Teammate attempting to cut board on a benchtop in the maintenance shop. Using angle grinder, teammate worked on a section which was not properly secured. Unsecured section shifted caused mechanic to lose control of board. Holding the grinder in EE's right hand, reached out left hand accidentally made contact with the rotating wheel, resulting in lacerations to two fingers.

Mechanic was completing housekeeping in maintenance shop. During the clean up under work bench, the employee had two magnet bars that connected pinching EE's right thumb. The employee received medical treatment.

Employee was lifting core samples when they felt a pop rupturing the bicep tendon.

Employee attempting to catch product bag from falling off of a line experienced discomfort in shoulder.

Employee pinched finger in swing gate requiring sutures.

Packaging employee loading fiber drums on #7 bagger. After completing load into a 95 lbs. container, employee began rolling off drum for palletizing. While placing onto storage pallet, employee noted holding onto the top of drum and continued gripping (drum) while attempting to return to scale position. Employee stated they felt a pull in left shoulder.

Verifying potential blockage at bottom of drag box, employee completed steps to clear blockage by normal methods. Removed flexible hose connected on top of transfer pump. While pulling hose off the connection point between a camlock fitting top of flange, connection failed. Employee received thermal burn to forearms from non-hazardous product.

Employee stated that EE was rocking back and forth on shoe heels that snagged EE's pants causing the fall.

Employee lost grip on tank trailer ladder and fell from 2nd rung of ladder. Employee has a rib fracture and was placed on restrictive duty for 14 days but is working in a different capacity for those days.

Co-worker was operating Genie manlift inside tank in front and above door sheet working on upper shell vert weld above scaffold. When booming lift up, we think bottom of lift or weld lead caught hand line or scaffold bracket, pulling bracket out of bracket clip. Bracket fell and struck EE as EE was walking into tank through door sheet.

Using a hammer to bend a bracket strap to fit contour of tank. Metal strap 1/4"x 2"x 14". Strap was placed at a 45 degree angle and hit with a 4 lb. hammer in the center of the strap, bending strap slightly. When hitting strap with hammer, hammer glanced off bracket strap twisting wrist at impact spraining right wrist.

Maintenance employee tweaked their back. Employee was standing on the cat walk in front of T/C blunger feeder assisting contract worker to remove cross auger guard. After leaning over to hand a portable battery powered impact wrench to contract worker, employee felt a pain in the lower back.

Maintenance employee slipped and fell as EE stepped from the concrete pad down approximately two inches to the ground in our North tank farm.

Maintenance employee was holding a piece of flat bar being cut by coworker when employee felt something pass behind the safety glasses and into eye. Employee when to local ER and had a small piece of metal removed that was imbedded into the left eye.

Employee noticed pain in arm and thumb was swollen and painful. Reported issue and doctor diagnosed lateral epicondylitis of right elbow from improper use of mouse; Thumb had medical issue so employee was referred to personal doctor for it. EHS provided wireless mouse and showed correct use without moving whole arm. Employee was given meds to treat pain and tenderness in arm.

EE fell from extension ladder striking upper neck area on a 4" protruding vertical pipe.

Employee using chemical (Naphta) for routine cleaning of lab equipment, complained of headache, disorientation, and nausea.

Employee was transporting 55gal drums of crude kaolin from Wrens to Macon. While on route one of the drums tipped over on the trailer. Employee stop and attempted to stand drum up on trailer, when he felt a sharp pain in his left arm and shoulder. Injury became a MSHA reportable after employee saw doctor on July 9th.

Employee was moving HVAC unit into place by sliding across floor. The unit came to a stop and the employees hand slipped and was cut by a piece of metal. The cut was where his thumb meets his hand and he received 6 stitches.

Two workers were installing the drive belts on an Elliott Pulverizer Mill. One was guiding the belts onto the sheave while the other worker turned the rotor by hand. The worker turning the rotor by hand turned his head to speak to the other worker. During this moment of inattention his right index fingertip was caught between the blades of the rotor and the machine housing.

Employee was standing on portable steps working on #5 Magnet close loop pump. When employee climbed down and reached last step to ground level, he felt pain radiating inside his right knee.

While stacking 50lb bags employee suffered a muscle strain to his lower chest wall.

Operator experienced a strain to the lower lumbar region while moving a pallet.

Employee was helping operator set up the drill rig to begin drilling. As the operator was lowering the kelly into the turntable the employee allowed his right index finger to be pinched between the kelly block and the turntable. Weather conditions were sunny, dry, and warm.

Employee was lifting 50 lbs bags and strained his left shoulder. Employee did not notify his supervisor until 5/22/09.

On January 23, 2009 at 9:30 pm, a plant employee experiened a slip, trip, and fall, that was duly reported to his supervisor and first aid was applied. It was not until later when a doctor's office visit was requested, (2/20/09) that it was learned that the employee suffered a fracture to his left radial head. (Forearm) The injury did not and will not result in lost time.

An operator cut his right ear on a pneumatic fitting while trying to remove a busted bag from a conveyor. Operator was moving from the backside of the bagger between the bagger spouts to remove the busted bag. Operator did not shut the conveyor off to remove the bag, therefore he was moving through the area quickly in an attempt to remove the busted bag from the conveyor.

Employee rested left hand on bagger frame crossarm. When hook ctrl arms were raised, his index finger was pinched between the crossarm and the metal part of the lifting arm. Employee works for a temporary agency at our facility. Since the temp agency does not have an ID#, I am reporting for them.

On June 7, 2006 employee reported to human resources that he thought he might have carpel tunnel in his right wrist.

Employee was coming into work when he slipped and fell on some ice outside his work area.

Disconnecting union on a 2" water line that was connected to a 4" water line that was being supported by the 2"line. When the 2"line came apart,the pipe dropped causing the pipe hangers on the 4"line to break allowing the line to come down and rest on top of employees head. during a follow up visit on 2/10/06, it was determined by the dr that dr therapy was needed at this time.

An employee operating a forklift backed into a another forklift that was parked and loaded with empty pallets. The impact caused the parked forklift to move forward and strike an employee who was walking between the parked forklift and a guardrail. The employee was caught between the pallets on the forklift and the guardrail and suffered bruises to his ribs and back.

A Bagger Operator was descending the steps from a platform when he felt dizzy. Employee fell backwards on floor and bruised left shoulder. Defective stairs were in use that likely contributed to the fall. Employee complained of pain in his shoulder on the following evening and was provided medical attention.

Employee was bagging product on bagger machine when a bag burst and spewed kaolin clay upward onto employee. The force of the material covered the employee's upper body resulting in some of the clay getting into his left eye. First aid was provided, but the eye became irritated the next day & medical treatment was needed.

EE EXPERIENCED NUMBNESS IN HER RIGHT HAND AND EXPRESSED CONCERN THAT IT MIGHT BE WORK RELATED. EE ENTERS DATA INTO A COMPUTER AND TYPICALLY WORKS LONG HOURS WITH INFREQUENT BREAKS. REVIEW OF THE WORK STATION REVEALED THAT IT WAS NOT ERGONOMICALLY SOUND.

EE WAS STANDING AT A FIRST AID CABINET GETTING A BAND AID. ANOTHER EE WAS BACKING HIS FORKLIFT TO GET IN POSITION TO PICK UP A LOAD OF BAGGED PRODUCT. HE PROCEEDED TO BACK UP WHILE THE GATE OF A PROPANE TANK HOLDER WAS OPEN (ABOUT 2 1/2 FT IN LENGTH). THE GATE STRUCK THE EE IN THE BACK CAUSING A DEEP BRUISE.

EE WAS HANDLING A 50 LB BAG OF PRODUCT AT NO. 2 BAGGER IN THE PLANT WAREHOUSE. AS EE WAS POSITIONING THE BAG ON A PLATFORM SHE FELT A SHARP PAININ HER RIGHT SHOULDER, ARM AND CHEST. FIRST AID WAS PROVIDED BY PLANT PERSONNEL. MEDICAL ATTENTION WAS REQUIRED ON 2-19-03

EE WAS WORKING IN ANALYSIS LAB. HE WAS TOWEL DRYING A 100 ML-GRADUATED CYLINDER (GLASS) FOR WEIGHING CLAY SOLIDS. THE CYLINDER BROKE IN EE'S HAND, CUTTING HIM ON HIS LT. HAND BETWEEN HIS INDEX FINGER AND THUMB. HE RECEIVED 6 STITCHES TO CLOSE THE WOUND.

EMPLOYEE WAS OPERATING A FORKLIFT IN THE CALCINE WAREHOUSE. AS HE TRAVELED DOWN THE AISLE NEAR THE BACK LOADING DOCK, A PIECE OF DEBRIS BLEW INTO HIS LEFT EYE. HE WAS WEARING SAFETY GLASSES. EMPLOYEE WAS TAKEN TO THE DOCTOR AND HIS EYE WASIRRITATED. HE WAS GIVEN AN ANTIBIOTIC PRESCRIPTION.

EE WAS WORKING IN THE BAGGER WAREHOUSE. HE WAS PREPARING TO LOAD BAGGED PRODUCT INTO A BOXCAR AND PROCEEDED TO MOUNT A FORKLIFT. AS HE WAS POSITIONING HIMSELF IN TEH SEAT OF THE FORTRUCK, HE F ELT A SHARP PAIN INHIS LEFT HIP AND LEG. EE RECEIVED MEDICAL TREATMENT AND WAS ORDERED TO HAVE RESTRICTED WORK ACTIVITY FOR 3 DAYS.

EE WAS REPAIRING A PUMP AND CHEMICAL FEED LINE FROM A CAUSTIC SODA CHEMICAL TANK. WHILE WORKING ON HIS KNEES TO REINSTALL THE PUMP, HIS ACID SUIT DEVELOPED A HOLE AND ALLOWED THE DILUTED CAUST IC TO BE ABSORBED ON HIS CLOTHES. HE EXPERIENCED A BURNING SENSATION TO HIS RIGHT KNEE AND REMOVED HIS SUIT. HE RECEIVED MEDICAL ATTENTION FOR 2NE & 3RD DEGREE BURNS TO THE KNEE.

EE MISSTEPPED WHILE EXITING PERSONNEL CONFERENCE ROOM, STUMBLED AND FELL ONTO GRATING. EE EXPERIENCED PAIN AROUND RIGHT KNEE AREA, THEN NOTICED A CUT BELOW THE KNEE. (LACERATION REQUIRED 7 SUT URES).

EE WENT TO RESET AIR CONDITION UNIT. WHEN HE WAS COMING OUT OF THE AREA OF DUCTWORK THAT HE HAD TO GO THROUGH HE BUMPED HIS HEAD ON A PIECE OF THE DUCTWORK AND IT LACERATED HIS SKIN. HE HAD 11 SUTURES.

EMPLOYEE WAS DOING HOUSEKEEPING IN AREA OF #7 BAGGER. HE FELT PAIN IN HIS LOWER BACK AS HE STEPPED UP ONTO FORKLIFT AND SAT DOWN. THERE IS NO STEP ON FORKLIFT AND SEAT CUSHION IS DETERIORATED AND WORN. THE HIGH STEP AND HARD SEAT MAY HAVE CONTRIBUTED TO EMPLOYEE'S BACK STRAIN.

EE WAS PREPARING STOCK AT #5 BAGGER. HE PLACED THE PALLET ON THE STRETCH WRAPPER TO BE WRAPPED. HE NOTICED THAT FILM WAS NOT WRAPPING PROPERLY, SO HE REACHED OVER TO ADJUST FILM. HANDRAIL GAVE AND HE WAS CAUGHT BETWEEN SAFETY HANDRAIL AND CONVEYOR. LACERATION TO LEFT THIGH - 8 SUTURES.
