Luminant mechanic was using an impact wrench to loosen a bolt overhead while working on a railcar and received a foreign object in their eye. Was evaluated off-site by medical professional to have object removed and received a prescription.

During the dragline outage, an employee was using a prybar to assist in the task of removing a fairlead sheave. During the task, the pry bar shifted upward and struck the employee in the face and neck area. The employee sustained two cracked teeth and placed on no work for two days.

Contract employee was leaving the house of the dragline via the staircase. Once EE reached the bottom step, EE lost EE's balance and began to fall to the dragline shoe below (25 inches). EE's momentum carried EE forward, where EE fell off the dragline shoe to the ground below (51 inches). The impact to the ground resulted in 2 fractured vertebrae and lacerations to EE's face.

On 1-19-26, a Lube truck operator stumbled on dozer tracks and fell to the ground sustaining injury to their knee. Operator was able to work up until 2-7-26, when MRI results were received making this a lost time injury.

Equipment operator got off the piece of equipment to move a tractor pan wheel out of the roadway that had come off the pan, and in the process of lifting the wheel, the employee felt and heard a pop in their left shoulder. The employee was taken to an occupational medical clinic in College Station, Texas and diagnosed with a torn bicep muscle.

Employee fell on a seat arm rest on the shift change bus injuring their left rib area. Doctor diagnosed as first aid injury with no restrictions on 11-11-24. On 11-21-24 employee contacted safety indicating was still having discomfort. Employee seen by medical professional on 11-22-24 and placed on no work beginning on 11-23-24 until follow-up appointment on 12-3-24.

Employee received a pinching contusion to right arm and had to seek medical attention at nearby hospital.

Hydraulic ram was placed on a lug mounted to the side of the dragline. The ram slid off the lug. The safety line attached to the ram caused it to swing and contact EE's left hand.

The employee was working in the warehouse rigging yard putting pins on a pallet. When EE rolled the pin into place, the pin moved faster than expected and smashed EE's left thumb in between another rigging pin.

Employee was operating a farm tractor when the engine began to overheat, the employee lowered the loader and shut the tractor down. While opening the hood, the employee stepped over the loader tilt linkage when employee's foot slipped causing left knee to strike the loader tilt linkage resulting in a laceration about 3 inches below the knee requiring stitches to close the wound.

Employee instinctively reached out to catch a coffee pot that had dropped while EE was in the process of cleaning it resulting in a laceration between the pinky and ring finger of EE's left hand. 3 stitches were required to close the wound, making this reportable to MSHA.

Employee stepped out of car in the employee parking lot prior to work shift and slipped on ice causing them to fall to the pavement. Employee reported soreness with hip, shoulder and bicep but did not request medical attention. Later on 4.8.21, employee started a series of physical therapy treatments which made this incident reportable to MSHA.

Employee was stepping up onto a dozer and felt a pop in their right knee. A few days later, the employee said that it was a pre-existing injury, but EE did re-aggravate knee while climbing onto the dozer at work. The employee was taken to the doctor on 2/23/21 and diagnosed with a meniscus tear.

Employee started to slip while carrying a load and grabbed a door jam to regain their stability. The employee heard a pop and felt immediate pain in their right arm and immediately reported the incident to their supervisor. The employee was taken to the ER and the physician diagnosed the employee with a right distal bicep tendon rupture that would require surgery.

Employee was standing by foreman's truck waiting for shift to begin and have temperature taken when they passed out for unknown reasons. When they passed out they fell to the ground and struck head on small gravel resulting in a laceration. EE received hospital medical treatment to correct a medication imbalance that caused them to pass out.

Installing Reeving Ropes on dragline drag drum. Reeving rope secured to pro-locks on drum and was being lowered by crane from above via nylon rigging. Pace of drum rotation became greater that pace limit of crane causing nylon rigging to break and reeving rope to fall awkwardly below striking employee at right shoulder and left ankle.

Two mechanics were putting a tire on a rim. As one mechanic would pry a portion of the tire bead over the rim with a tire tool, the other mechanic would pry another portion of the tire bead over the rim. When the second mechanic started prying on the tire bead, the first mechanics tire tool slipped and the end of the tire tool hit the first mechanic in the lower lip.

Employees were installing pins into the carrier for the drag drum on an 8750 dragline. During the hammer process the fingertips of the injured employee's right hand ring finger and little finger were caught between the pin and the hammer. The contact resulted in corrective surgery to repair the damage.

Employee had completed an installation of an hydraulic hose on an end dump when he felt a sharp pain on the right side of his abdomen. After eating lunch he discovered a knot in his abdomen. On 08/05/15 he was diagnosed with a work related umbilical hernia. He does plan on having surgery.

Employee was sampling at a coal pile, felt hot and dizzy. Went inside to A/C room for 45 minutes to cool off. Still dizzy, an ambulance was called and took him to emergency room. He was treated by the ER doctor with an IV and released with a one day restriction of no work in hot conditions. Employee was diagnosed with heat exhaustion. The temp was 100 degrees.
