Two employees were moving a Flammable Storage Cabinet that was to be scrapped. They were utilizing a fork truck to move the empty cabinet out of the plant and onto a pallet. One of the employees were operating a fork truck and set the cabinet onto the pallet and the other then slid the cabinet backwards and crushed their finger between the cabinet and concrete wall.

EE claims injury (lower back sprain) occurred on 7/4/24. EE reported injury to company on 7/9/24. EE requested to seek treatment on 7/11. On 7/11 EE was taken to Concentra and received work restrictions of, "No shoveling, may lift up to 50lbs constantly, and May push/pull up to 100lbs constantly". On 7/11 the injury became a MSHA reportable.

Incident was initially a no treatment, incident became reportable on 11/1 when EE requested to seek treatment and was taken to clinic where EE was prescribed a steroid patch and physical therapy. On 8/28, EE reported to supervisor while shoveling outside plant around 300 feeder, EE stepped on the blade of a garden hoe causing the handle of the tool to strike elbow.

Employee claims he heard a "pop" in his shoulder area while climbing ladder to tank 3. He reported a personal injury to his neck/L shoulder on 11/2/10 and then on 11/4/10 reported the injury as having occurred at work on 10/29/10. Employee was placed on restricted duty by physician from 11/2/10 until 11/8/10.

EE was operating the bagging machine. When a bag fell, he bent over to retrieve it and accidently bumped the hit switch which dropped the kickplate on his hands still resting on the machine. His fingers were caught under the kickplate. Open wounds on fingers of both hands and Rt middle finger fractured.

Employee was beating on the outside of a raw stone bin trying to dislodge the frozen material. He was using a sledge hammer and scaling bar which caused pain in his right hand. The initial report of this injury was made on 12-19-08, the injury did not become reportable until 2-5-09 when a prescription was filled.
