Jetter hose had a weak area that was not recognized in initial shift inspection. Upon use, the hose burst injuring operator on the left hand. Was treated at local medical center for hand injuries and referred to secondary care at burn center in Augusta. Care was advised to apply ointments and wrap. Return for follow-up in two weeks.

Employee unsure of when incident happened. Stated EE woke up on the 7th with eye irr/swelling. Employee was working on roof during windy conditions with grinding being performed by others. Employee was wearing safety glasses while working. Optometrist removed small metal fragment from employees eye and prescribed antibiotic eye drops. No restrictions were given by physician.

Employee opened recycle valve on clay tank and and sample valve dislodged. Hot clay sprayed employee causing burns to lower back.

While inspecting equipment after a power failure employee was looking up at the center shaft of a calcine and tripped over the cooling water drain line for the hotboxes. EE fell backwards and struck EE's elbow on the guardrail beneath the bottom of the calcine.

EE was hanging and stenciling big bags. EE had just hung a bag and was turning to their right to get the stenciling equipment and the toe of their boot got caught in a gap beneath the bagger. As EE continued turning to their right their left knee twisted in the direction they were turning while their left foot stayed in the forward position. This resulted in twisting their knee.

Worker received a piece of foreign matter in right eye while cutting a piece of metal with demolition saw. Worker had safety glasses and face shield on at time of accident. Demolition saw had all guards in place.

Employee was opening a boxcar and strained his arm while pulling to try and get it open. Employee went to the emergency room but xrays were negative. Doctor told him it was a strain and put him on 3 days of light duty and asked him to ice arm down and take pain medicine as needed. After 3 days, the doctor put him on two more weeks of light duty.

He had one hand on handrail and the other hand had tools in it. When he reached the bottom step his ankle gave way. He used the handrail to lower his body to the ground. His supervisor then helped him to the breakroom and called safety to look and report. She asked him did he need medical treatment and he replied no. He was driven home and later seen at the local ER.
At approximately 4:30 pm, a maintenance employee was stepping onto a piece of pipe that was approximately 2 ft off the ground and inadvertently fell onto the concrete flooring, hitting his head.

WHILE GATHERING UP TOOLS & EQUIPMENT AT END OF SHIFT, EMPLOYEE WAS ROLLING UP HOSE & HAD BARE HAND ON END OF SPRAY WAND WHEN HE ACCIDENTALLY BUMPED THE WAND TRIGGER - PRESSURIZED WATER CAUSED PUNCTURE WOUND TO LEFT HAND.

EE WAS INSTALLING STAINLESS STEEL BANDING ON PIPE. EE WAS GOING TO CUT OFF EXCESS WITH SIDE CUTTERS WHEN THE SIDE CUTTERS SLIPPED, CAUSING EE TO STRIKE HIS RIGHT FOREARM ON THE STAINLESS STEEL BAND - 1 1/2" LACERATION TO THE RIGHT FOREARM.
TRUCK DRIVER WAS UNLOADING MATERIAL, BACKING DOWN RAMP, WHEN GOT TO BOTTOM TRIPPED AND FELL OVER MATERIAL ALREADY UNLOADED AND LAYING ON THE GROUND CAUSING HIM TO STRIKE HIS LEFT.

OPERATOR WAS REMOVING A WATER HOSE THAT WAS WEDGED IN PLACE ON A DRAG BOX TO FLUSH SCREEN DRAIN. WHEN THE OPERATOR WENT TO REMOVE THE HOSE, THE CHAIN JUMPED AND THE DRAG CAUGHT A HOLD OF OPERA TOR'S GLOVE AND PULLED THE OPERATOR'S HAND BETWEEN THE DRAG AND THE SIDE WALL OF THE DRAG-BOX, CRUSHING AND BREAKING TWO FINGERS ON HIS LEFT HAND.

CLAY BUILT UP IN CHUTE OF SCREW CONVEYOR & WHILE CLEANING OUT CHUTE WITH A T-BAR TOOL. EE HIT HAND ONFLIGHT OF SCREW CAUSING LACERATIONS AND BROKEN BONES TO INDEX AND MIDDLE FINGER ON LEFT HAN D. SCREW CONVEYOR WAS LOCKED AND TAGGED OUT.
EE COULD NOT SINGLE OUT A SPECIFIC INCIDENT THAT OCCURRED BUT FELT PAIN AFTER RIDING ON THE LOADER. EE HAD PRIOR BACK PROBLEMS IN 1999 & FELT THAT SOME BOUNCING ON THE LOADER MAY HAVE AGITATED HIS BACK. EE CAME IN TO WORK AT 3:00 & WORKED THROUGH ENTIRE SHIFT UNTIL 11:00 PM.

CHEMICAL BURN ON RIGHT FOREARM. INTERIOR OF PROTECTIVE CLOTHING WAS INADVERTANTLY CONTAMINATED BY CHEMICALS WHICH HAD BEEN EXPOSED TO RUBBER GLOVES. EE PLACED PROTECTIVE CLOTHING ONTO SELF WHI LE WEARING CONTAMINATED GLOVES. WHILE PLACING THE CLOTHING ON, THE INTERIOR OF CLOTHING WAS CONTAMINATED BY THE GLOVES THUS TRANSFERRING THE CHEMICAL TO FOREARM.
EMPLOYEE ARRIVED AT WORK, GOT OUT OF VEHICLE AND WHILE WALKING TO WORK AREA, WALKED IN SLIPPERY SURFACE ( ALGAE BUILD UP) FALLING AND BREAKING RIGHT LEG.

EMPLOYEE PUT HIS LEFT FOOT ON TUBE PALLET IN ORDER TO SQUARE UP BIG BAG WHICH WAS ABOUT 1/4 FULL OF CLAY. WHEN HE TURNED TO WALK AWAY, HE TWISTED HIS LEFT KNEE. THIS LEFT KNEE HAS A PIN IN IT. THIS IS AN OLD INJURY.
EE WAS WALKING UP A SET OF STEPS AND TRIPPED AND AS HE WAS FALLING, HIS LITTLE FINGER CAUGHT IN THE CATWALK BREAKING HIS LITTLE FINGER ON HIS RIGHT HAND.
EE PULLED GOLF CART INTO BLIND SPOT OF A 980 FRONT END LOADER AND THE LOADER STARTED IN REVERSE TO TURN AROUND. THE LOADER REAR TIRE STRUCK THE GOLF CART WHICH PUSHED THE STEERING COLUMN INTO THE EMPLOYEE'S RIGHT LEG CAUSING A FRACTURE.
