Team member got dirt and a small cut on their left eyelid on 6/24/2024. By 6/26/2024, the eye was swollen and became painful. Team member sought medical care at a local eyecare center.

MINER WAS USING A PRY BAR TO LIFT UP THE LATCH OF A PINTLE HITCH. THE PRY BAR LOST PURCHASE ON THE LATCH WHILE THE MINER WAS PUSHING DOWN ON THE END OF THE BAR RESULTING IN THE LOSS OF BALANCE. MINER FELL FORWARD AND STRUCK THEIR FACE ON THE TOP SUPPORT BAR OF A MUDFLAP RESULTING IN THE DISLODGEMENT OF A DENTAL CROWN. THE UNDERLYING TOOTH WAS CHIPPED AND STILL AFFIXED TO THE CROWN.

LIFTING 8" PIECE OF STEEL CHANNEL TO REPLACE A LEG/SUPPORT BEING INSTALLED ON AN OVERLAND CONVEYOR. CARRIED ITEM APPROX 30' AND SET IT DOWN. MINER EXPERIENCED RIGHT TESTICULAR PAIN.

Team members were picking up a sand screw equipment off a trailer with a crane. One of the rear rigging legs was hung up on a bolt/stud. Team member went to correct the rigging. As EE's hand got near the chain it released tension, taking EE's hand with it towards the sand screw frame rail, pinning it between the chain and the frame. Tip of right index finger was fractured.

THE EMPLOYEE WAS RETURNING FROM THE FEEDER TO THE PIT WITH AN EMPTY SCRAPER. HE ENCOUNTERED A LOADED SCRAPER TRAVELING TO THE FEEDER ON THE WRONG SIDE OF THE HAUL ROAD. HE TOOK EVASIVE ACTION OFF THE ROAD TO THE LEFT. THE RESULT WAS A BOUNCING EFFECT FROM THE SCRAPER CAUSING A COMPRESSION OF VERTEBRAE TO THE LOWER BACK.
