Kenworth tractor trailer truck was in the shop being repaired. The mechanic was letting the hood down, she did not know the driver was trying to get a shop light from the top of the front tire. The mechanic eased the hood over striking the driver causing bruising of the upper body area.
Foreman was cleaning up the cut with a Cat D6M dozer. He went over a small hump of dirt, his cell phone engaged the parking brake on the left arm rest causing the dozer the stop suddenly. The foreman had his seatbelt on, the stop caused his neck to snap forward. He was stunned for a few minutes, then drove himself to the doctor.

While lowering drill pipe to connect tool joint, helper put left hand under drill pipe being lowered to remove dirt. Middle finger was jammed between drill pipe causing laceration of finger. Helper was wearing gloves.
EE WAS REPLACING THE HOIST FRICTION BAND ON LARGE DRAGLINE #17. THE TASK REUIRES THAT THIS DEVICE BE HOISTED OVERHEAD AND MOVED TO OUTSIDE AREA. DURING THIS PROCESS, THE DEVICE FELL DUE TO INADEQUATE RIGGING. IT DEFLECTED OFF THE MAIN GEAR BOX AND STRUCK EE CAUSING A SPRAINED WRIST AND BRUISED RIBS

EE, #17 DRAGLINE OILER WAS WALKING DOWN THE DRAGLINE LEVER - STAND STEPS WHEN HE TWISTED HIS LEFT ANKLE ON THE LAST STEP. HE STATED THAT ONLY HIS LEFT HEEL MADE CONTACT WITH THE LAST STEP, CAU SING HIM TO TWIST AND SPRAIN HIS LEFT ANKLE.

AFTER REMOVING AND LIFTING TH EPUMP FLANGE WITH A NYLONG STRAP, IT WAS LOWERED ONTO THE CONCRETE AREA. HE DISCONNECTED THE NYLON STRAP AND WAS LOWERING THIS FLANGE ONTO THE FLOOR. HE LOST HIS FOOTING AND FELL. HIS MIDDLE FINGER (LEFT HAND)
BECAUSE OF THE BACK/NECK INJURY EE SUFFERED ON MAY 24 1989 REMOVING A CANNISTER FROM THE PILOT PLANTS MAGNETIC SEPARATOR HE CONTINUES TO HAVE TROUBLE WITH HIS SHOULDER HE IS HAVING SURGERY TO CORRECT THE PROBLEM HE IS HAVING WITH HIS SHOULDER CNA HAS NOW DETERMINED THAT THIS WAS A RESULT OF HIS 1989 INJURY

BECAUSE OF THE BACK/NECK INJURY WILIE JONES SUFFERED ON MAY 24, 1989 REMOVING A CANNISE FROM THE PILOT PLANT'S MAGNETIC SEPARATOR, A DRAIN TUBE HAD TO BE IMPLANTED IN HIS NECK. THIS TUBE HAD TO BE REPOSITIONED IN 1992. WILL HAD TO HAVE A ROTATOR CUFF REPAIRED IN HIS SHOULDER IN MARCH OF THIS YEAR. IT HAS NO BEEN DETERMINED BY CNA/INS. THAT THIS WAS A RESULT OF HIS 1989 INJURY.

AS HE BEGAN TO PU LL HIS RIGHT LEG UPWARD TO STEP ONTO THE SECOND STEP, HIS RIGHT FOOT (ARTIFICIAL LIMB) HIT BENEATH THE FIRST STEP CAUSING THE UPWARD BODY FORCE TO TWIST AND STRAIN HIS RIGHT KNEE.
WHILE REMOVING PILLOW BLOCK BEARING FROM IDLER SHAFT ON DRAG BOX USING A HAMMER AND COLD ROLL PUNCH, APIECE OF THE BEARING CAME OFF HITTING INJURED IN THE LEFT CHEEK.FOREIGN BODY REMOVED -SUTU RES REQUIRED.
WHILE INSTALLING TOOTH ADAPTERS ONTO A 13 YD DRAGLINE BUCKET IN THE SHOP,THE ADAPTER SLIPPED AND CAUGHT EMPLOYEES FINGER ON RIGHT AHND BETWEEN ADAPTER AND CONCRETE FLOOR.INJURED WENT TO DOCTOR ON 05-14-92 AFTER HIS FINGER BECAME INFECTED.HEHAS BEEN ADMITTED TO THE MEDCIAL CENTER OF CENTRAL GA.

HE TRIPPED AND FELL HEAD FI RST THROUGH THE3CROSSOVER, ABOUT 3 FEET TO THE FLOOR OF THE DRAGLINE. HE SCRAPED AND BRUSIED BOTH KNEES AND HIS RIGHT THUMB.
HE WAS OPERATIN1 SCRAPER AND AS HE WAS COMING OFF THE DUMP AND HIT A ROUGH SPOT THAT CAUSED HIM TO BOUNCE IN THE SCRAPER SEAT CAUSING HIM TO BRUISE TENDONS LIGAMENTS AND MUSCLES IN HIS BACK.

THE BOARD IS LOCATED BETWEEN THE FRONT OF T HE MACHINERY FR AME & THE CATWALK OVER THE DIRT TROUGH IN CLOSE QUARTERS. HE WAS IN AN AWKWARD POSITION BECAUSE OF THE OBSTRUCTIONS & WHEN HE LIFTED THE BOARD IT CAUGHT AGAINST THE SIDE OF THE DIRT TROUGH.
