No 1 Surface Coal

IT HAD BEEN RAINING AND THE SURFACE OF THE EQUIPMENT WAS WET.

EE was unhooking 1.5 cables from crane hook while holding the safety latch the back cable came off and hit him in the face busting top and bottom lip and chipping his front tooth

Mantrip had taken men to their equipment and parked up at about 6:15 am. EE was in his hoe and noticed steam or smoke about 6:30 from under the van and went to check on it. It was on fire so he called for help on the radio. Several other operators responded with fire extinguishers. Van was a total loss. No one was injured as a result of the fire.

While trying to remove a 6' water line from a Gorman Rupp pump, ee started to fall he let go of the hose. another ee who was helping ee also let go of the hose and it fell on ee left leg pinning it to the ground. ee received a broken fibula from the accident.

Employee was traveling down the 5 block on a decline of approximately 9 percent grade when he lost control of his truck ran off the road to the left up on the embankment and over turned it on its right side.

On 1/9/11 EE had just coupled the Add-Car Cathead and was placing into the Add-Car tray, when it snagged on his glove, causing it to land on his left hand fingers. EE completed his shift and then sought medical treatment for a broken middle finger, left hand.

EE was removing brake pack from Komatsu loader using a 1" drive impact wrench with a made up socket keeper (made from welding rod) while operating wrench the keeper caught his glove and cut his left hand index finger.

On 9/21/10 ee was in the process of installing a hydraulic bake steering pump in the CAT 992D loader and while winching into position the pump lodged and when it came free it struck and mashed ee right hand against the transmission mount. First aid was administered and ee transported to Summersville Regional Hsp for treatment. EE was treated and released to RTW.

EE was setting up tooling to press out the king pin on the Rock Truck and while hooking up the chocker cable to re-position the 100 ton ram sitting on the spindle the ram fell over and the cable hooked on his left thumb. First aid was adm. and EE was transported to hospital for treatment. EE was referred to a orthopedist by the ER Dr.

EE was walking to the launch and while going around the pan loader he rested his arm on the back upright. When he pulled away a sharp burr of metal cut his right forearm. Seven stitches were used to close the laceration.

EE was in the process of removing a sprocket with another mechanic. EE stepped back to observe the sprocket while the other EE struck it with a 8 lb sledgehammer. A piece of metal came off and struck EE in his right forearm. First aid was given and EE was transported to ER. Dr. referred EE to a surgeon.

An unintentional highwall fall occurred on 12/8/2009. The fall occurred at Robinson North / Blackberry. No injuries occurred from the fall. The fall severely damaged the #1 HWM Launch and 3 mechanic trucks.

On 10/22/09 EE was in the process of unloading D-11 tracks from the boom truck. EE was lifting with the boom one track when it came caught on the other track. While trying to swing it free the chain broke and EE jumped from the truck where he landed wrong and tried to catch himself and fell. EE retuned the truck and drove himself to Summersville Memorial Hospital for treatment.

On 10-21-09 EE was operating the maintenance truck boom/winch by remote while in the process of pulling belt through the highwall miner add-car frame. Co-worker was watching for hangups at the belt roll end. The steel belt roller that was used for the cable upward pull dislodged & struck him in the face. First Aid was administered and employee was transported to CAMC for treatment.

EE was in the process of manually pinning the blast shield and while co-worker was working the handle to align the holes, injured EE was pushing the pin and cut his left hand ring finger. 1st aid was administered and EE was transported to Summersville Memorial Hospital by the safety department for treatment.

EE was tramming around the sediment control ditch, when a loose rock released from the slope above the safety bench. The rock slid down the bank and hit the safety bench and continued down the slopes, striking the front end of the excavator's cab. 1st aid was administered at the scene and EE was transportd by Health Net to Charleston Area Medical Center.

Employee was removing wire cable from ripper on dozer, when sharp edge caused a laceration of the left hand.

On 4/2/2009 Employee was in the process of hooking up a 4' retrieval hook link and it slipped out of his hand and fell on his right hand middle finger mashing it against the launch pad. First Aid was administered and he was transported to Summersville hospital for treatment.

Employee was rolling out lead line. When he crossed over a berm, he twisted his ankle and fell. A rock then rolled onto the ankle. Employee went to local ER, diagnosed with contusion and abrasion.

On 2-27-09, employee had been performing work on an Add Car which included grinding a piece of brass. He felt something in his left eye a short time after completing the grinding. First aid was administered and flushing his eye was unsuccessful. He was transported to Summersville Memorial Hospital where he was treated (foreign material removed from left eye) and released.

On 2/23/09, EE was in the process of dismounting the D10R Dozer he was operating, when his foot slipped off the step, causing him to put strain on his right arm and shoulder. EE continued to work and did not seek medical treatment until 3/3/09 and again on 4/13/09. The first day EE missed was for pre-surgery testing on 7/27/09.

EE was attempting to break free a frozen highwall miner catherd on the add car by striking the cathead against the add car. In the process he smashed his right index finger, between the cathead and the add car. First aid was adminstered and he was transported to summersville memorial hospital for treatment.

Ee was removing a 1" steel cable from the path of the d11 Dozer and did not see that a couple of strands on the steel cable were broken. The weight of the cable caused the broken strands to penetrate his glove and cut the palm of his right hand. First aid was adminstered and he transported to hospital for treatment.

On 1/7/09 Ee was in the process of placing the last right rear jack pad on the drill deck to bring out. He had placed the jack pad on his leg and when he began to raise the jack pad up onto the drill deck it fell and caught his left hand ring finger between the jack pad and the handrail. First aid was adminstered and he was transported to hospital for treatment.

On September 28, 2008, employee was on top of the miner checking the center gear case when the grease hose started to reel up, catching the blast shield lever. This resulted in the blast shield lowering down on employee's head causing fractures.

Employee was stemming holes, twisted his back.

On 8/22/08 EE was pulling on the water line that was falling out of the car when the car dropped off the pad. His left foot was positioned under the car when the car dropped off the pad, crushing his left foot.

On August 15, 2008, Injury was dismounting the 992 G Loader to re-check the oil while the tire man was finishing up changing a front tire. He was walking down the steps when he twisted his ankle and felt a sharp burning pain in his ankle. He had 3 points of contact and did not fall down.

Employee was conducting pre-trip inspection when he strained his back lifting the hood.

While putting car on, EE was putting miner cable in trough of Addacar. Car dropped off pad and his right foot was smashed/crushed under car.

Employee was removing a rock from HWM and it fell and caught his index finger between the rock and metal on the car.

EE cut a tree. It fell on a tree that was previously cut when it had been logged. A stick from the tree on the ground flew up and punctured a hole in his shield on his hard hat and went into his eye.

Employee was stemming holes when he slipped on some loose rocks and twisted his back.

Employee was using sledge hammer to drive trailroller on conveyor chain. When hammer struck chain, piece of metal flew off hitting employee in lower lip.

On March 31, EE was pushing in shot and when he turned on air conditioner, particles came out and went into his eye. He was inside cab of dozer and didn't have eye protection on.

End loader caught on fire/tire exploded

EE was walking down launch on highwall miner, slipped on ice and fell to ground approximately 4 1/2 feet hitting left knee, then was transported to hospital.

Employee was checking bench when he slipped in ice/snow into a hole (not a drill hole) and injured his back.

On 12/22/07, EE was cleaning mud in the highwall miner pit when right front tire went in hole, causing operator to bounce back and forth. Note: 1/8/08, Nicholas Energy Safety Dept. requested official info from doctor, a neurologist in Charleston, WV, regarding injury. Info was recened on 1/21/08 and has been deemed as lost time accident, will reflect on 2007 NFDL Rates.

Employee was welding on D-11 cutting edge. The boom malfunctioned and raised cutting edge. A bolt fell out striking employee in the head.

On the above date, employee & co-worker were working on a door latch. When he was checking the latch, co-worker shut the door on employee's finger. He was taken to S.M.H. where x-rays on finger were negative. Doctor cleaned finger tip and applied a splint.

EE was replacing engine encloser. He was using a grinder and lost his grip on the grinder. When he grabbed for it, he started grinder back up. When it hit the floor, it bounced back striking his left forearm.

EE was putting a connection link in a conveyor chain on a Joy 1415 continuous miner head on a high wall miner. He struck the connection with a hammer. A piece of metal sheared off, cutting his left bicep on the inside.

Smoke was found coming off of the Stockton 450 pile. The smoke was discovered at approximately 11:00 am.

EE was getting a ride back to the parking area from the tire yard from Rt 772 coal truck. He was climbing up the right side ladder up to the passengers side of the truck when on the second step he slipped and fell approximately 6 ft to the ground landing on his left side.

EE was removing a differential housing from a Caterpillar 992G end loader. The housing fell, EE twisted his rt. ankle avoiding the falling housing.

EE was replacing sheet metal on D10 Dozer after servicing it. EE finished putting guards on left side & was going to right side of dozer. He stepped off dozer with rt foot & rolled ankle on a rock. EE heard a pop and felt shart pain in ankle.

Tank truck top loads from a fixed platform with a hinged ramp to the top of truck. Upon completion of loading, employee pulled the ramp up and gloved hand slipped causing him to fall back 2 feet striking the safety rail brusing low back.

The EE was assisting the driller with the removal of rods (drill steel) from the borehole. There was an ignition of methane (gas). The EE received 1st degree burns to his left arm, face and neck. The EE also received a 2nd degree burn on his left arm.

Contracting company M & W were drilling a hole into an abandoned mine for slurry ejection. While removing the drill steel from the hole, the drillers encountered methane and an ignition occurred. One driller sustained miner burns to his left arm.

On April 8, 2007, employee was getting tools out of tool box. Lid shut while he had his hand on top of tool box causing injury to his right ring finger.

Victim was cutting a tree approx 10" in diameter. The tip of the chainsaw bar hit a smaller tree behind the tree being cut, causing the saw to kick back, cutting the victim's leg.

Employee was checking antifreeze in truck. He started taking cup off of radiator and realized it was hot. He started putting cup back on when it blew off covering him with hot antifreeze.

EE was trying to change a table bushing on drill because it would not go down to allow the breakout wrench to clear it. EE began to pry up on the table bushing with a pry bar when he felt a sharp pain in his lower back.

EE stepped in a hole and fell sideways injuring his knee.

On 9/1/06, EE was using a sledge hammer to drive a center pin on an equalizer bar on #621 D11R. A small sliver of the pin broke off and entered EE's right arm.

EE was trying to remove drain plug to drain engine oil. plug was stuck, when it broke loose his carried him back where he hit his head on the bumper of truck.

Removing rock off of miner had hand placed on edge of pan, rock slide off of boom smashing finger.

Employee was getting off bus when his feet slipped. He was holding on with his arm and shoulder popped out of socket. Note: history of shoulder problems prior to working in mining industry. Employee has had multiple shoulder dislocations.

EE was helping load blast on drill bench. As he was walking he tripped over a rock causing him to fall to ground. Treated for cut at local ER received three stitches.

Employee was guiding a truck liner into a new 930E truck bed. The liner was being moved with a boom truck and employee was holding the plate while it was swinging. His hand caught under the plate and in between the truck bed. His left middle finger was pinched.

On April 25, 2006, EE was pulling into the shovel pit area and the truck shifted because of a dip in the terrain. The shift caused EE to twist his right wrist.

EE WAS WORKING ON A ROLLER MACHINE AND WAS CHECKING FUEL LINES WHEN THE HOOD FELL, STRIKING HIM ON THE HEAD. EE HAD A LACERATION ABOUT 2" LONG ON THE TOP OF HIS HEAD (4 STAPLES).

EMPLOYEE WAS INSTALLING CUTTING EDGES, WHILE CUTTING SPACER PLATE OUT OF MOLD BOARD TO CLEAN DIRT OUT FROM BEHIND IT. THE SPACER PLATE HAD PRESSURE ON IT WHEN HE CUT IT LOOSE IT SPRUNG OUT & HIT HIM ON HIS FOREHEAD.

EE was hauling rock down Fork Road in a Volvo 830C when the truck kicked out of gear. He couldn't get the jake brake to work and regular brakes wouldn't hold the truck on the steep grade. 3rd trip down the hill and everything was operable on the truck til the time of the accident.

Employee was raising the hook of a utility hoist with his left hand. The hook caught a lip on the side of the tool bed, smashing his finger and pulling his fingernail off.

Employee was cutting a zip-tie with his pocket knife when it slipped cutting his left hand. The cut required 4 stitches and he returned to work the next day. He was instructed to never use his pocket knife while cutting zip-ties and to use issued cutting pliers.

After completing his pre shift. He left the parking area heading for the robinson north coal pit. He noticed the driver side door was not shut. He open the doors and attempted to re-shut it. The truck came into contant with a rough patch in the road causing the door to slam shut on his right hand.

EMPLOYEE HAD REMOVED A HANDRAIL BLOCK AT FRONT BUMPER ON A 777B TRUCK FOR STEP REPAIR. WHILE CLEANING UP THE OLD WELD AND SLAG WITH A 4" GRINDER THE GRINDER HUNG UP. IT CAME AROUND CUTTING EE IN LEFT WRIST RIGHT ABOVE THE WRIST JOINT.

SMASHED FINGER WHILE UNLOADING METAL FROM SERVICE TRUCK.

EMPLOYEE WAS HITTING OVERHEAD BIN WITH 8 LB HAMMER. THE HAMMER BOUNCED OFF THE BIN AND STRUCK HIM IN CHIN. SAW ER PHYSICIAN REQUIRING SIX STITCHES

Injuries was installing a tooth on the 992 loader bucket when the rooth fell off bucket and landed on top of his left foot.

While clear cutting in a valley fill, employee's saw kicked back out of a tree striking his lower right leg behind his chaps. Employee was taken to E.R. for sutures.

Injured was tramming endloader to the parking lot when he encountered a rough place in the haul road. The bucket of the loader struck the ground causing pain in his lower back. Injured continued to work until 5/3/05.

AN UNINTENTIONAL FIRE WAS DISCOVERED ON THE STOCKTON COAL STOCKPILE AT 10:00 AM. THIS FIRE MEASURED 6' X 8' AND WAS BURNING AT OR NEAR THE SURFACE OF THE PILE.

Injured was helping a co-worker unhook cables from the no.1 car of the nighwall miner when the cable slack became caught on the cable shield and forced it closed on his right hand and fingers.

Injured was air-arching a hitch pin block on the back of a parts truck when a piece of metal flew behind his safety glasses striking him in the eye.

Operator exited cab to clear offside mirror. He slipped on a bolt laying on the deck. He tried to catch his balance and his left wrist struck the breather on the truck.

EE was cutting off bolts when acetylene hose punctured at back of torch and catching fire from where he was cutting. the flame set his clothes on fire burning his right side above waist in toward center of chest.

EE AND COWORKER WERE CHANGING RIGHT REAR TIRE ON SERVICE TRUCK. TIRE WAS MOUNTED AND EE WAS CHECKING THE GAUGE WHEN THE SIDEWALL EXPLODED KNOCKING HIM TO THE GROUND.

EE AND COWORKER WERE CHANGING RIGHT REAR TIRE ON SERVICE TRUCK. TIRE WAS MOUNTED AND EE WAS IN PROCESS OF AIRING. EE LEANED OVER TO CHECK PRESSURE AND THE SIDE WALL OF THE TIRE EXPLODED KNOCKING EE OFF BALANCE.

Employee was prying on a rock with a slate bar. As the rock rolled outward it hit the bottom safety cable, the rock broke and flipped up striking employee.

EMPLOYEE WAS IN THE PROCESS OF DISMOUNTING THE 403 IR DRILL WHEN HE SLIPPED CAUSING HIM TO CATCH HIS WEDDING BAND ON THE GRILL OF THE RADIATOR. DUE TO THE LOSS OF CIRCULATION, HIS FINGER WAS AMPUTATED.

Injured was working on the driver side truck door trying to repair the door latch. when a co worker shut the door on the injured right hand injuring his right middle finger.

While operating a dozer ee ran up on a rock causing him to be jarred around in the cab. EE has suffered pain in his upper back and neck. On Aug 3rd he was seen by a E.R. physician and referred to a specialist. He was seen by the specialist on Aug 25th. He is still waiting on the specialist report.

EMPLOYEE WENT TO PUT MIG GUN OVER SHOULDER & ACCIDENTALLY HIT MIG LINE & THE END OF LINE STRUCK HIS SHOULDER.

EMPLOYEE WAS GETTING OFF THE DOZER WHEN HIS FOOT SLIPPED ON THE DECK CAUSING HIM TO FALL ON THE DECK HITTING HIS RIGHT HIP.

INJURED WAS REMOVING A BROKEN ROLLER BOLT FROM A D11R DOZER WITH A CUTTING TORCH WHEN A HOT PIECE OF SLAG BURNED A HOLE THROUGH HIS GLOVE, CAUSING A BURN TO HIS HAND.

Employee was in the process of putting conveyor chain together while using a sledge hammer to force the links together. A piece of metal from the link flew off striking EE in the inner left thigh. EE has not missed any of his scheduled work at this time.

MECHANIC WAS MAKING REPIARS ON ENGINE. WHILE WALKING FROM THE TOOL BOX TO THE TRUCK, HE STEPPED INTO A HOLE AND TURNED HIS ANKLE. ER DOCTOR TREATED WITH SUPPORT BRACE AND RX.

CHANGING HOIST CYLINDER. EE WAS INSTALLING THE REAR RETAINING PLATE. THE RETAINING PLATE SLIPPED OUT OF HIS HAND AND STRUCK HIM ON HIS MOUTH.

FUELER FAILED TO PROPERLY PRE-TRIP UNIT AND HAD INADEQUATE BRAKES ON A GRADE. HE EASED THE UNIT INTO A MUDDY DITCH TO STOP. VEHICLE IMMEDIATELY INSPECTED AND FOUND TO HAVE NEW BRAKES THAT WERE OUT OF ADJUSTMENT. THE EMPLOYEE CONTINUED TO WORK BUT SOUGHT MEDICAL TREATMENT FOR ALLEGED INJURIES AFTER LEARNING OF PENDING DISCIPLINARY ACTION. THE COMPANY HAS BANNED HIM FROM THE PROPERTY

INJURED WAS PULLING ON A RACHET TRYING TO LOOSEN A DRAIN PLUG. HE REMAINED WORKING UNTIL 1-28-04 WHEN HE SAW A PHYSICIAN WHO INFORMED HIM THAT SURGERY WOULD BE NEEDED TO REPAIR A TORN TENDON. THIS WILL BE AN OFFICE SURGERY AND WILL BE PERFORMED ON HIS DAY OFF. HE WILL RETURN TO HIS REGULAR JOB ON HIS NEXT SCHEDULED SHIFT.

MACHINE WAS DOWN WITH A BROKEN TRACK AND THE RIPPER WAS BEING USED AGAINST A ROCK TO RAISE THE MACHINE UP OFF THE GROUND. WHEN TRYING TO TRACK UP MACHINE THE RIPPER SLIPPED OFF OF THE ROCK CAU SING THE TRACK MASTER LINK TO COME INTO CONTACT WITH THE EMPLOYEE'S FOREARM MAKING A DEEP CUT. THE MACHINE WAS NOT BEING RUN AT THE TIME.

WHILE TROUBLE-SHOOTING THE AIR CONDITIONING UNIT IN THE 988F PIZZA PAN EMPLOYEE WAS CHECKING THE WIRING TO THE BLOWER MOTORS. THE MOTORS STARTED CAUSING THE FAN BLADES TO SLICE THE TIPS OF THE THIRD AND FOURTH FINGERS ON HIS LEFT HAND.

EE WAS USING 2 WRENCHES TO LOOSEN A BELT ON THE IDLER CAP WHEN THE BOLT BROKE LOOSE, THE OPEN END OF THE WRENCH FELL AND HIT EE, STRIKING HIM ABOVE HIS EYEBROW.

EE STARTED SEEING A DR. ON ABOUT 8/7/2002 DUE TO HEAD, NECK AND SHOULDER PAIN ON THE RIGHT SIDE WHILE OPERATING A D-10 DOZER. EE STARTED MISSING WORK ON OR ABOUT 7/12/2003 DUE TO THIS ILLNESS. NO ACCIDENT REPORT WAS FILED WITH THE COMPANY FOR THIS ILLNESS. INFORMATION RECEIVED ON 9/2/2003.

EE CUT A TREE ABOUT 6 INCHES IN DIAMETER. THE TREE FELL ACROSS A TREE LYING ON THE GROUND. THE BUTT OF THE TREE HE CUT SPRUNG UP AND STRUCK HIM IN THE JAW.

WAS STARTING TO CLIMB UP LADDER WHEN I HAD A SHARP PAIN IN MY LOWER BACK.THE PAIN CONTINUED DOWN MY LEFT LEG. I WALKED OVER TO THE SHOP TO HAVE A CO-WORKER CALL MY FOREMAN.

EE WAS USING A RACHET TO REMOVE A BOLT FROM THE LEFT DRUM. THE ALLEN PLUG BROKE CAUSING HIM TO SLIP AND FALL AGAINST THE KNEE OF ANOTHER EE WHO WAS WALKING BY AT THE TIME.

INJURED WAS USING A SLEDGE HAMMER DRIVING A PIN IN A SET OF TRACK FOR A CATERPILLAR HOE WHEN A PIECE OF METAL CHIP OFF AND STRUCK HIM IN THE CHIN.

EE WAS STEMMING BLASTHOLES WITH DRILL CUTTINGS WHILE CHANGING SHOVEL FROM ONE HAND TO THE OTHERHE TWISTED BACK. TREATED AT LOCAL EMERGENCY FACILITY PRESCRIBED MEDICATION AND REST.

EE WAS TRYING TO SPIN A SET OF TRACKS BACK ON THE RIGHT SIDE OF THE DOZER WHEN A PIECE OF THE SPROCKET BROKE OFF AND STRUCK HIM IN THE STOMACH. HE WAS TAKEN TO SUMMERVILLE HOSPITAL WHERE THE P IECE OF METAL WAS REMOVED AND HE RETURNED TO WORK AT 3:00 PM.

EMPLOYEE WAS OPERATING THE ENDLOADER. AT 10:30 A.M. HE FELT A BURNING SENSATION IN HIS NECK. THE PAIN INCREASED THROUGHOUT THE SHIFT. HE LEFT WORK AT 3:00 P.M.

EE WAS CLIMBING AN UNSECURED LADDER THE INITIAL START-UP OF THE WORK SHIFT. EE AND OTHER COWORKERS WERE IN PROCESS OF GETTING OUT OF THEIR EQUIPMENT & SETTING UP IN ORDER TO WELD HOOKS ON THE CAB PROTECTOR TO SECURE THE LADDER. INJ EE CLIMBED THE LADDER TO GAIN ACCESS TO THE TOP OF THE CAB PROTECTOR & THE UNSECURED LADDER SLID OUT FROM UNDER HIM CAUSING HIM TO FALL.

MEMBER WAS SIGNALING FOR CO-WORKER TO STOP. HE RAISED HIS HAND AND HIS HAND STRUCK THE COVER ON THE OVERHEAD HEATER RESULTING IN A LACERATION.

EE WAS WORKING ON HIGHWALL MINER. THEY HAD PULLED OUT OF HOLE & WERE GOING BACK IN. EE WAS PUTTING CABLE BACK IN CAR & HIS RT. FOOT BECAME STUCK IN MUD. FOOT WAS THEN RUN OVER BY CAR.

EMPLOYEE WAS SHOVING A PIECE OF ROCK OFF THE CONTINUOUS MINER HEAD & THE ROCK FELL ON HIS RIGHT THUMB. THE LACERATION REQUIRED STITCHES.

EE SLIPPED OFF THE LADDER ON A HOE WHILE IN THE PROCESS OF FUELING SAME. HE REPORTED THAT HE HIT HIS CHIN AND STRAINED HIS BACK. THERE WAS NO LOST TIME AT THAT POINT AND HE CONTINUED TO WORK F OR 61 DAYS WITHOUT COMPLAINT. CO. HAS CAUSE TO DOUBT THE SOURCE OF HIS INJURY.

EE WAS DRIVING AN EXPLOSIVE TRUCK ON A ROUGH AREA OF THE HAUL ROAD. HE HIT A BUMP WHICH BOUNCED HIM IN THE SEAT. HE LATER REPORTED MINOR BACK PAIN. HE WENT TO HOSPITAL. X-RAYS WERE NEGATIVE & HE WAS RELEASED FROM DOCTOR'S CARE.

EE WAS STANDING BESIDE CO-WORKER. CO-WORKER STRUCK KEEPER WITH BALL PEEN HAMMER. A PIECE OF METAL FLEW UP AND STRUCK EE ON THE CHIN. (THE PIECE OF METAL WAS EMBEDDED IN CHIN) OBJECT WAS REMOVE D.

EE WAS ASSISTING IN REMOVING A PIN FROM HOIST CYLINDER ON 785 TRUCK. THE PIN CAME OUT ALL AT ONCE, BOUNCED OFF THE TIRE & LANDED ON TOP OF HIS RIGHT FOOT. PIN WEIGHED APPROX 35 LBS.

EE WAS INSTALLING A GEAR BOX ON THE DML DRILL. HE WAS POSITIONING THE GEAR BOX TO START THE BOLTS WHEN IT SLIPPED OUT OF HIS HANDS. WHEN HE REACHED AND CAUGHT IT HE FELT PAIN IN HIS BACK.

EE WAS STEAMING BLASTHOLE- HE FELT HIS LOWER BACK BEGIN TO HURT.

EE WAS REPAIRING CAUSTIC LINE TO THE 029 POND. CAUSTIC LIME BLEW LOOSE, SPILLING CHEMICAL ON EE. EE WAS WEARING GLASSES BUT ENOUGH CHEMICAL GOT AROUND GLASSES AND INTO EYES TO CAUSE DAMAGE.

WHILE WAITING TO FUEL, THE DRIVER WAS CUTTING (WHITTLING) A WOODEN STICK. THE KNIFE SLIPPED AND HE CUT HIS FINGERS.

EE WAS ATTEMPTING TO UNHOOK THE LIGHT PLANT FROM HIS TRUCK, WHEN HE LIFTED UP ON THE LIGHT PLANT TO GET IT LOOSE FROM THE BALL, THE LIGHT PLANT PUSHED FORWARD, MASHING HIS FINGER ON THE HITCH ON THE TRUCK.

EE WAS REMOVING CUTTING EDGE AND THE MOULD BOARD THAT THE CUTTING EDGE BOLTS ONTO ON THE BLADE HAD WORN. CONTACT WAS MADE AFTER PUSHING THE EDGE LOOSE.

USING A 3/4" DRIVE AIR IMPACT REMOVING BOLT, FINGER GOT TRAPPED BETWEEN GUN & METAL FRAME SMASHING FINGER. LOCATION: ALEX ENERGY, WV CLOSE TO WAREHOUSE NICHOLAS ENERGY SITE

ON 6/26/01 EE WAS DIGGING THE BINDER OFF THE COAL WHEN A SLAB OF BINDER BROKE FREE CAUSING THE LOADER BUCKET TO HIT THE HIGHWALL CAUSING PAIN IN LOWER PART OF HIS BACK. ATTACHED MEDICAL INFO RMATION SHOWS THIS AGGRAVATED A CHRONIC OR PRIORCONDITION. EE WAS PREVIOUSLY TREATED FOR LOW BACK IN 1994,1998, & 1999.

WHILE LOADING A METAL CHAMBER ONTO THE REAR BUMPER OF TRUCK IT SLIPPED OFF AND FELL ON EE'S LEFT FOOT. DIRT FELL BEHIND SAFETY GLASSES AND WENT INTO HIS RIGHT EYE.

EMPLOYEE WAS OPERATING A D-1OR DOZER. AS HE WASBACKING UP THE DOZER, HE RAN OVER A ROCK, CAUS- ING THE DOZER TO DROP. HE FELT PAIN IN HIS NECKSHOULDER AND LOWER BACK AREA.

EMPLOYEE'S TRUCK WAS BEING TOWED BY ANOTHER TRUCK. EMPLOYEE LOST CONTROL OF HIS TRUCK, HE THOUGHT IT MAY HIT THE OTHER TRUCK. HE JUMPED OUT ONTOTHE GROUND AND INJURED FOOT.

EE WAS REMOVING THE BRAKE DRUM OFF LEFT REAR AXLE OF GREASE TRUCK WHEN IT FOULED MASHING LEFT INDEX FINGER.

EE WAS TAKING TURNS WITH 2 OTHER EMPLOYEES SHOVELING DIRT OUT OF THE END OF A 8' PIPE THAT WAS TO BE USED AS A TUNNEL FOR A CONVEYOR BELT. THE DIRT WAS LEFT IN THE PIPE AFTER IT WAS DRAGGED TO THE LOCATION. ON ABOUT THE 2ND OR 3RD SHOVEL FULL, HE COPMPLAINED OF BACK PAIN.

DRIVER WAS STEPPING FROM THE STAR PLATFORM TO THE TRUCK WHEN HE MISJUDGED THE DISTANCE AND FELL STRIKING HIS RIGHT ARM ON THE GROUND FRACTURING SAME.

EMPLOYEE WAS GETTING OUT OF LUBE TRUCK AS HIS FEET TOUCHED SNOW COVERED ICE, BOTH FEET SLIPPED OUT FROM UNDER HIM. HE CONTINUED TO WORK UNTIL 2-21-2000.

EE WAS CHANGING A HOSE ON A 992G, EE WAS COMING OUT FROM UNDER MACHINE AND WALKED INTO A LADDER.

EE AND A COWORKER WERE CHANGING A TIRE ON A MACK TANDEM TRUCK. THEY PLACED THE TIRE ON THE TRUCK AND RELEASED THE JACK, WITH THE JOB FINISHED, AND AS EE WALKED AWAY, THE TIRE BLEW OFF THE RIM, STRUCK THE SIDE OF THE MECHANIC'S TRUCK. A PIECE OF THE RIM AND TIRE THEN STRUCK EE ON HIS BACKSIDE. STITCHES WERE REQUIRED TO REPAIR A LACERATION ON LEG.

OVERBURDEN SHOOTER WAS IN THE PROCESS OF LOADING BLAST HOLES WHEN HE SLIPPED AND FELL OVER HIGHWALL. EE FELL TOTAL OF 93 FEET.

EMPLOYEE CLAIMS THAT HE SLIPPED ON ICE WHILE FILLING HIS STEAM TRUCK WITH WATER. IT IS THE COMPANY'S POSITION THAT THIS DID NOT OCCUR IN THAT STEAM TRUCK WAS DOWN WAITING FOR PARTS ON THIS DAY

EMPLOYEE WAS IN THE PROCESS OF GETTING OFF HIS DRILL AND WALKING TO ANOTHER DRILL WHEN HE SLIPPED IN THE HEAVY SNOW THAT WAS COVERING THE GROUND. EMPLOYEE DID NOT MISS WORK.

EE WAS DISMOUNTING 85 TON ROCK TRUCK WHEN HIS FOOT SLIPPED & HE STRUCK THE LADDER WITH HIS RIGHTLEG.

EMPLOYEE WAS OPERATING A 200 TON ROCK TRUCK WHEN HE WAS JARRED CAUSING STRAIN TO HIS NECK.

EE DROPPED HYD LINE ON FINGERS ON RIGHT HAND.

EE STATES WHILE OPERATING A DOZER ON A FILL AREA HE PULLED A MUSCLE IN HIS CHEST WHEN PULLING ON THE STEERING CLUTCH AND RAN OVER A ROCK.

EMPLOYEE WAS IN THE PROCESS OF DRIVING A MECHANIC'S TRUCK (VERY SLOWLY) DOWN A SLIGHT GRADE AND WAS PREPARING TO MAKE A RIGHT TURN AT AN INTERSECTION WHEN A CO-WORKER OPERATING A 777 ROCK TRUC K STRUCK HIS VEHICLE FROM BEHIND. THE INJURED SUSTAINED A WHIPLASH TYPE INJURY. THE ACCIDENT WAS CAUSED BY POOR VISIBILITY DUE TO FOG AND ROAD CONDITIONS.

EE HAD JUST ARRIVED ON PROPERTY, REPORTING TO WORK. WHILE EXITING THE VEHICLE, HE INADVERTENTLY CLOSED THE DOOR ON HIS FINGER. HE SUSTAINED A FRACTURE TO HIS INDEX FINGER.

EMPLOYEE WAS IN PROCESS OF LIFTING A 30 POUND JACK PIN OFF OF A LOADER WHEN HE FELT PAIN IN HIS LOWER BACK.

INJURED WAS IN PROCESS OF PULLING ON A 90 WEIGHT GREASE HOSE WHEN HE FELT SOMETHING PULL IN HIS BACK. HE SUSTAINED A STRAIN. NOTE: INJURED NOT START MISSING WORK UNTIL 4-10-95.

THE INJURED WAS IN PROCESS OF USING A TORCH AND WAS CUTTING ALONG A GREASE BLOCK ON A HIGHWALL FRILL. APPARENTLY THE GREASE WITHIN THE BLOCK GOT EXTREMELY HOT AND BLEW OUT A FITTING OR THE BLO CK ITSELF, AND THE FLAME IGNITED THE GREASE, BLOWING BACK IN HIS FACE. HE SUSTAINED MOSTLY FIRST DEGREE BURNS ON HIS FACE AND NECK.

INJURED WAS IN PROCESS OF OPERATING A GRINDER FROM A "BUCKET TRUCK" WHEN HE LOST HIS BALANCE AND WHEN HE TRIED TO CATCH HIMSELF, THE GRINDER STRUCK HIS HAND CAUSING A LACERATION.

INJURED WAS IN PROCESS OF HELPING LIFT A TRANSMISSION OIL COOLER FROM UNDERNEATH A 785 KOMATSU TRUCK WHEN HE STRAINED HIS NECK.

EE WAS IN PROCESS OF LIFTING/PULLING ON A BRAKE DISC WHEN HE FELT PAIN IN HIS LOWER BACK. HE WAS WORKING ON A LOADER IN THE MINER SHOP.

EE WAS HELPING LIFT A TRAM MOTOR FOR THE COAL HOPPER WHEN HE STRAINED HIS BACK--HIGHWALL MINER.

EE HAD COMPLETED STEAM CLEANING A TRUCK AND AS HE WAS DISMOUNTING, HE SLIPPED ON A STEP AND FELL TO THE GROUND.

INJURED WAS IN PROCESS OF SPRAYING OFF A DRILL WHEN SOME MATERIAL FLEW BACK IN HIS EYE. A SMALL RUST FRAGMENT WA SMBEDDED.

THE INJURED WAS IN PROCESS OF CUTTING A DUST RUBBER FOR A DRILL MACHINE WHEN HIS KNIFE SLIPPED CUTTING THE PALM OF HIS HAND.

THE INJURED WA SIN PROCESS OF LOADING MATERIAL WITH THE 285 SHOVEL WHEN A LARGE ROCK FELL FORM TEH HIGHWALL STRIKING THE RIGHT FRONT TRACK CASUING HIM TO GET JARRED AND BOUNCED WITHIN THE CAB COMPARTMENT.

THE INJURED SUSTAINED AN EYE INJURY WHILE HE WAS USING AN AIR WRENCH AND SOME MATERIAL FLEW UP AND WENT UNDERNEATH HIS SAFETY GLASSES. THE OBJECT COULD NOT BE FLUSHED OUT.

INJURED WAS IN PROCESS OF CLOSING THE DOOR ON A DRILL WHEN THE DOOR HANDLE CAME OFF CAUSING HIM TO LOOSE HIS BALANCE AND THE DOOR CLOSED ON HIS FINGER. HIS FINGER TIP WAS MASHED.

WHILE PRYING AND USING A SLATE BAR, THE BAR SLIPPED STRIKIGN AND CUTTING THE INJURED ON HIS LEFT EYE LID.

INJURED WAS IN PROCESS OF STRAIGHTENING A TAILGATE ON A COAL TRUCK WHEN HE MASHED HIS THUMB HE SUSTAINED A CUT & BROKEN BONE

AFTER SPRAYING AN ELECTRICAL CLEANER ON A SWITCH THE EE BECAME SICK WITH DIZZINESS, HEADACHE AND NAUSEA. IT WAS NOT KNOWN UNTIL SEVERAL WEEKS LATER THAT HIS CONDITION WAS CAUSED BY BREATHING V APORS.

INJURED WAS IN PROCESS OF WORKING ON THE CAROUSEL LATCH OF A DRILL WHEN IT POPPED LOOSE STRIKING AND CUTTING HIS THUMB.

EE WAS IN PROCESS OF CHECKING OUT WORK AREA ATA A "DRILLED" AREA, WHEN HE STEPPED IN A DRILL HOLE THAT WAS COVERED BY WATER. HE SUSTAINED A BRUISED AND CUT LEG.

AFTER MOUNTING HIS TRUCK, THE INJURED OPENED THE DOOR AND WAS IN PROCESS OF ENTERING HIS CAB WHEN HIS FOOT SLIPPED ON A SMALL AMOUNT OF LOOSE COAL CAUSING HIM TO SLIP AND FALL. HE SUSTAINED A PULLED LIGAMENT IN HIS LEFT KNEE.

INJURED WAS IN THE PROCESS OF LIFTING A CRIB BLOCK WHEN HE FELT SOMETHING "PULL" IN HIS LOWER BACK AREA. NOTE: INJURED DID NOT START MISSING WORK UNTIL 6-6-94.

THE INJURED WAS IN PROCESS OF CUTTING A PLASTIC TIE WIRE WHEN THE KNIFE HE WAS USING SLIPPED CUTTING HIM ON THE ARM.

INJURED WAS IN PROCESS OF HELPING INSTALL A LOADER BUCKET TOOTH WHEN HIS CO-WORKER HIT HIS HAND WITH A SLEDGE HAMMER.

WHILE DUMPING COAL AT THE CLEAN COAL LOADOUT AND WITH THE BED OF THE COAL TRUCK ABOUT 1/4 THE WAY UP A HYDRAULIC HOSE BURST CAUSING THE BED OF THE TRUCK TO FALL ALL THE WAY BACK DOWN ON THE FR AME. EE WAS JOLTED AND JARRED AND SUSTAINED A NECK AND BACK SPRAIN.

THE INJURED WAS IN PROCESS OF PUMPING HYDRAULIC OIL IN THE GOB TRUCK WHEN A SECTION OF MUD OR DEBRIS FELL OFF THE SIDE OF THE TRUCK STRIKING & BRUISING HIS SHOULDER AREA.

AFTER DUMPING A LOAD OF COAL ON THE GROUND AT THE STOCKPILE THE BED OFTHE DARTCOAL TRUCK CAME DOWN HARD JARRING THE INJURED THUS CAUSING HIS NECK & BACK TO JERK HE SUSTAINED A CERVICAL STRAIN NOTE THE INJURED CONTINUED TO WORK AFTER THIS ACCIDENT & DID NOT START MISSING WORK UNTIL 03-21-94

INJURED WAS IN PROCESS FO LIFTING A TABLE BUSHING ON A DRILL WHEN HE FELT SOMETHING PULL IN SHOULDER AREA. NOTE: INJURED DID NOT START MISSING OWRK UNTIL 3-2-94.

INJURED SUSTAINED A FRACTURED HAND WHEN HIS HAND GOT CAUGHT/TWISTED IN THE GREASE HOSE HANDLE. AFTER GREASING HIS TRUCK THE INJURED PULLED ON THE GREASE HOSE IN ORDER TO ENGAGE THE HOSE REEL APPARENTLY THE REEL TOOK UP THE SLACK ABRUPTLY &WHEN HE ATTEMPTED TO STOP THE HOSE HIS HAND WAS INJURED.

INJURED WAS IN PROCESS OF HANDLING A LARGE ROCK WITH HIS LOADERS BUCKET WHEN THE ROCK FELL OVER CAUSING HIM TO SUSTAIN A STRAINED BACK WHEN THE MACHINE WAS JOLTED

INJURED WAS IN PROCESS OF OPERATING A GRADER AND WHILE SCRAPING A ROADWAY, THE BLADE "CAUGHT" A ROCK AND CAUSED HIS NECK TO JERK. HE SUSTAINED A STRAIN TO THE NECK AREA.

INJURED WAS IN PROCESS OF ADJUSTING THE GOVERNOR ON A WELDER WHEN HIS WRENCH SLIPPED & HIS FINGER STRUCK THE FAN CAUSING A LACERATION.

WHILE HELPING THE POWDER CREW AND WHILE HOLES WERE BEING LOADED, A SPOOL OF PRIMER CARD GOT JERKED OFF THE POWDER TRUCK AND STRUCK AND CUT THE INJURED'S FOREARM. AFTER A FEW DAYS, THE WOUND GO T INFECTED.

INJURED WAS OPERATING A BULLDOZER ND WHILE BACKING UP, HE RAN INTO A LARGE ROCK CAUSING HIM TO GET JARRED AND BOUNCED. HE SUSTAINED A STOVED NECK.

WHILE THE INJURED'S TRUCK WAS BEING LOADED, A BELL CRANK FELL OUT OF THE ENDLOADER'S BUCKET CAUSING THE BUCKET AND MATERIAL TO HIT THE TRUCK BED HARD. HE RE-INJURED HIS BACK AND NECK.

INJURED WAS WALKING NEAR HIS POWDER TRUCK WHEN HIS FEET SLIPPED CAUSING HIS BACK TO JERK. HE RECEIVED A STRAINED BACK.

INJURED WAS IN PROCESS OF CLEANING OFF HIS WINDOW ON HIS 992 LOADER, WHEN HIS FOOT SLIPPED , CAUSING HIM TO CATCH HIMSELF IN ORDER NOT TO FALL. HE RECEIVED A STRAINED BACK.

THE INJURED WAS IN PROCESS OF WORKING UNDERNEATH A GREASE TRUCK, PERFORMING EXHAUST SYSTEM REPAIR. AS HE WAS SCOOTING ALONG THE FLOOR ON A CREEPER HIS BACK STRUCK A BRACE CAUSING A CONTUSION.

INJURED WAS IN PROCESS OF HAVING HIS ROCK TRUCK LOADED WHEN A LARGE ROCK SLID OFF THE BUCKET OF THE LOADER STRIKING THE SIDE OF INJURED TRUCK BED HE RECEIVED A STRAINED DUE TO THE BOUNCED OF T HE TRUCK.

INJURED WAS IN PROCESS OF CHECKING A STARTER IN THE ENGINE COMPARTMENT IN THE HYDRAULIC SHOVEL, WHEN HIS FOOT SLIPPED OFF OF THE LADDER CAUSING HIM TO STRADDLE A RUNG, INJURING HIMSELF IN THE SCOTUM AREA.
