Brownwood Quarry Metal/Non-Metal

Employee was descending the ladder of the haul truck at the end of the shift. While climbing down, EE's right foot slipped off the bottom rung, causing EE to roll ankle on the ground. Employee was diagnosed with a right ankle sprain and given restrictions.

EE was putting up the pressure washer wand when it slipped out of hand. EE reacted by trying to catch it which placed the left hand over the nozzle while the handle of the wand hit the ground. When the handle hit the ground, the water left under pressure in the hose released which caused a laceration to EE's left, middle finger.

At about 12:55 AM, the entire plant lost power. The transformers at the welding shop were sparking and had caught fire. The transformers were on the ground and the remnants of the cross members were smoldering with an occasional, small flame. Once power was pulled the fire was extinguished.

EE was walking back into the breakroom when slipped on a full water bottle that was lying on the floor. EE attempted to catch own fall but the weight of the momentum caused EE to move forward into the path of the window. Causing a laceration to the knuckle of the right-hand pinky finger.

The company was notified on 7/14/2020 that EE was diagnosed with a hernia that requires surgery. The diagnosis stems back to when EE was cleaning in the shop (on 5/7/2020) and tried to pick up debris from underneath a metal table leg.

EE was priming 3" water pump, unscrewed the fill plug, and internal pressure caused water to spray up onto left arm and cheek.

EE was walking downstairs of haul truck after filling coolant reservoir on haul truck when left ankle rolled upon taking the first step.

EE was trying to dislodge oversize, a rock hit EE hand causing a fracture to rt hand middle finger.

The maintenance crew was installing new blow bars and rotor in the Universal 130/150. The rotor was out of balance due to the blow bars opposite of 5 & 6 were not yet installed. EE removed the blocking from the rotor, control was lost and the rotor rotated. EE tried to stop the rotor and got arm caught. This resulted in a laceration on each side of EE's forearm.

EE was checking hydraulic oil on excavator & fueling. When EE went to secure the access door, EE lost footing and fell 5 feet to the ground. EE left hand made first contact with the floor causing a wrist sprain.

Two employees were changing a bearing on a tail pulley. Employee was holding the bearing with both hands while the other employee was tapping on the bearing trying to loosen it from the shaft. The tail pulley assembly shifted/fell 2-3 inches and crushed the tip of employee's ring finger between the bearing and the frame.

EE was descending from haul truck. EE continued to try and descend with lunch box in one hand and the seat cushion in the other, while holding handrail. EE slipped on the last step and fell approximately 30" to the ground, landing on back side. EE didn't wish to see a doctor until the following Tuesday (10/18) EE was put on light duty and went on vacation effective 10/19/16.

Employee was traveling to work on personal motorcycle. After employee went through the plant entrance, ran over a slight hole in the pavement and lost control of motorcycle, injuring knee.

Employee was cleaning up spillage under the bypass chute. As he was stepping off the concrete slab to the ground he grabbed an angle iron brace that was connected to a support beam. His right 5th finger got caught between the brace and support beam, twisting it. He sought medical attention on November 2, 2015 and is currently on restricted duty.

Employee was using a cordless grinder to remove a rusted bolt from an idler when a piece of metal went under his face shield and safety glasses and into his eye. He sought medical attention on 10/5/15. The piece of metal was removed and he was released to full duty.

Employee was assisting an electrician with the installation of new wires from the motor control center to the plant control room when he felt a mild shock to his left hand. He sought medical attention on July 20, 2015 and was released to full duty. He returned to the clinic on August 5, 2015 and is currently losing time.

EE was sliding tire off LF wheel position in order to separate wheel components. EE proceeded use 1800 tool to separate wheel parts when he moved the tool for reposition the wheel parts slid from the tire and struck EE on the back side of the right leg.

Employee was standing to the side of a company vehicle with his back turned to the back hoe. The bucket of the back hoe hit the employee in the back, throwing him into the company vehicle. This resulted in a bruised back.

EE was clearing a "bridge" out of the VSI crusher using a bar and hammer. When EE struck the bar with the hammer, a piece of bar broke off and struck the EE in the left forearm. Piece was removed by Dr. and 7-8 stitches were administered.

FOOT FELL INTO OPEN SPACE OF WORKING PLATFORM

Employee was removing rock from discharge chute when a large rock from the top of the chute dislodged and struck his little finger on right hand.

Supervisor was cutting rubber material with a utility knife and cut through the rubber and into his leg causing the need for ten stitches to the middle of his left thigh.

Was driving haul truck and stopped and got out and picked up rock off of pit road.

New deck being installed. Employee caught thumb between new screen deck and angle iron brace.

Mounting tail pulley guard. Making new mounting holes and touched right fore arm against hot metal.

Working on VSI Crusher putting on a V-Belt when dirt got in left eye.

WHILE DRIVING THE WATER TRUCK HAND SLIPPED OFF STEERING WHEEL STRIKING THUMB ON THE LATCH ON THE PILOT WINDOW.

WHILE DRIVING R-50 STEERING HYDRAULIC HOSE BROKE AND SHIPPED STEERING WHEEL BACK TO RIGHT, INJURING RIGHT WRIST.

REMOVING A U-JOINT FROM A TRUCK AND HIT LEFT THUMB WITH A HAMMER.

HELPING IN PLANT WHEN EMPLOYEE DID NOT HAVE HIS HANDS CLEAR OF EQUIPMENT WHEN CRANE OPERATOR WASGIVEN THE SIGNAL TO MOVE. WHEN EQUIPMENT MOVED, EE GOT HANDS CAUGHT AND CUT THE TOP OF HIS LEFT LITTLE FINGER OFF.

ELECTRICAL POWER WENT OFF CAUSING PRIMARY CRUSHER DISCHARGE BELT TO CHOKE UP. EMPLOYEE WAS HELPING TURN THE ROTOR OFF THE CRUSHER IN ORDER TO CLEAN MATERIAL FROM BELT. HIS GLOVE WAS CAUGHT O N THE TIP OF THE BLOW BAR, PULLING HIS FINGER BETWEEN THE ROTOR AND THE WALL OF THE CRUSHER.
