1604 Quarry Metal/Non-Metal

On 11/18/24, an employee was walking through the sand plant when employee slipped and hit left knee on the base of a support beam. Employee sought medical attention and was issued restrictions.

EE was reinstalling the AC belt on a pickup truck. In order to place the belt on, EE was using a wedge tool to guide the belt back on. EE was leaning on the core body/radiator and as EE went to move away from the hood, felt a pop in left rib cage. EE sought medical attention the next day and was diagnosed with sprained left rib.

1st EE & 2nd EE were welding/grinding single sand screw to mend it. 1st EE was under sand screw to weld when 2 EE began grinding causing sparks & debris to fly towards 1st EE. 1st EE removed self until 2nd EE finished. 1st EE was in full PPE; face shield safety glasses gloves hard hat & fall protection. EE reported it on 2/19. Diagnosed with conjunctivitis & prescribed eye drops.

EE was tasked with replacing troughing idler cans on a conveyor (C-12). They were using a metal prybar to lift the belt when they felt a pull in their back. This resulted in a lower back strain.

Pit loader operator in a 992K loader, drove parallel to the muck pile. A large boulder slid down the pile and struck the right front loader tire. The tire blew out and during the movement the employee's right hand was injured by an unknown object in the cab. Employee suffered from a laceration to the right thumb and a strain to the right thumb / palm area.

EE was going to start replacing ball valves, as EE stepped down a set of stairs, EE's right foot slid forward causing the knee to overextend. EE fell to the ground. EE sustained a right knee sprain.

EE was cleaning material off the concrete around belt #2 and when the task was completed EE slipped while walking to a different area.

EE1 was holding a hammer against the keeper. EE2 repeatedly struck hammer EE1 was holding. The hammer EE2 was striking against hammer EE1 was holding; slipped and made contact with EE1 left pinky finger smashing it.

Employee was using a 2 inch water hose to clean spillage from a cement slab. EE dropped the hose in full open position and when EE bent down to pick it up EE felt a pain in EE's right shoulder. EE sought medical attention and is currently on restricted duty.

Employee was preparing a sling to lift a 16 inch pipe with a backhoe when the sling tension tightened before he was able to remove his hand, pinching his thumb. He sought medical attention and is currently losing time.

Employee was working as a groundsman, performing house keeping duties, under the primary crusher when he experienced symptoms of heat exhaustion. Another employee assisted him to the office to cool down. He sought medical attention at the local clinic, diagnosed with heat illness and was released to full duty.

2 of 3 utility owned power lines came loose from pole and fell to ground. The lines continued arcing for over 30 minutes, preventing the fire department from extinguishing the small brush fire. The Utility Repair Crew replaced the lines and system parts up to the (utility owned) pole mounted transformers. No Injuries, or damage.

While ascending the 990 loader, the employee slipped on a loose cover plate and fell to the same level twisting his ankle.

While standing in a Cat 990 series II counter weight storage box to replace the right rear ladder, the employee's right foot slipped causing a strain to the right knee. Went to Dr. on 04/08/2013. Restricted duty.

The employee tripped on an air hose and fell to the second step/platform 17 inches below. He fractured his left arm which requires surgery that is scheduled for 12-13-12.

An employee was pulling an air hose to the screen tower to work on the screen deck. There were two hoses coupled by a quick connect, the quick connect hit a section of the hand rail, it became uncoupled at this time. The hose whipped upwards into his face causing a small laceration to his right eyebrow.

Employee was using a pry bar to loosen a broken liner in the 130/150 universal impact crusher. When the liner was hit with a sledge hammer the bar slipped and cut him on his right side cheek. The employee needed stitches.

EE was performing maintenance on 82 ton Linkbelt Crane. EE completed job on crane, was getting off truck and stepped onto 4' ladder he was using to access truck deck and slipped and fell to the ground. He fractured three bones in his left ankle. Ladder was still in place when he looked up from his fall. Surgery on ankle to repair fractures on 2/7/08 @ 21:30 hours.

Two employees were in the process of moving a large bearing (approximately 100 pounds) from a work bench to the shop floor. Injured lost his grip and the bearing landed on the top of his foot. He was diagnosed with a few broken bones in his foot as a result of this accident.

Loading sticks in the sun all day, 97-98 degree's outside, went to go to the truck and legs started to cramp up, then the entire body locked up, fell to the ground, sent to the hospital by ambulance - body was dehydrated.

Walking to the 2nd shot, feet got caught in the dried mud tracks, lost balance, fell; put right arm out to break the fall, landed with right wrist against ribs. Bruised ribs.

Employee was descending the stairs of the Cat 988G loader. He slipped on the third step causing him to fall into the steel louvered engine guard. The exposed edge of the guard cut his arm near the left elbow.

Unhooking safety chain from a butterfly valve on a large diameter water line. When the chain was unhooked, the flowing water caused the valve handle to swing around hitting the employee in the chin.

EMPLOYEE WAS WASHING DOWN THE TRUCK SCALE AT THE 1604 BASE PLANT USING THE WATER HOSE FROM THE PLANTS WATER TRUCK WHEN THE EMPLOYEE FELT SOMETHING HIT HIS ARM, WHICH WAS A STRAY BULLET THAT CA ME FROM SOMEWHERE FROM THE PROPERTY BOUNDARY.

WHILE ASSISTING ANOTHER MECHANIC, THE OTHER MECHANIC HIT THE EXPANDED METAL CAGE ON A SKID STEER LOADER IN AN ATTEMPT TO KNOCK OUT AN INDENTATION ON THE HARD HAT. THE HARD HAT PUSHED ON HIS SA FETY GLASSES CAUSING A LACERATION ON HIS FOREHEAD. STITCHES REQUIRED.

EMPLOYEE WELDING INSIDE OF CHUTE WHEN ROCK FROM ABOVE BROKE LOOSE, FELL STRIKING EMPLOYEE.

EMPLOYEE WAS STRIKING A CRUSHER HAMMER WITH A SLEDGE HAMMER A PIECE OF THE CRUSHER HAMMER FLEW OFF STRIKING THE EMPLOYEE

THE EMPLOYEE WAS PLACING FRONT END LOADER PARTS IN THE BACK OF A PICKUP WHEN THE PARTS SLIPPED CATCHING THE EMPLOYEES FINGER AGINST THE PICKUP BED.

REMOVING NUT FROM BOLT, LOOSENED IT BY STRIKING WRENCH WITH HAMMER STRUCK TOOK IT RICOCHETED BASCK STRUCK EMP

EMPLOYEE WAS USING A HAMMER & PUNCH TO KNOCK UT SOME BOLTS THAT HAD BEEN CUT WITH A TORCH. AFTER STRIKING THE PUNCH WITH THE HAMMER EITHER A PIECE OF SLAG OR METAL FLEW FROM THE BOLT, BOUNCED OFF THE CRUSHER STRIKING THE EMPLOYEE.

EMPL WAS WALKING ACROSS CONCRETE SLAB AT WASH STATION WHILE PREPARING TO WASH FRONT END LOADER. HE STEPPED ON SLIK SPOT SLIPPED & FELL BACKWARDS ON SLAB.

TWO EMPLOYEES WERE LIFTING A SHAFT TO BE PLACED IN A SNUBBER PULLEY. INJURED EMPLOYEE BENT OVER TO LIFT HIS 2ND OF THE SHAFT, WHEN HE BEGAN TO LIFT HE FELT A SHARP PAIN.

EMPLOYEE STEPPED ON TOP OF A TAKE-UP PULLEY COUNTER WEIGHT TO FAST A CRANE HOOK. A CHAIN HOLDING THE COUNTER WEIGHT IN PLACE BROKE CAUSING THE WEIGHT AND EMPLOYEE TO FALL.

INJURED AND A C2 WORKER WERE INSTALLING A HYDRALIC CYLINDER WHEN HIS FINGER WAS CAUGHT BETWEEN THE CYLINDER AND FRAME OF A TRUCK.

EMPLOYEE WAS HELPING TO REPLACE BOLTS IN SCREEN SUPPORTS. HE PULLED ON A BOLT AND FELT SOMETHING POP IN HIS BACK. HE DID NOT GO TO THE DR UNTIL 10-17-84.

WE HAD CHANGED CENTER SECTION OF BOOM ON DROPBALL DRANE AS WE RAISED BOOM ONE OF THE BOOM STOPS BEGAN TO BEND IN AN ARC WE LOWERED BOOM KATMANN PUT A COME A LONG ON THE 2 IN STOP MANUEL HERRE REMOVED THE PIN HOLDING THE STOP TO THE BOOM BOJM STOP FELL ON HIS LEFT HAND AND SMASHED HIS LEFT INDEX FINGER AGAINST THE ANGLE IRON EDGE OF THE BOOM

...AGREED TO HELP UNLOAD CRANE WITH OTHER EMPL...MACHINE TIPPED TO REAR AND LIFTED FRONT SECTION OFF FROM TRAILER.EMPL EITHER JUMPED OR WAS THROWN FROM MACHINE LANDED ON FEET ON GROUND BELOW C RANE CATWALK-15\.RESULTED IN FRACTURE OF RIGHT FOOT.

PULLING ON 3/4 DRIVE RATCHET HANDLE TO LOOSEN FROZEN BOLT ON CONE CRUSHER.PULLED MUSCLE IN RIGHT ELBOW DURING PROCESS.

LARGER BOULDING CAUSED CRUSHER TO STOP. EMPLOYEE WENT INTO CRUSHER TO CLEAR BOULDER. IN THE PROCESS OF LIFTING THE BOULDER, THE ROTOR OF THE CRUSHER STARTED TO TURN, PINNING HIS RIGHT HAND BET WEEN THE BOULDER AND THE ROTOR.

MR CRUZ WAS REMOVING HIS RAIN SUIT FROM HIS CAR WHEN HE PULLED OUT THE SUIT THE ELASTIC SUSPENDER FOR THE PANTS HUNG ON THE DOOR AND THEN POPPED LOOSE STRIKING MR CRUZ IN RT EYE

EMPLOYEE WAS APPROACHING SCRUBBER TO PUT OIL IN OILERS HE SLIPPED ON WET CONCRET AND FELL HE EXTENDED HIS RIGHT ARM TO CATCH HIMSELF AND DISLOCATED HIS FINGER ON THE CONCRETE
