation)
Employee was attempting to move an old railroad tie from a stack of old used ones. He got his right hand caught between a rail tie and a section of old rail, when the railroad tie turned in his hands.
Employee was washing the concrete floor of the primary crusher area, when he slipped on the wet concrete and fell to the floor, injuring his knee.
Employee was climbing a portable work ladder to work on a 992 loader and the weld at the bottom side of the ladder broke, causing the employee to fall with the collapsed ladder, landing on his knees.
EE was changing a Canica crusher feed plate liner, when the liner slipped down and struck the EE in the inside of the left shin.

EE FOOT SLIPPED ON LADDER OF DRAGLINE AND HE FELL AGAINST CORNER OF OPEN COMPARTMENT DOOR NEXT TO LADDER, CUTTING HIS RIGHT THIGH. REQUIRED ONLY FIRST AID. A FEW WEEKS LATER HIS LEFT LEG BEGAN TO HURT CAUSING HIM TO SEEK MEDICAL TREATMENT. DR TOLD HIM IT WAS RELATED TO THE FALL.
EE WAS LIFTING AN DINSTALLING HEAVY CROSS MEMEBER TUBE IN SCREEN UNIT WITH HELP FROM ANOTHER EMPLOYEE. AS THEY WEE PUTTING THE TUBE IN PLACE TAPANES SLIPPED ON BOTTOM DECK SCREEN WHICH HAD SO ME 3/8 STONE ON IT AND THEN FELT PAIN IN CENTE R OF LOWER BACK.
EMPLOYEE WAS USING A WIRE BRUSH TO REACH UP & OPEN THE DOOR OF A WELDER TO START THE MACHINE THE BRUSH SLIPPED OUT OF HIS HAND & FELL STRIKING HIS LEFT EYE WITH THE HANDLE OF THE BRUSH

EMPLOYEE WAS CLIMBING DOWN LADDER FORM DRAGLINE WHEN EH STEPPED ON LOOSE AND UNEVEN GROUND BELOW THE LADDER AND TWISTED HIS ANKLE, CAUSING A SPRAIN.
EMPLOYEE WAS USING A HOLE SAW IN A DRILL PRESS. HE LEFT THE CHUCK KEY IN THE CHUCK. WHEN THE DRILL STARTED, THE KEY KNOCKED HIS THUMB INTO TH E SAW, CAUSING INJURY.
EMPMLOYEE WAS STANDING ON NARROW PLATFORM AND HOLDING ON TO A HANDHOLD WHILE CLEANING THE FRONT WINDOW OF THE DRAGLINE. HE LOST HIS BALANCE AND FELL SEVEREAL FEET TO THE GROUND. HE LANDED O N HIS FEET THEN FLAT ON HIS BACK. SAFETY BELT WAS NEEDED BUT WAS NOT USED.

WHILE USING PNEUMATIC IMPACT WRENCH, RETAINING PIN HOLDING SOCKET TO WRENCH SLIPPED PAST HOLDING RING, LACERATING RIGHT HAND PALM AND RIGHT MIDDLE FINGER.
EMPLOYEE WAS PROJECTED FORWARD INTO THE WINDSHIELD OF HIS GRADER WHEN THE GRADER BLADE CAUGHT ON A RAIL JOINT, WHICH STOPPED THE MACHINE ABRUPTLY. HE WAS OPERATING IN A STANDING POSITION W/O A RESTRAINING DEVICE. A STANDING HARNESS WAS SUBSEQUENTLY INSTALLED.
EE WAS HOLDING A STRIP OF METAL WHILE ANOTHER EMPLOYEE DRILLED A HOLE IN IT. WHEN EE APPLIED TO MUCH PRESSURE, THE 1/8" BIT BROKE AND THE DRILL SLIPPED TO ONE SIDE, WHERE THE BROKEN BIT PENETR ATED HIS LEFT FIFTH FINGER.
I.NAME HAD A SEIZURE RELATED TO AN OLD HEAD INJURY WHICH CAUSED LOSS OF CONTROL OF TRUCK. DROVE TRUCK STILL LOADED DOWN THE RAMP FROM THE CRUSHER. DROVE INTO OPPOSING TRAFFIC LANE ON A CURVE A ND COLLIDED WITH A EUCLID HAUL TRUCK DRIVEN BY OTHER EMPL.
