Climbing down off of M-8 left foot rolled inward on bottom step and then fell to floor.
The employee was changing a 440 volt starter. Turned power back on to check starter after replaced. Checked the on/off button and determined new starter worked. He did not shut power down again, elected to hook up motor leads and in the process came in contact with the relay which in affect pulled starter into position causing flash back injuring him.
Fell on an upright unopened tube of silicone.
Employee sanding using a wire brush on a drill and wind blew metal into eye.
At this time it appears the employee had a heart attack.

EE WAS OVERCOME BY CARBON MONOXIDE GAS, MAN BECAME LIGHT HEADED AND NAUSEATED, WAS TAKEN TO HOSPITAL AND GIVEN OXYGEN. WAS ABLE TO RETURN TO WORK THE SAME DAY BUT JOB FOREMAN WOULD NOT ALLOW H IM TO.
EE WAS TURNING SAND PUMP WITH A PIPE WRENCH. WHILE TURNING THE SHAFT GOT CAUGHT BETWEEN THE WRENCH AND THE PUMP BASE.
